This is the first study to report one-year gene therapy safety and efficacy results in treating young adults with Leber Congenital Amaurosis (LCA), a hereditary condition that causes severe vision impairment in infants and children. Previously, such improvement in vision after was observed within weeks of undergoing gene therapy. The three patients - 22, 24 and 25 years old- who received the gene therapy, have a specific type of LCA caused by a genetic mutation in the RPE65 gene. The gene normally makes a critical protein in the visual cycle. Without this RPE65 protein, light-sensitive photoreceptor cells are starved of a retina-specific form of vitamin A and cannot function, blocking vision. For correcting this genetic defect, researchers targeted retinal regions with impaired, but intact, photoreceptors and injected healthy copies of the RPE65 gene under the retina. One patient was able to read an illuminated clock for the first time, and the new ability was not caused by a further increase in light sensitivity, which remained unchanged from 1 to 12 months after the treatment. One year after the single injection, the healthy genes continue to make this critical protein, increasing the retina's sensitivity to light. We had previously shown that RPE65 gene therapy can completely reverse one of the two components of this complex disease and provide patients with increased day and night vision within weeks. We now show that the longevity of the visual improvements extends to at least one year, said Dr. Artur V. Cideciyan, Research Associate Professor of Ophthalmology at the University of Pennsylvania School of Medicine and lead author of the publications. But the visual improvement was caused by a slow change in the direction of focus to her treated retina. The change appeared 12 months after gene therapy, but not before. The findings are published in Human Gene Therapy, now online, and in the New England Journal of Medicine (NEJM). Gene therapy has been rhapsodized and vilified in its nearly two decades of human testing, helping some and making others sicker. But a new 12-month clinical trial has shown that, at least in one ocular disease, it appears safe and-perhaps even more impressive-effective. The research, part of a phase I clinical trial to test the safety of the treatment, was published as a letter to the editor in The New England Journal of Medicine earlier this week and will be in the September issue of Human Gene Therapy. (The paper was co-authored by about a dozen researchers, two of whom own equity in a company that could profit from a commercialized version of this procedure.) The researchers report that three young adults with severe vision impairment from a hereditary disease maintained improved eyesight a year after gene therapy was administered-and didn't suffer any health side effects in the meantime. Gene therapy, which often employs viruses to deliver the good genes to a body's target cells, has been known to trigger severe immune responses and was blamed for the death of an 18-year-old in 1999, who was receiving gene therapy for a hereditary metabolic disorder. The test subjects suffer from Leber congenital amaurosis (LCA), a form of hereditary retinal degeneration that occurs in infants and young children and is relatively rare. Most people who have lost vision due to hereditary retinal degeneration have either no photoreceptors with which to perceive light or photoreceptors that don't work. This disease has a little bit of both, explains lead study author Artur Cideciyan, an associate research professor at the Scheie Eye Institute at the University of Pennsylvania. It's a complex disease. The participants, ages 22 to 25, all had a mutation on the RPE65 gene, the signature of LCA. Doctors administered normal RPE65 (delivered via an rAAV2 vector) into a site on one of the retinas that had the most photoreceptors left. And, Cideciyan says, Within weeks of the procedure there was a substantial, very significant improvement in sensitivity to see dim lights. Those in the study weren't entirely blind before the therapy. In fact, Cideciyan notes, some of them can read, but by that he means they can see the 'E' at the top of a standard vision chart. That does not represent the visual inability that they have because they also have a large loss of sensitivity of light. So they may be able to read on a high-contrast chart, but they are further disabled in less light environments. What has been more surprising to the researchers is not that the improvements have held for the year-that much had already been documented in animal studies-but that, at least in one of the patients, the therapy helped train the eye to see better. It can completely bypass the dysfunction so it can give as much vision as there are retained receptors. One patient reported that she was newly able, about a year after the treatment, to see the digital clock in the family car. Cideciyan attributes that not to continued improvement of the photoreceptors themselves but to the retraining of the visual system to take advantage of the treated areas of the retina. Despite having a checkered past, gene therapy seemed like a promising option for many patients with this disease. There were more volunteers than could be accommodated in this trial, Cideciyan noted. But the treatment isn't out of the woods yet. The research team will continue the study for a full three years, as is required by the U.S. Food and Drug Administration (FDA) for a phase I clinical trial. And the participants will be monitored for 15 years for health impacts, as is required by the FDA for any gene-therapy trials. Following the end of the first trial, there will be two more-to test the treatment's efficacy-before it can be approved for commercial use on the market. Cideciyan admits that the encouraging results are only for a rare condition, but he hopes that with continued research, it will have huge potential for much more

Previously, such improvement in vision after was observed within weeks of undergoing gene therapy. The three patients - 22, 24 and 25 years old- who received the gene therapy, have a specific type of LCA caused by a genetic mutation in the RPE65 gene. The gene normally makes a critical protein in the visual cycle. Without this RPE65 protein, light-sensitive photoreceptor cells are starved of a retina-specific form of vitamin A and cannot function, blocking vision. For correcting this genetic defect, researchers targeted retinal regions with impaired, but intact, photoreceptors and injected healthy copies of the RPE65 gene under the retina. One patient was able to read an illuminated clock for the first time, and the new ability was not caused by a further increase in light sensitivity, which remained unchanged from 1 to 12 months after the treatment. One year after the single injection, the healthy genes continue to make this critical protein, increasing the retina's sensitivity to light. We had previously shown that RPE65 gene therapy can completely reverse one of the two components of this complex disease and provide patients with increased day and night vision within weeks. We now show that the longevity of the visual improvements extends to at least one year, said Dr. Artur V. Cideciyan, Research Associate Professor of Ophthalmology at the University of Pennsylvania School of Medicine and lead author of the publications. But the visual improvement was caused by a slow change in the direction of focus to her treated retina. The change appeared 12 months after gene therapy, but not before. But don't let the hysterical anchors get under your skin and start wearing a mask each time you step out of the house, unless you are visiting a very crowded area. Then too, the mask will protect you only for a specified period. Without giving in to the swine flu panic and creating a stockpile of Tamiflu and N-95 masks at home and enriching pharma companies, there are a number of other measures you can take to ensure that the virus is not able to get you, irrespective of which part of the world you are in. It is essential to remember that all kinds of viruses and bacteria can attack you when your immune system is weak, or they can weaken it easily. Hence, building your own defences would be a better, more practical, long-lasting and much more economical idea. Here are some easy steps you can take to tackle a flu virus of any kind, including swine flu. It is not necessary to follow all the steps at once. You can pick and choose a combination of remedies that suit you best. However, if you are already suffering from flu, these measures can help only up to an extent. And, if you have been infected by H1N1, visiting a hospital and staying in solitary confinement is a must. 1. Have five duly washed leaves of Tulsi (known as Basil in English; medicinal name Ocimum sanctum) everyday in the morning. Tulsi has a large number of therapeutic properties. It keeps throat and lungs clear and helps in infections by way of strengthening your immunity. 2. Giloi (medicinal name Tinospora cordifolia) is a commonly available plant in many areas. Take a one-foot long branch of giloi, add five to six leaves of Tulsi and boil in water for 15-20 minutes or long enough to allow the water to extract its properties. Add black pepper and sendha (salt used during religious fasts), rock or black salt, or Misri (crystalised sugar like lumps to make it sweet) according to taste. Let it cool a bit and drink this kadha (concoction) while still warm. It will work wonders for your immunity. If giloi plant is not available, get processed giloi powder from Hamdard or others, and concoct a similar drink once a day. 3. A small piece of camphor (kapoor) approximately the size of a tablet should be taken once or twice a month. It can be swallowed with water by adults while children can take it along with mashed potatoes or banana because they will find it difficult to have it without any aides. Please remember camphor is not to be taken everyday, but only once each season, or once a month. 4. Those who can take garlic, must have two pods of raw garlic first thing in the morning. To be swallowed daily with lukewarm water. Garlic too strengthens immunity like the earlier measures mentioned. 5. Those not allergic to milk, must take a glass of hot or lukewarm milk every night with a small measure of haldi (turmeric). 6. Aloe vera (gwarpatha) too is a commonly available plant. Its thick and long, cactus-like leaves have an odourless gel. A teaspoon gel taken with water daily can work wonders for not only your skin and joint pains, but also boost immunity. 7. Take homeopathic medicines - Pyrogenium 200 and Inflenzium 200 in particular - five tablets three times a day, or two-three drops three times a day. While these are not specifically targeted at H1N1 either, these work well as preventive against common flu virus. 8. Do Pranayam daily (preferably under guidance if you are already not initiated into it) and go for morning jog/walk regularly to keep your throat and lungs in good condition and body in fine fettle. Even in small measures, it will work wonders for your body's resistance against all such diseases which attack the nose, throat and lungs, besides keeping you fit. 9. Have citrus fruits, particularly Vitamin C rich Amla (Indian gooseberry) juice. Since fresh Amla is not yet available in the market (not for another three to four months), it is not a bad idea to buy packaged Amla juice which is commonly available nowadays. 10. Last but not the least, wash your hands frequently every day with soap and warm water for 15-20 seconds; especially before meals, or each time after touching a surface that you suspect could be contaminated with flu virus such as a door handle or a knob/handle, especially if you have returned from a public place or used public transport.
