Hi Silvia,

We process them with the rest of our small stuff.  About 3 hours on a Peloris.  
We get them and process them on an ASAP run to embed cut stain and distribute 
overnight to be on the pathologists desk at 8.

 We do have a mix of on site and courier services.  When we transport to the

I used it exclusively in GI lab for years and loved it but we only did small 
biopsies, so it was super simple.  We use it now throughout the system I work 
in and there are a variety of opinions.  Some of the PAs love it and some 
really struggle.  Same with the pathologists.  So we live in dual worlds with 
both Dragon and MModal.  If you would like to contact me separately I am happy

PAs and pathologists are the only ones that gross them, we have six hours of 
formalin on the processing schedule to ensure we meet the minimum fixation time 
and Saturdays are rotated to ensure we never exceed 72 hours.

[Histonet] Beaker, Sendouts and ICD10 codes

Have you checked the processors?  It is easy to “mix up” the reagents.  
Has tissue collection changed? Are the specimens left out longer or placed on 
paper? 
 Do you use sponges?  Any chance the fixation time has been decreased?

[Histonet] Automated Embedder

We have been looking into the automated embedding system. The cutting room
is excited about this opportunity, but the grossing personnel have some
concerns. Is  anyone out there currently using this?  Can anyone provide
some input on tissue types, implementation, time differences in the use of

[Histonet] Digital path

I am exploring digital pathology and I am unclear about the current CAP 
requirements and how those may change if the platform is FDA approved for 
primary diagnosis.  Is the wash out period still necessary if it’s not 
considered and LDT?  Does the personnel still have to be qualified as high 
complexity testing personnel to scan the slides?  Any help or other information 
is greatly appreciated!

We use Beaker across our system and like anything it has its pros and cons.  
Tim is correct, it was NOT developed for Pathology, rather for physicians and 
the lab module was an afterthought.  Our IHC lab uses Vantage with Middleware 
and things go really well for them to my knowledge, but it certainly wasn’t 
without its challenges.

Changing any system is going to come with growing pains, for sure, but some of 
the things I would want “redone” include tracking, as a multiple site system we 
have issues with packing lists moving tests with materials to other labs and 
keeping the pathologists from signing cases out.  We lose our tracking from the 
gross station and pick it back up at embedding, using manual batch tracking 
sheets for what goes on the processors.  I believe something has been built to 
help with this and we use a wingman system, so others might not have this 
issue.  From a manager perspective, The report system is getting a little 
insane as with upgrades there seems to be unforeseen changes that impact 
reports you may rely on, also there are a variety of ways to develop or run 
reports (workbench reports, Clarity and Universe!) and the redundancy is 
getting confusing.  From the bench perspective, depending on the type of lab 
you are, accessioning is accomplished three different ways for us, client 
specimens that we have to register, within system specimens that are ordered in 
a manner that we have to go “grab” the order and the ideal way where we simply 
scan the label and input the task protocol (again within the system, but why 
can the doctors/nurses use two different ways to input the order??).  Scanning 
the barcodes from the cassettes can be an issue, but that is not Beakers fault, 
make sure you have solid cassette printers.  Lastly, the physicians view in 
Epic is much different than your view in Beaker, this has caused confusion when 
trying to find things like additional testing reports performed outside and 
then scanned in.  We have hammered through the majority of these issues and the 
Beaker team is great at helping out and thinking outside the box to smooth out 
workflows.  Oh, and billing!  Make sure you test all the way through patient 
accounting!  Stay positive, as noted, likely the choice isn’t yours to make, 
but how you approach it is.  Build a good relationship with your Beaker team, 
prioritize your needs and chip away.  It will work out.

[Histonet] Fixation and IHC

Can anyone tell me what, if any, guidelines there are for fixation time for
animal tissue with potential subsequent IHC stains.  I am very familiar
with CAP guidelines for receptor testing with breast cases, but I can't

Hi Histoland!

We have been reviewing our send out procedures and I have been tasked with
researching what TAT's other institutions define.  Currently, we allow 48
hours for the preparation of "orders" to the time sent out.  Is this
reasonable?  Do other facilities allow more or less time?

I feel like Amazon can get something to my door step in a matter of hours,
so I must be missing something obvious in our steps that could be
simplified, but I can't put my finger on it.  Any feedback on what others
are doing is greatly appreciated.

[Histonet] ANP.22780 and validation

Hi Histoworld and Happy New Year!

I am currently validating new processing schedules and am a little concerned 
about my understanding.  NSH guidelines don't say special stains, IHCs etc., 
need to be re validated when validating a new processor, schedules etc., but 
this CAP standard seems to imply it is required.  This, in turn, doesn't make 
sense to me as many labs send their blocks or unstained slides for subsequent 
testing to other labs.  Those labs don't know the processing schedule, 
equipment, etc.,  Can someone shed some light on this for me?

[Histonet] Peloris Processing Schedules

Hi Histoland,

I am currently establishing new processing schedules for the Peloris
processor and wondered if anyone had a schedule for large tissue they would
be willing to share?  Also, maybe one for excisional/lumpectomy/mastectomy
cases?  I have been testing one for large tissue that is approximately 4
hours, but I think a little longer in formalin may be necessary.  Anyone
willing to share their experiences with the Peloris is greatly appreciated.

[Histonet] Cold Ischemic time

Hi histo land!

Referencing CAP requirement ANP.22983, how do you all document cold
ischemic time?  Do you have a "macro" that the pathologists input or a
disclaimer?  We were recently inspected and we include a "disclaimer" in
the reports that the time is less than one hour, but the inspector said
that it was not "Accurately documented in the report"...I want to fix this
this, of course but wondered if you all had any guidance such that I don't
try and recreate the wheel.  Thank you for your time!

We do strictly GI biopsies, but if we have a stat case, we have a processing 
schedule as follows: five minutes in each station of one 70%, two 95%, two 100% 
and three xylenes, then 10 minutes in each of three paraffins.  We have the 
ASP300 and this has been very successful.  I have heard from others they do as 
little as 2 minutes in each, With a bit longer in the paraffin.  Generally our 
STAT cases are in formalin overnight so we skip that step altogether.

We neutralize ours and have no problems with it.  I am not sure how much you 
use, so I will say it is easiest in smaller batches as you do have to shake it 
up to make sure it dissolves.  We purchase ours from BBC Biochemical for a very 
reasonable price and then test before dumping.  We have doing this for four 
years with no issues.

I told my doctor everything I dealt with on a daily basis and he said it was no 
problem.  I worked in a 400 sq ft space where we do everything including 
grossing!  My son is perfect, but had I known about everything you all are 
saying I would have done things differently!  Nothing is worth risking your 
little one.  If I am lucky enough to have another I want to thank you all for 
the input as I will not do it again!!

 remember ever working with any pregnant HTs in the past so I honestly do not

 predominately work with 10% formalin, reagent grade alcohols, paraplast, 
 xylenes, clear-rite 3, Decal I and II (from Surgipath), cytofixative and some 
 routine special stain items. Basically the typical myriad of stuff a very 
 small routine histo lab would have (no immunos)

We print out a procedure list that has the number of pieces on it and it next 
to the embedding center for reference while performing that task.  The only we 
write on the cassette is the accession number.

As most other bases have been covered, do you leave space between the slides in 
the rack?  Once in a blue moon we had stains from the same rack that differed 
in intensity, after much troubleshooting we started putting space between the 
slides and we haven't had the problem since.  Just an idea.

 About a year ago, I posted about issues with staining mouse kidney sections 
 (see below for original post).  We realized that part of the problem was the 
 sections drying during staining, which has been resolved, but we are still 
 having some issues with some slides staining well and others being very pale. 
  This time it seems to be consistent across the slide, so I know it is not a 
 drying issue.  They are serial sections, so they are all cut together and 
 stained together, and they are from the same animal.  I don't believe 
 deparaffinization is an issue - we have tried longer times with fresh 
 solutions and it doesn't make a difference.  We have also tried re-staining 
 the slides after letting them sit in xylene overnight, and there is no 
 difference in the staining.  I am now thinking that it may be related to how 
 long the sections soak or how long they sit on the waterbath.  They are NBF 
 fixed, and there is a fine line between soaking well for
 hydration and oversoaking, resulting in swelling.  Has anyone experienced 
 anything like this?  Any other ideas?  I am at a loss, and I'd really like 
 for this to work!

 We are doing a Hale's colloidal iron stain (no counterstain) on serial 
 sections of mouse kidneys.  We are staining an entire kidney at a time (about 
 90-150 slides), and after many successful runs, we are now finding some 
 slides in each batch with very uneven staining.  Half of a section will stain 
 as it should, and the rest of the section is very pale.  It seems to be in a 
 similar area from one section to the next, but not exactly the same area.  It 
 does not look like a deparaffinization issue.  There may be two or three 
 slides in a row like this, or just one section on a slide, followed by 30-40 
 that look fine.  The sections are 6 microns. Any ideas on why this might be
