Here's my lpstat output. I'd better check the CUPS manual and read up on
how to manually specify a PPD file.

While reading the howto at http://www.cups.org/cups-help.html#tell I got

You do not say what versions of the software programs you are using.
The subject line says FreeBSD-5.1 RELEASE, so I assume you are using the
ports/packages from the CD.  If that is the case, printing was broken
with cups and gimp-print at that time due to an incompatibility between
ghostscript and cups-pstoraster.  The answer is to cvsup a fresh ports
tree and install the latest versions of the ports.  If you already have
the latest versions installed then you have some other problem.

Your ppd file is the same as mine. The only notable differences I can 
see in the output of lpstat is that you're using /dev/ulpt0 and I'm 
using /dev/unlpt0. So I tried using /dev/ulpt0 as well. I'm still 
getting correct output.

This is strange. Sorry I can't help you any more.

p.s. One thing I did notice was that I had to restart CUPS before I 
could add another printer.
