The problem is that this DVD drive is a relabeled BenQ drive, and it has the OEM (first version) of the Philips firmware, so it's likely at least a couple years prior firmware to the latest available firmware, which as far as I can tell is the firmware for the BenQ model DW1650 version BCIC. There were many upgrades for the BenQ version to this unit, and most people report this is a good, solid performer, so if possible, I'd try to cross-flash to the BenQ firmware, which seems to be a common thing to do. (is your Toast program or some other program phoning home perhaps? Little Snitch can help phone home instances). When I Googled failed: Bad Instance Name (-65540) I see someone having a problem with a HD, so it doesn't appear to be specific to the optical drive unit, but rather perhaps a bus issue? I didn't see a solution, but you might try a Safe Boot and see what happens when no 3rd-party extensions are loaded (you may not be able to use Toast, but Disk Utility or other Apple apps should burn DVDs still). Also, the Safe Boot will clear the cache files, so IF it works in Safe Boot mode, there's a chance it MAY also work in a normal boot when you restart. If if DOES work in Safe Boot, but DOESN'T work in normal boot, THEN you've got a software problem with some non-Apple software extension. If it DOESN'T work in Safe Boot, you could have a System software issue, which MAY be solved by reinstalling the latest COMBO update.
