[Histonet] Technical/Performing

We currently have a "performing location" listed on our reports. CAP is telling 
me that if you have technical work at one lab, and sign out/Interp at another 
lab, you must list both on the report.
We are a multiple facility where my lab does our own work, but will be 
performing technical work for another hospital in our health system. I also 
have a bigger hospital that will be doing this for different facilities. So 5 
facilities in total.
Thoughts on how to handle this? We are thinking a generic disclaimer but it's 
going to be lengthy! Hope my explanation made sense?!

Histotechnologist.

JOB SUMMARY:
The Histotechnologist at St. Joseph Hospital is responsible for performing 
technical duties in preparation of surgical specimens for microscopic 
examination.

RESPONSIBILITIES:
Receive and accession tissue specimens and prepare specimens for processing.
Cut thin sections and stain according to established procedures for routine, 
special stains, immunohistochemistry and coverslip and label finished slides 
for the pathologists review.
Assist the pathologist with cutting and staining of tissues for frozen sections.
Maintain equipment, instruments and cleanliness of working area and maintain a 
safe, clean work environment including proper labeling and storage of chemicals 
and specimens (and disposal of same).
Prepares written documentation as required by regulatory agencies, the 
profession and the department. Regulatory agencies include, but are not limited 
to, FDA, JCAHO, CAP, CLIA, OSHA and the state in which this facility is located.

MINIMUM QUALIFICATIONS:

Associates degree required
At least two years on-the-job training or experience is required
Experience with personal computers is required

PREFERRED QUALIFICATIONS:

Bachelors degree preferred
Certified as HTL/HT (ASCP) or equivalent preferred or eligible for 
certification as HTL/HT (ASCP)

[Histonet] Tissue discard

What sort of PPE does everyone use when "dumping" tissue? Interested mostly in 
if folks are using a N95 mask, respirator, PAPR? We pour the formalin off our 
tissues and bag the tissues to be taken away.

[Histonet] TempTrak monitoring system

Good morning!
Is anyone using the TempTrak automated temperature monitoring system in their 
Lab? We are getting ready to go live with it, but are having problems with 
TempTrak probes reading the proper temperatures for our cryostats. And also a 
-30C refrigerator in another part of the Lab. The system doesn't seem to like 
cold temps! Any input would be greatly appreciated! Tanya

[Histonet] Medicare 14 day rule

I am just curious on  how everyone is handling the Medicare 14 day rule when 
sending blocks out to reference labs for testing?
Are you just holding on to those blocks for 14 days post discharge for 
inpatients then sending out? Reregistering these with the
Date of Service as when you are sending these blocks out? Any input is 
appreciated!

[Histonet] Bar Code and Tracking

Hello,
I manage a small Pathology lab where we generate approx. 80-140 blocks for 
Histo and 5-6 Cytology fluids a day. I have 2 Histotechs that swap doing 
grossing monthly. I have 1 full time and 1 part time Cytotech and a Cyto 
assistant. I am curious if there are any smaller labs out there that have gone 
to a Bar Code and Tracking system and can offer any input. I came from a much 
larger lab that used CoPath from accessioning to sign-out, so that is the kind 
of system I am used to. It is certainly a system I wish to integrate in to my 
lab on a much smaller level, we currently have a very manual process, other 
than some entry/labeling in our LIS.

[Histonet] New CPT Code 88350

Immunofluorescent studies? Is it like IHC? First IHC is 88342, the rest are 
88341. So the  first IF is 88346, the rest ordered are 88350?

Help!!! We changed our processor on Monday, all reagents. Run Monday into 
Tuesday a.m. was fine. Tuesday nights run had some blocks in it that weren't 
fully processed; chamber looked like it had droplets of residual water (or 
maybe alcohol?) in it. Dumped and refilled everything, ran a few test blocks, 
same scenario.
Any suggestions?!

[Histonet] Coverslip tape

This is more a Cyto question, but, does anyone out there use the Sakura 
Coverslip tape to coverslip their Thin Preps? We seem to be having a problem 
down the sides of the slide where the tape is not being pushed down and access 
glue seems to accumulate there. Sakura tells us it's a drying issue but I 
really don't think it is, we let the slides dry long enough. It seems to me 
that the bar that pushes the tape down is not pushing down well enough on the 
sides. We have changed the springs recently and are on a new roll of tape. Our 
H slides don't seem to be affected.

[Histonet] AFB contaminant

Has anyone ever had a problem with a possible contaminant in their AFB hand 
stain? If so, how did you deal with it? Any suggestions?
Ours actually looked like an AFB like organism, which we later determined to be 
negative.

Hello,
I am posting to ask for any information or direction this group may be able to 
give (or leads for contacts) concerning the possibility of donations to a high 
school science/ histology laboratory.

I am a high school teacher (Conrad Weiser HS, Robesonia, PA) and Cell/ 
Molecular Biology PhD student (Univ. of the Sciences, Philadelphia, PA).  I 
developed a histology laboratory this year in my high school and it has taken 
off!  The students are extremely interested in learning and experimenting 
utilizing histological technique.  I am holding a summer program opening the 
lab for students to work.  Initially I thought I would have 5 to 10 students 
that would want to work over the summer; 28 students are attending.  It is 
inspiring to see such enthusiasm!  With this in mind, I would like to try to 
accumulate some additional equipment/ supplies.  If you know of any 
laboratories that are closing/ shutting down/ or equipment/supplies that are 
not being utilized and there is a possibility of donating these items to a 
public high school please email me off list

[Histonet] Pipettes

CAP checklist ANP.23085 refers to Pipettes that are used for quantitative 
dispensing of material are checked for accuracy and reproducibility at least 
annually, and results documented.
We perform our Special Stains by hand. So we use disposable, glass, Non Class A 
pipettes. About 15 in a bag. We obviously do not check these one by one for 
reproducibility. I did call CAP and although he tried to be helpful, I still am 
confused!  Any suggestions on how anyone else handles this?

Luckily we do have a research coordinator who takes care of all of these for us 
and he is very thorough. He obtains consent and deals with all costs. I am
thinking that I just need to verify that the facility receiving the block (s) 
are retaining them for the appropriate amount of time, which they are.

Bill is correct, CAP is actually the least of your worries.  There are many 
regulations about human tissue used in research (I do not know them all), but 
patient consent is essential and in some cases the patient must actually agree 
to the specific research project.  Here we have regulations about control block 
tissue that we use, we cannot get tissue directly from a local hospital.   Also 
the laws are different in each country.  There is a lot of bioethics involved.

There are other issues, besides CAP, that you should consider when releasing 
blocks.
1. A block is a medical record. Has the patient consented to release for 
research?
2. If the block is released prior to 14 days discharge and there are any 
charges, the charge must be sent to your hospital. Post 14 days the charges are 
sent to insurance and patient.
3. Will the research lead to a commercial product? Again, written patient 
consent.
4. You may be able to charge the research facility for sending the block. 
Administrative costs.
5. Communicate with patient is a must.
6. Get written consent and have the receiving facility verify they will handle 
properly, retain or return the block d you will get copies of data for patient 
file.

Make sure you reduce your risk.

I did call the CAP and they stated first of all, that it is a very confusing 
checklist component! But I was told that the research facility is taking on the 
ownership of the block, and we need to ensure that whoever we are sending our 
blocks to are following the proper retention protocols.

 In a couple of weeks, Histonet will celebrate its 19th anniversary! It is 
 hard to believe it has been in existence that long. I would like to say 
 thanks to all the members who contribute to the list, sharing their time and 
 expertise with others in histology-related fields. We currently have 3800 
 subscribers from at least 30 countries ( last time I counted).  It is amazing 
 to see how the list continues to provide new insights and information after 
 all these years.
 I'd also like to say welcome to Dr Anita Sengupta, who is taking over for Dr 
 Sandy Cope as a co-administrator of the list with me. Anita is also a 
 pediatric pathologist and she will help with some of the issues with list

This is for you Histoneters In Pennsylvania! Are you all familiar with PA 
Department of Health Act 122-2013? I am wondering how the Pennsylvanians are 
interpreting the rules for sending out slides for consults to other states? Are 
you expecting those consulting labs to have PA DOH permits? Some of our 
consulting labs perform further testing that we cannot perform here in our lab, 
and some perform further testing that we can perform here, to aid in their 
consult opinion. And sometimes they just look at our material and give an 
opinion. Any ideas?

[Histonet] Storing specimens

How does everyone handle storage of specimens for 2 weeks post sign out? We are 
a small lab (as is everyone on Histonet, I'm sure!!) and trying to figure out 
the best way to organize to make the flow go smoothly. Meaning, we are hoping 
not to have to check each specimen to determine it was signed out before we 
discard. Any suggestions?

I agree with Rebecca. We had this problems constantly at the previous hospital 
I worked at and were in contact with Ventana frequently regarding this issue. 
It is my understanding that this hospital is still having problems and that a 
Ventana rep is in there frequently to perform decons. It's all about the 
decons! Very frustrating! Where I am now, we hand stain our retics, no 
problems!! Good luck!

Sorry I was a little vague on my quest for answers!! It was our first 
one,!
Pathologist was down in the Lab (we are a small hospital) and slides were sent 
down. He gave a dx/adequacy (none of them were really adequate for a proper dx) 
for each slide, they sampled 3 levels multiple times, some passes were grouped 
together. Slides came down about 7 different times for a total of 22. We 
received slides that some appeared to be dropped on and some were smears. And 
then after the procedure received aspirates in Cytolyte.
It was a very interesting day!

Sakura saying parts for this will

[Histonet] Clearium?

This is a bit more of a Cytology question, but I thought I would survey my 
Histo friends! We are starting to have bubbles with our PAP slides, and it 
actually seems to occur after drying, initially after coverslipping (by hand) 
there appears to be no bubbles. We use Clearium, so we can go directly from 
alcohol. I am a new manager, and this lab is very adversed to Xylene.  Any 
ideas?

[Histonet] Acid Cleaned glassware

We just acquired a brand new laboratory microwave to replace ours from the 
1980s,..so we are optimizing our GMS stain. Currently we would let the 
glassware sit in
Chromic acid for 1 hr to chemically clean the glassware before staining. Now we 
are using the CAP required polypropylene vented microwave containers. Can we 
skip the chromic
Acid step?

[Histonet] Combo xylene/xylene substitute

We use xylene substitute for everything in our lab instead of xylene, including 
in our Prisma stainer. We are looking to purchase the coverslipper and link, 
which needs to use xylene with the coverslipper tape, and also in the last 
station of the stainer. Has anyone ever had to use both xylene and xylene 
substitute (Citra-clear) in their staining process?  Any suggestions?

[Histonet] New Microwave

We have finally taken the plunge (thanks to our CAP inspection!) to a new 
beautiful stainless steel vented microwave to replace our kitchen one from the 
80s!
Any suggestions on how to meet all the CAP guidelines for this? We only use the
