The slides have to be reviewed and the best block chosen by a pathologist. If there is only one block then the pathologist needs to look at the slides and determine if there is enough tissue for molecular testing. It's a professional charge. Use it on re-accessioned cases for which molecular testing is requested and the best block needs to be chosen. Yes we use it when sending out for Oncotype DX if the case was signed out over 30 days before. [Histonet] Ventana's HER2 Dual ISH DNA Probe Cocktail
Ventana just received FDA approval for the first fully automated diagnostic assay for HER2 gene status determination in breast cancer patients. I do not have a Ventana immunostainer and from what I have been told, it is a closed system and this new probe cocktail could not be used on another stainer, such as the Dako. Is anyone out there in HistoLand using this new cocktail and am I correct that it would not work using the Dako Autostainer. A Ventana rep may contact me with this information also. We do the following for slide and block disposal: Slides are discarded into a rigid, puncture-proof biohazard sharps container and discarded with the regular trash. All paraffin blocks are discarded into the regular trash, as well. I believe I once read that Joe the Toe Nocito remarked that paraffin blocks are not hazardous (infectious) and to prove his point, he licked the block! Our two pathologists have just started using the Dragon speech recognition - the medical version that our hospital had purchased for other physicians. One of our pathologists, in trying out the software, read from a pathology textbook. It never missed a word. We have been using it for approximately three weeks now. It has eliminated a full time position (she had just given her notice one month ago). The other parts of the transcriptionist position have been absorbed by three laboratory secretaries. So far, the pathologists believe that using the Dragon software has decreased our turn around time. We have Meditech here. Our lab secretaries accession all cases - not the individual departments. We do not use OE for this. After the secretaries have ordered the specimens, I do go back into each case to be sure that the correct tissue description and CPT code have been entered. [Histonet] Compromised specimens
When our histology lab receives a compromised specimen (not labeled with patient's name, wrong DOB, no label whatsoever, etc) we have the offending party correct the error and fill out an accountability form accepting full responsibility for the identification of the specimen. We then scan the accountability form into the patient's medical record and include the information in the specimen comments section of the pathology record. This information does not print on the official pathology report. My question - should the pathologist include in his/her report the compromised specimen information? Presently, on our clinical lab reports and cytology reports it is included - simply saying Compromised Specimen. Our pathologists historically have not wanted to include this in the official pathology report and I feel that it should be in there - sort of a cover your butt addendum.
