RE: [Hardhats-members] Patient access to medical records.

2005-02-17 Thread GARY MONGER
There is a My HealtheVet pilot program running at several sites. Here's the
link http://www.health-evet.va.gov/. The pilot is a prototype. A national
version is under development. Here's the link: http://www.myhealth.va.gov.

The new version does not yet pull data from VistA, but will in releases
slated for later this year, the first being for prescription refill.

 From what I understand MyHealth-e-Vet employs a 3 tier
architecture.with Oracle storing the patient's data extracted from
VistA. It is not a real time link for updatesit is done by a batch
process. I agree with Nancy that it would be best and perhaps easier to
replace it. I should add that the functionality of the current system is
very good...another kudo to the VA here...while the rest of the world
has been talking about patient access to medical records for years the
VA is doing it.

I don't think it is quite as bad as a batch process. I looked at it when this
topic first
came up some months ago and from what I recall of it, it updates the data for an
individual patient record via HL7 on request from the patient while the patient
is viewing
the record. I didn't actually try the update for the demo patient I was looking
at, but I
expect that it would update within a few minutes when requested.

Starting with an OpenVistA database and building on M2Web, I think it would be
fairly easy
(as programming goes ;) ) for someone familiar with the data structure (files
and fields)
of the medical record to provide a similar view of live data.

I expect that the extra layers of isolation and encryption in the current
MyHealth-e-Vet
could be implemented as well with GT.M and other Open Source tools.

I should add that the functionality of the current system is
very good...another kudo to the VA here...while the rest of the world
has been talking about patient access to medical records for years the
VA is doing it.

Re: [Hardhats-members] Patient access to medical records.

2005-02-04 Thread Kevin Toppenberg
Nick,

I believe you are correct that only entries to the NEW
PERSON file may be users, and only users can log in,
so only users can access CPRS. But not all users can
use CPRS, only those with OR CPRS CUI as their primary
menu (I believe).

If a user is also a patient, then they will have a
second entry in the PATIENT file (file #2). And as a
default, they could view their own record.

If you want to limit access, you have two options. 1)
mark a patient as sensitive. Then all access to that
patient is logged (see other thread). Option 2) is to
provide a limited list of patients that the user may
see. This is an field in the NEW PERSON file--I can
get you details if you need.

Hope that helps.
