Dear George,
I understand that while writing to the government it is important to give 
specific examples to illustrate the lack of accessibility of the system. 
However, it must be mentioned that the points mentioned are only examples and 
the remedy is full compliance to the guidelines with third party testing and 
certification. 
We want accessibility for everyone - the totally blind, low vision, cognitive 
disabilities, mobility impaired - one arm, no fingers and so on.  there is no 
way to test for everyone and get their feedback. Compliance to the 
international guidelines is the best way forward. 
Many departments are certifying their websites themselves but they are not at 
all compliant to the guidelines.

I have tried IRCTC.co.In on Mozilla with NVDA and Chrome with Jaws and
NVDA. The problems are same.
1.  Many times login link or Button is not recognized by screen reader.
2.  Even if we click on login link User name and password field are not
recognized by screen readers.
3.  If we locate Edit fields with OCR, focus can’t be taken to that fields.
4.  After selecting OTP option, we don’t receive OTP.
