In one case the first subscribe after a restart was rejected:

SIP/2.0 481 Call/Transaction Does Not Exist


Later subscribes from the same phone needed authentication and after
this where rejected:

SIP/2.0 404 Not found (no mailbox)



There seems to be another problem. Even if the mailbox information
is available, the phones are not notified immediatly when a new
voicemail has arrived. The notification is delayed until the phone
subscribes again.

Here it seems to be the other way round. Occasionally I see that
peers have lost there mailbox setting and don't get notify messages
with voicemail information. It is Asterisk 13.5.0

"sip prune realtime peer ..."
"sip show peer ... load"

After this the setting is restored, but until now I have no idea why
this happens. The database field mailbox remains unchanged.

Could you post the Realtime SIP Settings?

Could you post a sanitized version of your res_config_mysql.conf and
extconfig.conf files?  I'm thinking maybe you've got an error in there

Do you cache realtime stuff ?
If so, that would explain it

but voicemail?
How does that work?

Oh man, this is some freaky stuff. I commented out 'format=wav49|gsm|wav' in 
voicemail.conf and did a reload.
I set the format field to the user to 'gsm'.
And... Asterisk record a wav file. Huh?

I dont recal realtime voicemail using caching. Sorry.

In logger.conf, enable debug mode: debug = notice,warning,error,debug,verboseStart asterisk in debug mode: asterisk -d

I think you just comment out dbhost
statement and leave the dbsock statement in, and it assumes its a localhost
connection on the socket specified.

That's not a solution, but just a workaround.
1.2.1 has a bug where it always uses an empty context when searching for
a mailbox when using realtime config.
At around line 546 of apps/app_voicemail.c there is a line that says
var = ast_load_realtime(voicemail, mailbox, mailbox, context,
retval-context, NULL);
Change it to
var = ast_load_realtime(voicemail, mailbox, mailbox, context,
context, NULL);
Then recompile and contexts will work.

That looks like the minimal config... 

Something I found out was that I need to set the context = '' for it to
work. I was using default, but for some reason I could never get that
to work. Perhaps it is a bug?

AFAIK, you *must* specify the context when using realtime, even if the
context is default. Change the above to:

exten = 8501,2,hangup
Or something like that I assume.

Uh..no..I've got about 5 different instances of the extension 3113. They are
all in different contexts.
Contexts (for us at least) are broken into customers. Each context has
specific extensions for their company. All have their own VoiceMailMain
entry. Pretty easy.

A debug trace is simply tail -f /var/log/asterisk/debug | grep
res_config_mysql

pretty simple...

So, how does the debug trace really need to be setup? Or simpler do I 
have a syntax error to authenticate for the mailbox?

AFAIK, you *must* specify the context when using realtime, even if the
context is default. Change the above to:

exten = 8501,2,hangup

Or something like that I assume.

 I realize that I did not need to use the EXTEN variable, since I had
 unique entries in this case.  I added [EMAIL PROTECTED] ( or
 could have used the variable) and all works correctly.  Thank you.  I
 assumed that the context entry in the voicemail_users table
 identified the mailbox location.  In the past, before realtime, and
 with the mailboxes defined in voicemail.conf, I did not have to
 append the context in the extension table. I don't really care that
 it is required now, but why did it work before?

Don't know why it worked before. I've always had Voicemail([EMAIL PROTECTED]) in
all my extensions from day 1.

You entered 'other' as the context in the voicemail_users Database, but
you failed to specify that context when you made the call for voicemail
from the dial plan.  The dial plan should be:

VoiceMail(SIP/601-a9a3, [EMAIL PROTECTED])

As stated in other posts on this subject, the Voicemail application
assumes the context to be 'default' unless otherwise specified.

Having said that, can you specify what the actual problem is?  Can't get
voicemail to pick up; MWI doesn't work; etc.

anything about the extensions context? My original email is below..and I
don't see anything in there about extensions context.

Mr. Horton, could you please post the relevant section to your
extensions.conf so I can see how you are calling Voicemail?

VoiceMail([EMAIL PROTECTED])

If you don't have the @context listed above, Voicemail will assume
'default'.

There is no column 'context'.
Add a column 'context' to your voicemail_users table, default value of 
'default', make sure the column is filled with 'default' as a value. Or 
edit the source code to modify the SQL call, to ignore querying 
the AND context = '.

Do this and your login should work.

 Let me clarify my last message.
 
 If I put in the wrong password I get polled
 again for the password.  When I put in the
 correct password, the system errors and returns
 an incorrect password message, which is due to
 asterisk doing a sql query using the callerid
 and not the mailbox.

Have you looked at the debug logs to see if there is any SQL errors?

I realized the problem, was using a semi colon to separate addresses
to send the VM email notification from one extension to 2 addresses.
I solved that issue by using a forward address which contains several
email addresses, keeping the single address in the email field for
notification
ex. [EMAIL PROTECTED] points to 2 real email addresses
