It was new on test environment. Our last 3 test upgrades also failed but there 
was no email error in the logs.  
This is the first time we had email issues and duplicate key error.

We have the same error, but going from 8.1.02 to 9.1...  Also, 9.0 failed
as well...  Got the same duplicate key Value error.  We have engaged BMC,
will let you know how it goes

Was this an upgrade on new hardware? If so, you will have to license it before 
your start. We installed 7.6.04 on a new box, swapped out the database with the 
existing DB, licensed it on the new server, and then ran the 8.0 upgrader. BMC 
recommended doing that one first then moving on to 9.0.01 upgrader. 

Try that.
