 In regards to the 88305 cuts, every state/region is different. In NC, medicare 
part B global billing was cut from $99.84 to $66.68, the TC (technical 
component) was cut from $64.60 to $30.83 and the PC (professional component) 
went up from $35.23 to $35.85. These new reimbursement changes started as of 
Jan 1, 2013.
Also, to keep in mind, even though you can bill MC $30.83 for the TC portion, 
they will only pay 80% of what you bill. So the actual reimbursement is only 
$24.67.

I do not know all of the facts, but yes as of July 1st, 2012, a grandfather 
clause was discontinued by Congress. It states: Congress has approved 
legislation that discontinues direct Medicare payments for the technical 
component of anatomic pathology services provided to hospitals under the 
grandfather clause as of July 1, 2012. It makes absolutely no sense. We are 
having the same issues also. If that patient was even seen as outpatient, you 
will not get reimbursement if you are an independent outside lab that did the 
work. There is a 11 page March 2012 article in College of American Pathologist 
that explains it better. Also you can search for Pathology Technical Charge No 
Longer Billable by Independent Lab by JoNell Moore. I will be glad to email 
them to you.

[Histonet] (no subject)

Since there is discussion of formalin and methanol, I would like to ask 
everyone.Why would a vendor add methanol to their reagent alcohol and their 
10% buffered formalin? 

What are the advantages/disadvantages to the tissue especially for processing?

[Histonet] Re: Embedding beads

I started using the embedding beads several months ago as a quick way to track 
who embedded what cassette. Each tech is assigned a number according to the 
number on their microtome. I get the beads from Cancer Diagnostics. You can 
probably find something similar at a craft store.
These come in small round plastic containers that fit easily any where on the 
embedding center. They place the bead in a bottom corner of the cassette when 
topping it off with paraffin.
They have actually saved us time. Once the techs get used to it, it might add a 
few seconds to the embedding. Before, the techs had to write down which blocks 
they embedded. (Very time consuming and often not complete). If I needed to see 
who embedded a certain block, I had to go check that log book. Now I can see 
who embedded it just by looking at the block.

requirements to perform High Complexity Testing. You can have a tech who is 
certified, but they may not meet the educational requirements. That 
person MUST have 60 semester hours, 24 of those in science, 6 hours must be 
Biology and 6 must be Chemistry and documented training in the area of high 
complexity testing. It has NOTHING to with certification, unfortunately.

[Histonet] embedding

 I have produced a very detailed guide for my techs. A little long, it is about 
8 pages including diagrams pertaining mostly to derms. I would be glad to 
forward to anyone interested.
 I would also like to share a book. I met the author and she autographed my 
book at a North Carolina meeting many, many years ago. It is Pearls, 
Preventatives and Anecdotes in Histologic Technic, by Billy Swisher. I have 
found it very useful over the years and I always have my trainees read it.
One of her statements is an everyday quote in my lab. The finished cassette 
should almost give the appearance of already been faced off when it is removed 
from the mold. Orientation is most important,  but if the block does not have 
the faced off appearance, it will be re-embeded until it does! Sure makes 
cutting a breeze and the Docs love our slides!

I work in a CLIA certified lab.
 
I do not know how different CAP is, but here is what CLIA requires for High 
complexity testing personnel:
 

 
TESTING PERSONNEL (42 CFR 493 1489) 
1. Licensed MD, DO or DPM. 
2. Doctorate, master's, or bachelor's in laboratory science. 
3. Associate degree in laboratory science or 
♦ 60 semester hours including 24 semester hrs of medical lab technology; or, ♦ 
60 semester hrs including 24 hrs of science that includes 6 hrs chemistry, 6 
hrs biology, and 12 hrs chemistry, biology or, medical lab tech in any 
combination and laboratory training that includes either: 
♦completion of a clinical lab training program or 
♦3 months training in each specialty in which high complexity testing is 
performed, 
4. Previously qualified or could have qualified as a technologist under federal 
regulations prior to February 28,1992(42CFR493,1491).

testing personnel records!

They did not reference any CLIA regulation. It was just suggested that the dry 
chemicals were not very useful after 10 years.

next re-certification.
I am sure it is best to keep the chemicals current, but is this what all labs 
are doing?

My techs use our Para Trimmer from Thermo Scientific Shandon. It will save your 
hands, but it takes a lot more time than the old fashion hand scraping. I 
prefer to hand scrape. I can scrape at least twice as many by hand than with 
the para trimmer.

You will have to time worself as I did to find out that waiting fo the paraffin 
to melt in order to eliminate it takes 2.5 times more time than doing it 
manually with a pocket knife (as I used). Manually = 32 secs/block vs. 80 
secks./block

I made the switch to Mercedes a while back. I use their clipped corner slides 
(MER7200) because they work great on our slide printer. I also use their Plus 
Starfrost slides (MER7255) for our IHC. The sections stay on great, even 
through antigen retrieval. Probably the best plus slide I have used. Try their 
samples, I'm sure you will be pleased.

MER 7255 - Slides, adhesive, 90, Starfrost, 10 gross - $235. 
I am thinking about changing to these slides from our VWR superfrost
plus slides to save money but I would like to hear how they hold up in
IHC Ag retrieval etc... I have samples from them and will try them on
some IHC but I thought I'd get someone else's thoughts.  If you have an
opinion on the quality of the slides please let me know.

Biocare's Cat Hematoxylin is really good. One real nice feature is that you 
blue your slides in TBS.

I use Dako Mayer's for 5 seconds followed by roughly a minute in bueing 
solution. Gorgeous blue.

[Histonet] Billing question

Hi all!
 
Question for those familiar with all the billing regulations: If you use an 
antibody that is for RUO (research use only) or ASR (analyte specific reagent), 
can you bill the patient for these immunos?

My wife, the best section cutter in the world worked right up to the
bitter end of her pregnancy with our third daughter, dropped an old

Besides a few little nicks that we all get, about 10 years ago after
having worked in histology for almost 25 years I was finally in a lab
with windows!  Early one morning as I was sitting there cutting with my
back to the window, dawn just breaking.  A head popped up outside the
window and the tech facing the window screamed just as I was running my
thumb over the block.  I jumped; the chuck came down and sliced into the
pad of my thumb down to the bone. I ran to the sink holding my thumb and
ran it under water.  It's pretty gross to see the bone in your thumb!
Funny thing, there was no pain until the ER doc filled it full of the
stuff to numb it.  Got lots of stitches and even now it sometimes hurts.

I've heard that tale, but didn't know who did it! 

I had a coworked who cut the pad off his thumb. We went to the ER, where
I learned he had put it in the trash. I went back and dug through the
paraffin scraps and got it - washed it in saline and Betadine.. The ER
dr glued it back on and he has a thumbprint again! 

When I caught myself on fire, cleaning with solvants too close to the
hot plate (the fumes ignited), he was the same guy who yelled, Get the
marshmallows - the supervisor's on fire!. I still get a good laugh
about that! J:)

And it's only Tues!! Trick or treat!!!

Not me, but the guy who trained me.  Remember the old knife sharpeners
that looked like record players?  He was removing the blade (they got
pretty oily) and it slipped.  His natural reaction was to grab for it,
slicing three fingers to the bone.  Made for a good cautionary tale...

 Me - 1982 - working in a S. Florida hospital I sliced open the base of
my thumb on the corner of a big steel blade as I was sectioning. Didn't
even notice it until I saw the blood dripping off my elbow.

I went to the ER and was sitting on a gurney with my hand submerged in a
bowl of betadine waiting to be stitched up. After what seemed like a
pretty long time,  I deftly fell off the gurney onto the floor tipping
the entire bowl all over onto myself.

Upon hearing the clamor of me hitting the floor and the metal bowl
banging around a rush of ER staff charged in and I got prompt service
thereafter. I still have the scar, but I never cut myself again :-)
