I can confirm your findings. I installed the Eng US langpack on top of my 
Swedish 
OOo 3.3RC5 install. After switching the UI language to English the Toolbar and 
menu appear as expected. When I switch back to Swedish they disappear. 
I think that a default fall back to English is to be preferred. In that way the 
extensions show up as expected and there are a lot of extensions that are not 
translated to that many languages. The problem must be present in other 
localisations than Swedish too.

Have found the true reason for problems (1) and (2) now.  In both cases
(gdocs_2.3.0.oxt and Linguist-1.4.oxt) the contained Addons.xcu files contain
localized properties that contain property values for some specific xml:lang
attribute values as well as for the empty, default xml:lang attribute value
(which can be denoted by completely leaving out the xml:lang="..." attribute),

If the OOo UI locale ("Tools - Options... - Language Settings - Languages -
Language of - User interface") matches one of the given xml:lang attribute
values (one of en-US, en, da, fr in the above example), the corresponding
property value will be used.  However, if the locale does not match, the empty
default xml:lang attribute value will be used instead (leading to an empty
property value, which then causes the problem of not displaying menu entries),
even taking priority over an "en-US" fallback.  (This has probably changed
slightly from OOo 3.2, which may have preferred the "en-US" fallback over the
empty locale fallback.)

I will change it in OOo2GD [gdocs_2.3.0.oxt] but I think that such changes like 
this one which cause problems with extensions are dangerous.
For me it looks that to keep consistent with document created 4 years ago 
OO.org 
breaks de facto API. If it worked by last 4 years, and didn't cause any 
security 
issues it should stay in API, especial that here are extensions which base on 
this behavior (so defaulting to "en").

And I think I know source of a problem with  :-) it is NetBeans plugin 
for creating extensions.

I understand. It means that several extensions need to get an overhaul before 
they work in non-translated languages. I think this will take a while and some 
extensions are abandoned. There are most certainly more extensions not working 
properly than the few I have tested. I believe this will upset some users who 
can't find extensions they had installed in OOo 3.2. 
I have no idea if it is possible to change back in how OOo handle extensions, 
but 
if the extensions work, despite not being written as they should be, I think 
that 
would be a better (temporary?) solution. 
N.B. I am not a programmer and can't judge the amount of work and if such a 
change is possible to do in a RC.

I have upgraded to RC5 now and the extensions still fail to show up. I have 
installed from the Swedish deb x86_64 installation file in Ubuntu Linux and the 
Swedish .exe installation file in Win 7. 
If the behavior is due to the Swedish localisation it can explain that I have 
seen the problem reported in some Swedish foras, but nowhere else. 
later today I can try to install the English US version and watch the result. I 
can then try to add the Swedish lang pack on top and see how it works. It will 
be 
a while before I return since I have some work to do right now.

Sorry for the noise, but points (1) and (2) in <#desc7> are not correct.  Both
gdocs_2.3.0.oxt and Linguist-1.4.oxt install fine for me in OOO330_m13
unxlngi6.pro (as well as OOO330_m15 unxlngi6.pro) via "Tools - Extension
Manager... - Add... - Only for me" and do display their respective UI elements
("Add-On 1" floating window and "File - Google Docs & Zoho" sub-menu for
gdocs_2.3.0.oxt, "Linguist" top-level menu for Linguist-1.4.oxt) as soon as a
new (text) document window is opened.  (The extensions both do contain invalid
Addons.xcu content, but that specific invalid content is simply ignored by the
OOo 3.3 implementation, just as it was ignored by OOo 3.2, so the debug=x output
presented in <#desc7> was a red herring.)

@adlerbeth,raybanz:  Maybe this, too, has to do with specific locales.  What is
the locale that your OOo installations display their UI in?

Indeed, I use Swedish localisation of OOo. Somewhat strange that the extension

unxlngi6.pro nor OOO330_m13 unxlngi6.pro.  My OOo's locale is English and the
extension's texts are indeed displayed in English.  What I could imagine is that
your OOo's locale is not English, but some language that is not covered by the
xml:lang attributes in BookmarksMenu-0.6.2.oxt (en, cs, da, de, fr, ja).  That
could well cause OOo 3.2 to fall back to texts in "en" and OOo 3.3 to fall back
to texts in some other of the available locales, probably "da."  Issue 114155
("configmgr: fall back to 'en' locale") fixes that for OOo 3.4.

BookMarks menu installs in Polish (I think, I don't understand Polish). English 
should be the default in non-polish installs of OOo. The extensions installs in 
English by default in OOo 3.2.1 and earlier.

I'm experiencing the same problem. Using OOo 3.3RC2 and Windows XP SP3. In OOo 
3.2.1 everything worked as expected.

Further testing in windows 7 64 bit with OOo 3.3RC3: It is possible to install 
the extensions, but there are no toolbar or menu after restarting OOo. the 
behavior is the same as in Ubuntu. 

I rolled back to OOo 3.2. The toolbar and menu show up after installation of 
the extensions. The extensions work as expected. 

I un-installed OOo 3.2 and installed OOo 3.3RC3 with the same userfolder. All 
extensions carried over as expected, but the toolbar and menu for Googledocs 
and Linguist were no longer present. Other extensions function as expected. 

I can't se any apparent pattern which extensions work and which don't. there 
must be differences between how OOo 3.2 and 3.3RC handle extensions. 

I think this bug is important. It is not possible to use several popular 
extensions anymore. I hope my tests are useful for you!

The extensions are shown as installed under Tools - Extensions, but the toolbar/
menu never show up. The appearance of toolbar/menu is the expected behaviour. 
The 
installation worked that way in OOo 3.2.1 and earlier. 
there is a bugreport on the homepage for the Googledoc extension with the same 
behaviour with no toolbar as I report here.

I checked on Windows. unopkg list shows that the extensions are properly
installed. The gdocs extension has a toolbar and the linuguist extension has a
menu. 

@adlerbeth: Is this the expected behavior?

It is not possible to install several extensions in OOo 3.3RC3/OOo330m13 build 
9539. I have failed to install GoogleDocs: http://
extensions.services.openoffice.org/fr/project/ooo2gd and Linguist: http://
extensions.services.openoffice.org/en/project/Linguist. Both extensions fail to 
appear after installation with their toolbars/menus. 
I have also had problems with bookmarks menus: http://
extensions.services.openoffice.org/en/project/bookmarksmenu which installed but 
in wrong language. 
I use OOo 3.3RC3 on Ubuntu Linux x86_64.
