Yes but that is a very long procedure to find all relations. Looks like I need 
to build a custom join form.

I had a few minutes to try before I thought I would.  I added Public
permissions to the 3 Request ID fields (it is a join being shown in the
table field).  The last grouping still doesn't show the records.

The data from is using row level permission; Submitter/Assignee.  I
am experiencing this as an admin user though.  If there is a bug with row
level permissions and table field grouping I suppose it isn't too far
fetched that the table field is confused and shows inconsistent results.

I'll remove the row level permissions this evening and try again.

This really does suggest a JSP/jar mismatch. 

Try installing a new version of Tomcat and drop a Mid Tier war file (I assume 
you can download) into the webapps directory.
