Did you use Overlays? I wouldn't have thought this type of issue would be possible if you did, which would seem more like a BMC defect. Did you creat the form as an overlay or as a base customization? You can go ahead with any of ITSM 7604 SP2 or 7604 SP4. But it is suggested to install SP4 as it a newer release than ITSM 7604 SP2 hence it promises better functionality and contains fixes of all the known issues of ITSM 7604 SP2.
