Peggy's last interview on HistoTalk www.histotalk.com was taped last year at 
the NSH. During her interview, I stopped taping several times, because the

Hi folks - The HistoTALK mobile unit is planning on arriving in Pine Mtn, GA 
for the GSH's 1st HISTOPALOOZA late tomorrow afternoon.

Interviewing will begin Friday morning at 9 am. Remember, anyone can be a guest 
on the show, not only workshop presenters! Everyone is invited to talk about 
any histology related subject.

Just stop by the 'mobile' unit (banquet table and three chairs). Look for the 
big bright yellow banner and let me know that you want to be a guest on the 
show and 'll have you scheduled. It's that easy. 

It is very important that you understand that all interviews are taped for a 
latter broadcast. HistoTALK is not LIVE! Umms, ooo's, and aaah's are edited out 
prior to airing.

Actually, I have a tattoo supplier four miles away from me. Several years ago, 
I went to their warehouse to take a look an possibly make a purchase. As it 
turned out, I had to buy a gallon of each color and it worked out to be more 
than the standard tissue inks sold by the histology vendors. 

Next stop - tattoo shop. Same story. The artist would sell a small 8 oz. bottle 
at cost, but it was still just a bit more than our vendors.

You can use Pelican drawing ink. It comes in several colors. It's around five 
something for an ounce+, much more than Davidson or one of the others.

Maybe someone will also chime in.

gotten tattoo pigment powder in two colors and that's what we used to mark 
tissue.  Don't know where it came from and don't know where it went!!!

Hi Histo World, as I was cutting to day I was thinking why don't we see if we 
could get specimen marking ink directly from a tattoo vendor? When I first 
started in histo I was told the ink we use was actually tattoo ink. As we know 
as soon as somebody labels something as a medical supply the price is 
increased. Just a cost saving thought, I mentioned it to my immediate 
supervisor but she think it would be a liability issue. I thought we could 
test/validate it on skin tissue left over from a mastectomy or extremity. Any 
thoughts?

have something to shout about on internet radio, here's your opportunity. The 
mobile HistoTALK (all digital this time around) studio will be available on 
the following days and times:
 
Saturday, September 21 - 9 am to 3 pm
Sunday,   September 22 - 9 am to 3 pm
Monday,  September 23 - 9 am to 12 noon
Tuesday,  September 24 - 12 noon to 3 pm
 
Interviews can be anywhere from 30 minutes, up to an hour! The time is YOURS!

at the testing site that they need to list that on their report and also they 
will need to have a CLIA license for their home or office that is offsite.

The UK Human Tissues Act (2006) is also relevant I think. To work with human 
tissue premises have to be registered. All tissue is tracked. The Health 
Service may have exemptions but generally tracking requires approved couriers 
etc. Registration of premises does require payment of a fee.
 
One can imagine the headlines if your home working pathologist was transporting 
slides themselves and say left diagnostic samples on the train. Patient 
confidentiality around physical notes would also be something to be considered 
I guess.

The only regulation about reading slides from home refers to 
cytotechnologists because they have a limit of allowed cases/day that has to be 
controlled?at the usual work setting, but no such regulation exists for 
pathologists and I really?do not think that a pathologist is limited to read 
slides from home.
The only possible objection would be a pathologist working in the UK because, 
under the NHS regulations,? pathologists have norms?about the?time per type of 
case and its complexity.? This time/case was designed to assure that the 
pathologist takes enough time for the diagnosis which, according with the 
system design, assures quality of work, and also links work-to-remuneration 
rates. Under those circumstances perhaps the pathologists would have to justify 
how much time they have used (to be paid for) per different cases, and that 
would require a work based control. 
Fortunately such a system does not exist here (yet!).

 If you haven't reserved your room for the upcoming GSH meeting next month, 
 please note that all island and standard Oceanside rooms have been reserved.  
 But,  just because you snoozed, were waiting on money for your institution, 
 or you're just a classic procrastinator, there are still options.  There are 
 upgraded rooms such as the lanai.  This room has a Jacuzzi, microwave, King 
 size bed, small refrigerator, and a private balcony or patio. There's also 
 the efficiency which has the kitchenette with a stove top, refrigerator,  two 
 double beds, sitting area, and private balcony or patio.  The symposium rate 
 still applies and if you have issues or concerns, contact Linda Schepps at 
 Oceanside.  She can't cure procrastination but she can help secure a room for 
 you.
 
 While you're strolling around the historic area of the island in your speedo 
 or crochet bikini, check out Becky's famous chicken salad.  It's a little 
 place with outside tables.  Just walk up to the window and place your order.  
 I attempted to have a chicken salad sandwich there but they were all sold 
 out!! The lady managed to let me sample a teaspoon of it. It was delicious 
 and didn't taste like that stuff in the grocery store.  I plan to attempt 
 another order when I return next month!!

This is a very good question and I look forward to answers from individuals 
that have accomplished this with PMMA and a rotary microtome with 
tungsten-carbide knives. If you are talking about an undecalcified specimen 
embedded in PMMA, then I would imagine that the age of the rat could affect the 
ability to achieve an adequate infiltration of the resin. Again, I look forward 
to what others have to say about their success by the method you have outlined.

On the other hand, I know you can achieve the micron thickness you desire if 
you were to use a non-contact femtosecond laser! The machine I am talking about 
is basically a laser microtome manufactured by Rowiak in Germany and it is 
officially called the TissueSurgeon. In fact, Dr. Heiko Richter from Germany 
has accomplished what you ask with human teeth, revealing the full anatomy of 
the tooth and even with ameloblasts on the enamel surface!

I would be interested to hear more about your project. I will be traveling to 
Germany one week from today to work with this laser microtome until the end of 
the month so I could arrange to have laser cut sections made for you if you are 
interested and unable to make your cuts using PMMA and a rotary microtome. If

It is not that I did not care about your response, it is that it was not a good 
advise.
You have to remind that when somebody asks HistoNet it is because they do not 
know about the question and are seeking advise and very probably they will 
follow the advise.
If somebody tries to improve the work flow of the lab and its TAT that person 
cannot look inwards because they will just understand their operation and 
that does not guarantee improvement.
The only way you have to improve is, after knowing what you do, is to compare 
and emulate those who have the best rates.
Competition is the basis and you cannot compete if you only look inwards.
Your initial advise was correct (as I state) and allowed to know yourself but 
the improvement process has to expand to others.
I also stated that I intended no offense so, if you felt offended, please 
forgive me.
I was only trying to help a fellow HistoNeter!

I feel the urge of commenting on this advise about how to determine a 
benchmark for a histology lab.
 
1- I agree that you should first know your lab and how long it takes for your 
techs to complete the work-flow from receiving the specimens to reporting the 
final diagnosis BUT
2- if you just stop at that point you will be unable to determine if you have a 
problem. Finding problems and developing solutions can only be achieved if you 
COMPARE your lab to others and how efficient you are, how your TAT in each step 
of the process compares with with other labs of the same size (similar 
workload, staff and technology).
3- unless you compare you cannot develop strategies to do at least as well as 
others do.
4- to achieve the comparison objective you have to have information about other 
labs and you can only find about that if you hire a consultant (very expensive) 
or if you reffer yourself to published information. I think that you will find 
useful some articles I have written on the subject and that you can find at

Finally, the article you have been referred to 
(http://www.biomedcentral.com/1472-6890/10/2) in its original version was a 
propaganda for the VentanaSymphony autostainer and had to be rewritten to 
achieve its present form. Essentially lacks the information you need (no 
offense intended, just the facts from my point of view).

The best option is your own process mapping and time studies of these processes 
working in your own lab environment. It is usually easiest if you intially 
break it down into the preanalyic, analytic and post analytic phases and the 
critical variables that reduce your delivery times or contribute to impact each 
of the bottlenecks/delays/errors/ problems, in each phasse,  and then put these 
in problem statements in your  LSS project charters. It gets convoluted if you 
put them all in one, so I think it is best if you find one that you can target 
as a quick win and use the momentum from this for further projects. Do a 
detailed as is process map  from your current data and then work on 
streamlining by elimination of those items to build a more standardized and 
improved  how you want it to be process map. This becomes your SOP more or 
less for the new procedure.  I know you wanted numbers, but I guess I feel your 
own numbers are going to be a lot
more useful.  However, there are a few references out there that are pretty 
detailed and are more or less case studies that have some numerical data. For

Hmmm...I think more people should have paid attention to Obamacare two years 
ago when it was being shoved down eveyones throat. Oh well.. as old saying 
goes...You ain't seen nottin' yet! I'm just amazed that there are so many in 
the profession who are surprised about the changes beginning to take place, 
there are many, many more to come in 2013  14 and all of them affect your job 
or lack thereof. :)
 
Everyone had the opportunity to change things on November 6th - they chose not 
to. So, you live with it.

While this stinks on many levels, I have to take issue with the shift
wealth from specialists to family practice - family practice docs
have been the frontline of medicine, all the while earning less than a
quarter of what specialists earn. It's about time they get a boost.
Too many specialists earn over a million a year, while the family
practice guys/ladies can barely pay their student loans.

[Histonet] HistoTALK in Vancouver!

Hello Histonetters -
 
Two NEW HistoTALK http://www.histotalk.com/ programs are up and running! 
September 16th show has Beverly  Doug Robinson talking about private labs in 
general and then on September 23rd's show, Lynda Ferry a newly registered 
histotech talks with us about why she chose Histology as a profession. Fun 
interviews!
 
I will be in Vancouver for the 38th Annual NSH Convention/Symposium. HistoTALK 
has a table out in the registration area (somewhere!?) and I'll be interviewing 
some of histology's most interesting, informative and entertaining 
professionals! If you would like to be taped for a future show, stop by Monday 
and Tuesday and I'll be sure to get you on the show! Tuesday I'll be doing a 
workshop from 10:00 am to 11:30 am, so I won't at the HistoTALK mobile studio 
during those hours. 
 
I'm hoping for return interviews with some of our NSH Officers and

I assume you are talking about HT(ASCP) Registered techs. The majority of Mohs 
Techs are off-the-street people. Here in FL for Registered Licensed techs, 
between $25 - $30 / hour. The off-the-street folks $14 - $18, sometimes as 
low as $12.00 / hr.

Good stuff. In the old days, 36 years ago for me, taking the HT(ASCP) exam it 
was said that you were registered by the ASCP, because the designation HT is 
given by the Board of Registry of the ASCP. Many of the NEW folks use the word 
certified. After a total of 39 years (3 years was training before you were 
eligible) I still only use registered by the ASCP / Licensed by the State of 
Florida and call myself a histologist. 
 
In those days ( long before the Internet), you took your ASCP exam (HT's, MT's, 
CT's, BT's, MLT's) at specially selected medical schools across the US. You 
chose the one giving the exam which was closest to where you lived. If 
you needed to drive 100 miles or further to get to the examining college on 
March 15 OR August 15th, (the only dates it was given) that's what you 
did. Getting into the exam auditorium before you were allowed to sit (that's 
what it was called) for the exam, was a challenge. You had to prove that you 
were who you said you were or you were not getting in. Once those guarded doors 
were closed - they were CLOSED! I saw several folks crying outside the 
auditorium that day n 1975. Chances for cheating were eliminated at every turn. 
Unfortunately, not so today. So you can see why for us older techs, if you 
were HT(ASCP) it really meant something. Unfortunately, not so today.

I see this subject tends to illicit strong sentiments from
professionals who are impacted or have an impact on HT/HTL's (sort of
everyone on the net)?.

I am still in school, but I want to fully understand how training,
certification, and registration work for HT/HTL’s.  I realize that
ASCP certification is voluntary, and that some States require some
sort of license or certification, but I’ve never heard of a “Registry”
for HT/HTL’s.

-The way I understand through what I’ve been taught at school is that
Histology is the study of tissue, And that...

-To study tissue there is another science that prepares specimens so
they can be studied. And that...

-There is a final sequence “Quality Control” that verifies the science
that prpares specimens is properly done so the tissue can be studied.
And that…

In order for this all to happen successfully and consistently, the
HT/HTL's make sure that during the whole preparation process, safety
is observed, proper adherence to federal and state regulations
maintained, plus train other technicians to do the same, and much
more.

If I understood it all correctly I can’t help but wonder:

If HT/HTL's do all of this crucial preparation work to make sure
specimens are acceptable for precise microscopic identification of
cells, tissue type, diagnosis of disease, and other needs:

Why wouldn't we want to have some method that can gage a set of basic
skills to indicate a level of competency that HT/HTL's should
initially have, in order to enter the field of work that can effect so
many people either directly or indirectly?

And also... Wouldn’t having NAACLS accredited training and ASCP
Certifications serve to gage those basic skills?

And also… Would gaging basic skills have a positive impact on the
quality of patient care and the efficiency of the HT/HTL’s impact on
labs?”

I'm not sure but...this fall… when I complete my NAACLS accredited
degree program, and voluntarily take the ASCP HTL certification exam,
my future employer will be able to expect a certain level of
competency that I hope to have established through training and
certification.

First - HAPPY NEW YEAR! I am very happy to announce that as of tomorrow, 
January 1st, I begin my new position as Florida Histology Consultant for IOP

Second - HistoTALK resumes on Januaary 8th, next Sunday, after a two month 
hiatus.Whew! Needed the break. Our guest will be a very exciting, informative

As you know HistoTALK will be taping interviews for its Sunday shows this year 
at the NSH Convention in Cincinnati. I am scheduling interview times now 
through Wednesday, September 14th. Here are the days and times available at the 
HistoTALK mobile studio -
 
Friday, September 16th after 6:30 pm (maybe)
Saturday, September 17th from 8 am to 1 pm.
Sunday, September 18th from 11 am to 4 pm.
and
Wednesday, September 21st from 1 pm to 4 pm.

Amy - Thank you for your kind words and support. The show is new and the bugs 
are still being sprayed. The show is actually everyone's show. I encourage 
anyone interested in being a guest to contact me. As you can certainly see, I 
have folks on who talk on a wide variety of topics. Not too much on the tech 
side, for sure. Just a fun late night or early Monday morning show. It's not 
meant to be heavy. 


In two weeks I will be attending my own state convention in Tampa, FL. I have a 
list of speakers who I would love to have as guests on the show. I don't want 
to 
mention their names and put them on the spot - Ada, Skip, Kaye, Jerry - I meant 
last names. LOL. I will have the studio equipment set up (Thanks again Sue 
Clark 
and the board) and be doing the interviews right there so attendees can watch 
and listen if they want.
