We just got CLIA inspected and had no deficiencies. The inspector did say he 
wants to see us write down on any reagent containers when it was opened, and 
write down the new expiration date.



The containers have the expiration date on them from the manufacturer, but the 
inspector said when the container is opened, the expiration will change and we 
need to write the date on the container under the opened date.



I've never heard of this requirement and I would like to ask if you have.
He said if the vendor does not give us any written documentation of when a 
reagent would expire after it's opened, then we should seek an alternate vendor 
who can.



Any comments, etc. are greatly appreciated.

We use an Azure B counterstain. Melanin has a greenish tinge after 
counterstaining.

Hai all Histonetters
 Please anybody clarify my this doubt if possible.
When doing Immunohistochemistry for confirmation of Melanoma with DAB 
chromogen(brown color)the interpretation is not possible because of obscuring 
by the dense melanin pigment. We dont have any other color chromogen. I tried 
ihc after bleaching but the section gets detached even from charged slides. Is 
there any other effective way to do this?

 We still have this issue after two years of complaining to Roche.  We've had 
multiple TAS's come in and we've sent slides to Tucson. They've gone over the 
instruments, etc. Long story short, their fix was adding something to their 
package insert that says "Schiff's Reagent is known to contain some needle-like 
precipitate. At expected levels, this precipitate should not affect assay 
performance". Good luck.

I am wondering if anyone else has been having issues with the PAS stain.  The 
crystals are not rinsing off of the slide using the benchmark special stainer.  
We are on our 3rd lot number, no change.  We have had our tubing changed and 
our instrument checked to make sure the rinse is appropriate.  This problem is 
causing our lab great frustration.  Any feedback would be appreciated.

[Histonet] curiosity may kill me

tissue sections but our Micro lab stains at room temp.  It is because "it's 
always been done that way"?

Hello Histoland,

I need some advice, I have a PT block that is positive with Spirochetes.
What would be the best way to use this block as a positive control?

1)  Cut (serial sections, stain the last slide for bugs) and oven time
(60) for 1 hr.
 then put slides in refrigerator for future use.
2)  Cut (serial sections, stain the last slide for bugs) NO oven time and put 
slides
 directly in refrigerator for future use.
3)  Cut "fresh" every time they order the Ab.

Our hospital Safety and Infection Control departments have policies in place 
prohibiting any potted plants from being in the hospital, anywhere.  We can 
have them if they are only in water, but the soil presents an infection control 
issue for patients because of potential mildew, mold, spores, etc.

[Histonet] Ventana dispensers

Good morning Histonetters,
Is anyone else having issues with their Ventana detection? My dispensers are 
clogging up with rubbery plugs and mis-dispensing. It's not that they sit in 
the refrigerator and don't get used for long periods of time. We are a high 
volume lab and we go through detection very quickly.   The biggest culprit is 
OptiView. I called in to log a complaint and was told to dig in the drop tubes 
with a pipette tip each time I use them. Really?
I've logged the complaint several times now. Just had a TAS out. She told us to 
do the same thing. I don't know but ever since Roche bought them, I get more 
and more dissatisfied.

[Histonet] Visual alert for processors

 histotechs say the tissue is crunchy and they are right. I am running a test 
 tonight of a small needle biopsy that I made from a colon. I placed it is 
 straight formaldehyde overnight and am processing it on our biopsy cycle 
 tonight. My director also wanted us to only put three levels on our Thermo, 
 but he wanted the middle level to have empty baskets. I stopped that today 
 because I think the other issue is that the poor biopsies may be on the top 
 level and as the reagents are used the level changes, and also due to 
 displacement with the middle level being empty the reagent levels may not 
 reach the top. We just do not have the manpower to inspect every reagent 
 every day, we have 6 processor and it would take a tech all day. We actually 
 take a digital picture when they come out of the processor. I want to check 
 my problems cases tomorrow. We do not use sponges but the only other like was 
 the PA who was wrapping the blue paper very tight around the tissue. I really 
 do not think this is the issue though.. Any other insight would be greatly 
 appreciated.

[Histonet] Oil Red O

Hello Fellow Histonetters,

We often get slides from another hospital for Oil Red O staining. On occasion, 
they send them at room temp. and until they get to us it's been a day or more.
I'm worried that because they are unfixed the cells will be autolyzed and I 
don't want to charge the patient for a useless stain.
  I haven't been able to find any guidance on how long a frozen section can sit 
at room temp. and still be acceptable to stain for Oil Red O.
Your thought and opinions please?

[Histonet] shelf life

We are being inspected soon and I'm trying to check the labels on all of my 
chemicals.  How do I determine the expiration date of a 1N HCl solution I 
prepared in house.  I went on line and a commercially available 1N HCl solution 
has a 36 month shelf life according to the vendor's web site. I don't know how 
they arrived at that. I know concentrated HCl has an indefinite shelf life, but 
a 5 year retest date appears on the COA. It's confusing to me.
Any help would be appreciated.

[Histonet] Gram staining

Our new Pathologist would like to see more yellow in the background of our Gram 
stain.  We use picric acid-acetone currently, which looks more pink than yellow 
most times. So  I'm wondering if a method using a Tartrazine counterstain would 
be better.

[Histonet] microtome laceration not an exposure

Does anyone have something that I can show to nursing to explain why a 
microtome laceration is not an exposure? I can't get the fixed tissue 
explanation to sink in. Anyone else had issues with this?

What is it that you want to know?  I used to make frozen multiple tissue 
control blocks for ER/PR many years ago before these antibodies could be used 
for FFPE tissues.  You could do the same thing with paraffin embedded tissue.  
I've made multi-tissue blocks using a skin biopsy punch tool to obtain small 
amounts of paraffin-embedded tissues to be used in the control block.

I have been following this stream of conversation and thought I might jump in. 
Paraffin blocks and microtomy waste are NOT biohazard waste. The process of 
fixation and tissue processing makes the tissue acceptable for general waste 
disposal. You do have to address the issue of PHI on the blocks, if you are 
using bar  code information and especially if patient information is included 
in the bar code. All bar codes used in the lab can be read in any standard word 
or spreadsheet software. I do not see the need to pay for expensive removal of 
biohazard waste, use general waste disposal for your blocks and cutting waste.

RE: [Histonet] Ventana Benchmark Ultra and FITC antibody stains

FITCs have to be purchased directly from Ventana. I've been trying to figure 
out how to trick their system because I have third partys that I'd like to 
try out. They don't offer an option to choose  a Prep kit in their FITC 
template. When I called in to ask why they don't allow it they told me they 
didn't have that much interest from people who wanted to do that.

institution number so that it doesn't confuse any of us refilling slides. What

We had one screen replaced early on but no problems since. However, we've had 
to replace the plastic lids several times and the parts are always on back 
order. The one that covers the molds breaks right off.

We track shallow/incomplete sections and have found numerous reasons. 
Inadequate fixation. 
The way a tissue is bisected can cause it to not lay flat in the embedding 
mold. For anstance a grossing tech who does a wrist turn kind of slicing 
motion and causes an arch shaped edge. 
Embedding pieces that are varying sizes. 
Too many pieces in a cassette. 
Some Histotechs are trained to err on the side of caution and do not like to 
cut too deep the first time. Their though being you can always come back  and 
recut it, but once it's cut away, it's gone. 

Our idea was that by tracking these we might find an opportunity for retraining 
an individual, but as it turns out, we all cut a shallow/incomplete section 
occasionally. 
Some steps we took -
  We had the grossing techs come and watch us embed derm cases. That gave them 
a perspective of why it's important for them to do things a certain way. 
 One of our Dermatopathologists came in and gave the HTs and in-service on what 
they are looking to see on a slide and what the inked margins mean. This helped 
with orienting specimens at embedding. 
 We have reduced our recut numbers by taking the time to identify the causes 
and offering additional training to the staff. Collecting the data is the first 
step, and though it's time-consuming, it is worth it in the end.

If you keep the layer of paraffin thin enough, it can still be scanned with 
most barcode scanners.

Many people are working on the Enzo probes just now. Mayo does have the probes 
up and running on the Bonds, but are keeping tight-lipped about their protocols.

Something to remember is that humidity is the enemy too. If you are storing 
your slides in a refrigerator make sure that the humidity is well controlled.

the same slide as the patient tissue) per antibody, per patient case, but there 
are multiple blocks (same tissue type) from the same patient case, that would 
be batch controlling?

I agree. The Leica stainer and Sakura are both good instruments, however I 
really HATE that Leica glass cover slipper and I had the same assessment as to 
need to baby sit. I sometimes preferred just hand cover slipping because it 
was less trouble, and even faster sometimes ( believe that or not). I had none 
of these issues with the Prisma covcr slipper, and no trouble with the user 
interface etc. I had the same issue with the door latch on the Prisma, but 
other than a quick replacement of that, it worked perfectly.

 Both stainers are powerhouses. The Leica has a plain menu screen with a 
 simple interface while the Sakura has a LCD screen with detailed information 
 about what stage the staining process a rack is along with multiple menus.  
 The difference between the performance changes drastically when the 
 respective coverslipper attachments become involved.
 
 The Leica is seriously no match for the Sakura in this respect. The Leica's 
 coverslipper is its Achilles heel and requires a LOT more attention and 
 alerts frequently, very frequently. It takes a separate rack for staining the 
 slides at the beginning of the process and eventually transfers them to a 
 different rack one the cover slip is complete. This one uses glass and 
 frequently drops glass, creates bubbles, drops and breaks slides. You will 
 have to frequently purge the system and clean the cover medium needle 
 dropper.  Once done, it only holds. Two racks of 30 slides and will alert 
 until you remove it. You can't leave this one alone for more than 5 minutes 
 without an alert. Seriously.
 
 The Sakura's coverslipper uses cover tape which won't need to be replaced not 
 even remotely as soon as the glass in the Leica.  Finished slides remain in a 
 carousel at the top and can hold about 10 racks of 20 before it alerts. For 
 high volume, the Sakura pair wins hands down. You won't lose productivity 
 time by needing to check on this machine pair.

 recommendations?
 Are quantity of HEs is increasing and we need adequate equipment to 
 meet our workload. The incorporated oven seems excellent on both 
 stainers. Any pros/cons would be greatly appreciated! Also, if you are 
 currently using the stainer, does it meet your workload and what is your

I was given info from a Dako rep last week that if the primary antibodies are 
applied separately to the slide as is true with Ventana instruments, you can 
charge for  both.  I am skeptical. Has anyone else heard this?

Its actually not allowed already.  Any cocktail where all stains are done at 
one time on one slide can only be charged x1 even if its 2 or more antibodys in 
the cocktail.  One of our reps updated us with our PIN cocktail

Just spoke with my Ventana rep about this same topic. My understanding is ANY 
vendor who sells HPV probe for ISH is out of compliance with FDA and is 
eventually going to get in trouble. I specifically asked if I could buy probe 
from elsewhere and was told not likely. This is only a problem for us here in 
the US because of FDAs restrictions. I hope someone will tell me I 
misunderstood. Ventana suggests running p16 if you don't already. Just throwing 
in my 2 cents.

My lab is interested in starting HPV High and Low Risk ISH testing, but it 
seems like we just missed the boat on it. We have Benchmark XTs, but Ventana 
discontinued their HPV probes sometime around last December, and they don't 
have an alternative at this time. After talking with nearby labs that are 
currently using stockpiles of Ventana probes, they told me the following 
courses of action they were planning on using when their stock runs out:
-One lab is thinking of doing HPV by PCR -Another is thinking of switching to 
Dako probes

After discussing the possibility of Dako probes with Ventana and Dako, neither 
could recommend using them together because each side was unfamiliar with the 
other's reagents or system, though both said it might be possible.

Is anybody in a similar situation, looking for a replacement for the Ventana 
HPV High/Low Risk probes? Any insight on possibilities to pursue would be much 
appreciated!

a bit. However, depending on the cost per label, it may be worth the trouble.

I received a spec sheet from Dianova that has their Ventana XT protocol. It 
suggests using CC2 standard retrieval and incubation for 32 minutes with amp. 
They give a dilution range of 1:20-1:50. It seems to work well for us although 
we are still validating. I hope that helps.

stain for a little over six months and seem to be having success with the

Hmmm...I think more people should have paid attention to Obamacare two years 
ago when it was being shoved down eveyones throat. Oh well.. as old saying 
goes...You ain't seen nottin' yet! I'm just amazed that there are so many in 
the profession who are surprised about the changes beginning to take place, 
there are many, many more to come in 2013  14 and all of them affect your job 
or lack thereof. :)
 
Everyone had the opportunity to change things on November 6th - they chose not

While this stinks on many levels, I have to take issue with the shift wealth 
from specialists to family practice - family practice docs have been the 
frontline of medicine, all the while earning less than a quarter of what 
specialists earn. It's about time they get a boost.
Too many specialists earn over a million a year, while the family practice 
guys/ladies can barely pay their student loans.

The odd thing that struck me is that this fictitious scenario was planted 
on Histonet just a few day before the presidential election and I am suspicious 
of the final intent of the posting.
Is it to link a frightening scenario with the so called Obama care?
Is it the possible solution of this imaginary scenario to eliminate Obama 
care as has been promised by Romney?
Is this a way to get a few additinal votes for Romney? I would not be surprised 
at all!
I am always very suspicious of coincidences because coincidences do not 
exist.

This is disturbing news. As an employee of an in-house lab (which started in 
1996/1997) that does mostly skins, GI biopsies, and outpatient surgery 
specimens I'm pretty disheartened to hear about the 88305 issue. Melanoma 
excisions, prostates (even lower block # cases, we don't always get 12), breast 
biopsies, and other more difficult cases can be a lot of work on both the 
professional  technical end of things.  

As for prostate biopsies, CMS has already lowered reimbursement with the G 
codes. This is despite the wording that they are for saturation biopsies. We 
rarely have saturation biopsies, but Medicare denies us the 88305 charge if 
more than 5 specimens. 

Other insurance companies tend to follow their lead  after a little time.  I 
believe reimbursement is 50-75% less for 5-20 biopsies, but don't quote me on 
that.  I expect we may see the end of saturation and multi-container prostate 
biopsies in the near future. 

Another issue for many outpatient labs in my area is that larger insurances are 
requiring their patients to go to large multinational labs.  We cannot accept 
many PPOs or Medicare replacement plans because of this. 

I feel it can be a disservice to the patient because they do not get the same 
personal, local service with good turn around times. Even my insurance requires 
me to go to one of these labs where I feel inconvenienced and frustrated at the 
wait time required to submit my sample and get results to my physician.
