I did as you suggested and looked for siblings. As it turns out, it appears 
that Manuel b. 1852 was the youngest child in the family. He had an older 
brother, Manuel b. 1842, plus two sisters named Rosa and brothers Antonio 
and Francisco. Now I'll have to look in Obitos for the first time. The 
oldest child was Maria b. 1838. Their mother's full name was Maria Rosa de 
Medeiros. Fortunately the maternal grandparents were listed in all the 
other baptisms: Antonio Francisco da Costa and Rosa Maria de Medeiros. It 
appears that this is the case where the mother's surname was chosen. Now I 
need to translate all these other baptisms. Wish me luck!

 Yes, the names of the maternal grandparents are missing.  Maybe the priest 
 had too much vinho.  :)

 They could be children or they could be unmarried.  You'd have to research 
 that family to figure it out.

 I'd collect all the siblings of Manuel.  You'll see if the mom's name 
 flips back and forth (Maria Rosa, Maria de Medeiros, Maria Rosa de 
 Medeiros).  And one of the siblings will have the maternal grands.

Good luck!  By searching all the siblings, you got the mother's full name,
increased your reading ability, and found out that priests sometimes have
too much vinho at a baptism!

According to his marriage record, Manuel Amaral, age 26, native of this 
parish, son of Manuel Amaral and Maria Rosa, married Maria Rosa Francisca, 
11 Nov 1878 in Nossa Senhora do Amparo, Algarvia, Nordeste, Sao Miguel. 
Searching the records for 1852 there was only one person who seemed to fit.

My translation: All and any corrections, welcome

Manuel, legitimate son of Manuel de Amaral and his wife Maria de Medeiros 
parishioners of this Chapel Our Lady of Amparo of Lomba de Algarvia (?) of 
the Parish Church of the Synod of Sao Pedro of the place of Nordestinho. 
Paternal grandson of Francisco de Amaral and Rosa Maria de Sousa born on 
the 24th of April eighteen fifty two and was baptized on the 2nd of May of 
this year in this Chapel by Candido Muniz Furtado, clergyman. Godparents 
Francisco and Lodavina “filhos familias” of Francisco Cabral de Mello and 
Maria Jacintha Carvalho. Witnesses Franco  de Pimental, sexton of this 
Chapel and Francisco da Costa, married, both residents of this Lomba.
