My opinion is that it is a good idea because many hospitals are shutting down 
their EM department because of a lack of skilled workers.
So the more skills they have the better for them.

Greetings

I am a regular reader of this list, but from a peripheral setting.

I work in an electron microscopy certificate program. Our students are prepared 
to do either or both certificates in EM, biological and materials.

During their biological training, students are instructed in thin sectioning, 
specimen prep., etc. I would like to get ideas about how valuable adding more 
light microscopy specimen prep, staining, and sectioning might be for these 
students. Would it be good for them to have these skills and knowledge but not 
a histotech cert.?

I thought about doing a full cert. program, but am daunted by the requirements 
to set one up and I don't want to compete with other better established 
programs near by.

Bottom line is I would like to know if teaching basic histo tech skills, w/o 
certification, is a viable path. Could students leverage these skills into jobs 
at non-health care type facilities? Could having these skill help them complete 
a certificated program if they wanted to take that direction later? 

Your input will be valuable to both me and my students.

I always tell my students if you can cut, you can get a job. It appears that 
you school did not properly prepare you for the demands of an average histology 
job.
You need to take every opportunity to work on your craft and the major focus of 
histology is cutting. With 6 to 7 years of experience you are expected to know 
the basics and cutting is basic.

I wouldn't give too much detail information as the histology world is very 
small and everyone knows everyone.

I am in a dilemma. I have been a histotech (ASCP HT) for almost 6-7 yrs. I went 
to a NAACLS school and have a Associate in Science in Histology. In the 6-7 yrs 
I have changed jobs 3 times. All the jobs were graveyard shifts. The first 
place I worked for was Quest Diagnostics and I did a good 3 yrs. The other 2 
places I won't mention and I currently still have a histology job. My problem 
is all the places I worked were factory style lab work and they all did derm 
work. In my career I really only embedded most of the time. I did occasional 
other stuff like special stains both by hand and using Dako Artisan and other 
things like cytology cytospin. But I never got to develop in cutting. My first 
job in quest..I maybe cutted one time for 2 or 3 weeks before they yanked me 
and put me back to embed. My 2nd job put me to cut the last 2 months (full 
8hrs) I was working there. My current job I have been cutting since April 2014 
( but only 2-3hrs in the day and then I embed, I have been here now 1 yr, I was 
embedding most of the time before th cutting started). I was told by my 
director I need to speed up in cutting because corporate is asking why I am not 
increasing in speed. And if I don't speed up eventually then they will have to 
demote me to a lab aid and give me a pay cut. (where I work and the state I 
work in they have lab aids doing alot of stuff without being certified, it 
wasn't like that in the other state I am original from as you have to be state 
licensed and ascp) I sometimes laugh inside my head because before my director 
hired me I told him I don't have alot experience in cutting. 

Now everywhere I have gone...speed is the name of the game. They say they care 
about quality but in the end if you can't put up then you will be put out!  So 
I am just thinking I should just get out of histology world all together. Every 
where I have worked unfortunately have management who believe quantity over 
quality. OR Do you guys think I need more time cutting to develop speed? 
Beforehand I did need a little learning curve to cut and I have gotten through 
that now. It's just the speed that is killing me. And I also see if anyone at 
my work detours me for any reason like for example data entry person from front 
desk ask for missing gross dictation, then that lost time is very hard to 
recover as I am not s fast to recover. I feel I may have to become very 
rude(not help) with everyone I work around in order to stay glued to my seat 
when I am cutting my blocks. One thing I want to say also...until this day I 
never been written up for quality issues and I never lost any tissue while 
embedding. Embedding I am fast as most histotech (1 block a min or most times 
30-45 secs 1 block) with proper embedding techniques demonstrated (tissue on 
same plane, tissue embedded with proper orientation and follow any other 
necessary embedding instructions. ) I just feel I haven't done my time in 
cutting as I did in embedding to become a fast cutter. I don't know if its 
because of working in a derm lab that management won't wait too long for you to 
develop like maybe a hospital lab may do. I was also thinking to find another 
histo job but not mention any of my experience so expectation won't be so high 
and I can get more time to develop. All of this also causes alot of stress and 
anxiety as it gets hard to coop with.  What do you guys think and how I should 
go about with this. I am also not limited to histology. I have expertise in 2 
other major fields that I wont mention because I don't want to be identified.  
I am also in my late 20's. Thanks for reading my post and I await your opinions 
as some of you all are veterans in the field of histology.

Hello all, 
We are wondering harder paraffin makes blocks more fragile.
We have been using Leica's Formula R and on a few occasions  when we have 
mailed blocks out, we receive them back in pieces. Could this be because of 
type of wax we are using? The quality of our slides is great, we just seem to 
be having a problem lately with our blocks breaking after they have been sent 
out  we want to try to prevent that.

[Histonet] RE: Recycled Xylene

We have been using recycled xylene for years and have found that it only 
present a problem on the clean cycle of the processor.
Everywhere we have not experienced a problem, so we use new xylene on the 
cleaning cycle.

I have heard of both methods, currently we put double ribbons on one slide.
The only time we have repeats is if the initial cut is superficial.

One of our pathologist has requested a change in the way we cut GI specimens.  
We presently put three levels on one slide. Exp.  we face the block, cut a 
ribbon and take one section of that ribbon and catch on the slide. We then 
progress into the block and repeat the process until we have three levels on 
the slide.  The new suggestion is to simply face the block, create a ribbon, 
and take three to four section of that ribbon and that's it no more progression 
into the block.  The thinking is that the Pathologist will see what they need 
to in those levels, and if not they will call for deepers. I have never cut 
like this and just wanted to hear some thoughts on this method.

The DAKO unit is very good but we are being forced to sign a reagent agreement

We have an electronic requisition that looks like the paper that we use in some 
units. The ORs didn't use it because they didn't have good access to computers 
from each room. But the GI labs and Breast Health used it successfully.

A Pathology is ordered and fields pop up that the info is typed into. Two 
copies of the req prints - one in Histolgy with a bar code and one in the unit 
that is sent down with the specimen. That has served us well for several years.

Unfortunately, with our partnerships with Emory and the upgrade to the system, 
we have to go back to paper. Maybe, someday..

Before you holler political and blame the candidates, ask yourself who was hurt 
most by POL's?
Large reference labs.
With this change they will get back the business because it will not be 
profitable to establish a POL.
Also they lobbied for and increase on the PC, reference have pathologist, most 
POL's don't.

This is disturbing news. As an employee of an in-house lab (which started in 
1996/1997) that does mostly skins, GI biopsies, and outpatient surgery 
specimens I'm pretty disheartened to hear about the 88305 issue. Melanoma 
excisions, prostates (even lower block # cases, we don't always get 12), breast 
biopsies, and other more difficult cases can be a lot of work on both the 
professional  technical end of things.  

As for prostate biopsies, CMS has already lowered reimbursement with the G 
codes. This is despite the wording that they are for saturation biopsies. We 
rarely have saturation biopsies, but Medicare denies us the 88305 charge if 
more than 5 specimens. 

Other insurance companies tend to follow their lead  after a little time.  I 
believe reimbursement is 50-75% less for 5-20 biopsies, but don't quote me on 
that.  I expect we may see the end of saturation and multi-container prostate 
biopsies in the near future. 

Another issue for many outpatient labs in my area is that larger insurances are 
requiring their patients to go to large multinational labs.  We cannot accept 
many PPOs or Medicare replacement plans because of this. 

I feel it can be a disservice to the patient because they do not get the same 
personal, local service with good turn around times. Even my insurance requires 
me to go to one of these labs where I feel inconvenienced and frustrated at the 
wait time required to submit my sample and get results to my physician.

 Besides  what kind of impact on morality of the techs, back problems and 
 carpal tunnel syndrom is laying ahead for the cutter after cranking the 
 microtome repeatedly that many blocks without a break.

What was the initial purpose for starting the Histonet, I assume it was to 
offer a forum for Histotech to get assistance with problems they may encounter.
It offends me when there are those that feel they can control what is posted, 
the histonet is for the group not a few individuals. 
Histocare may be excessive in their postings but if it does not apply to you 
don't read it.
They are no different then the other staffing agencies posting job listing 
because in turn they are promoting their company.
Both provide a service, again if you don't need their service don't read it. 
However someone may need their services and they should not be excluded.

Your messages seem to have a very petty tone and not respectful of the nature 
of the work we do. It's almost as if you are not even of this profession.
I care about all those people who go to doctor to have a million tests done to 
see what's wrong with them and have to wait days and weeks before they get any 
kind of news to settle their nerves or some glimmer of hope. I want to minimize 
that inconvenience as much as I can.

I am a real person first, a histology professional second on the other side of 
this email just like there is a real patient on the other side of that slide.

You only care about HistoCare advertising on one of the few forums histology 
professionals have to exchange thoughts, ideas, and RESOURCES. If the best you 
can add to histonet is criticism, it doesn't need you or those like you.

This is a small community with very high turnover, low job satisfaction, some 
employed individuals with marginal or inadequate abilities, and little interest 
in others to want to break into this profession. Heck even the ones that's been 
his field for a long time express dismay, let alone the newbies who can't get 
the proper support from their own supervisors! Good grief!

Those who rely on histonet for advice and ideas should also be able to search 
for dependable lab support and hope that there is a good resource for them to 
call upon.

Please do me a favor and respectfully bow out and resist the urge to respond 
further as I have no interest in debating. I'd rather spend my time being 
productive.

There isn't a policy specifically stating i can't make HistoCare available for 
those who may search for assistance. Lets not forget Histonet is a courtesy to 
us all for interests in histology and what it is not, is a vehicle to complain.

I was initially open to your suggestions for alternatives and offered you the 
opportunity to respond with solutions acceptable to 'you' but I see you would 
rather be a complainer than a helper.

Try using histoscreen cassettes and that will eliminate the need for wrapping 
the tissue.
If still insist on wrapping, ensure that they are dipping the tissue paper in 
formalin prior to putting the tissue on the paper.
Also at embedding, if the tissue is stuck to the paper, take a blade and put it 
against the tissue and gently push to loosen the tissue, repeat this the length 
of the tissue.

Donna, try the new SecureSette from ThermoFisher. They enclose the sample at 
grossing and do not require manipulation at embedding - just add wax and cool. 
Very interesting product. We tried them and they work well for some samples. 
However, they only accommodate straight needle bx of 2cm or less. Our prostate 
cores were 2.5 cm at times so it didn't quite fit.

I would have to agree with Rene. I have had two VIP's that were replaced after 
they were over 20 years old and they both were still running. I currently have 
3 VIP 6's, thus far the only problem I've experienced was the fluid level 
sensor malfunctioned once.

At this moment, just like in automobiles, all tissue processor can give you a 
good service and your selection should be based on your budget.

I studies at the famed AFIP while in the military and have two degrees and 
never thought I would stay in histology. 22 years later I am still in the field.
I don't think OJT is a bad route it has often generated some of our best techs 
but I am a firm believer that new techs need to go through a formal program or 
have a B.S. degree. I feel this way because most facilities require a degree to 
move up in positions of authority. I have a wonderful tech that went through a 
formal associates histology program but does not have a B.S. degree, my 
facility will not allow me to promote her to a assistant manager position. So 
if you are a young tech out there and you have dreams of moving up into a 
leadership position, get a B.S. degree in something. That is why med techs are 
held in higher regards and why most assistant director and director positions 
come from the clinical lab.

Like everyone else, I was going to keep quiet, but I can't.
I am sensitive to those downgrading us who took the OJT route.
I did the OJT route, had a BS in Biology, tried for Veterinary School, but that 
wasn't so.  What do I do with all of this Science?  As part of my Pre-Med 
curricula had to take histology, loved it.  Had no idea that it was a paying 
field and such.  Had I known, I would have applied to a school as a backup.  It 
took me a while, but when I was hired as a tech, I did not excel quickly 
(Cheryl remember my mistakes), but I learned and had patient coworkers.  
Move on down the line several years and another coworker questioned my 
knowledge of the chemistry behind a stain, and all of a sudden all of my 
organic and biochem can running out of my mouth.  I didn't realize that I 
really knew all of that.  I had always felt a little disadvantaged because I 
did not get formal training through a school, just studied and passed the HT 
with the help of my coworkers (shout out to  LSU VetPath).  
There is nothing wrong with OJT for Biology majors.  They should have the basic 
background to understand the chemistries and processes behind why we do what we 
do.  With the modernization of technologies and procedures in the histo lab 
some formal education is needed.  There are many ways to receive this 
education, it can be online, or in person.  It all depends on the learner.
To overcome the stigma we should continue with some of the things that are now 
in place to stabilize the training of our successors (yes we all are going to 
have to retire one day).  A continued push for formal training, promotion of 
the field, professionalism by our colleagues, and respect from the customers 
(pathologists, patients, and gen lab personnel). 
In order to facilitate change for respect, we must first present a unified 
front.  
I know good techs with no certification, I know bad techs with certification.  
We all do.  
Last year I celebrated my 20th year in histo and never thought I would be where 
I am.  I never wanted to do research, and didn't like management (don't like 
telling grown folks what to do), but I love teaching.  It helps me to learn the 
theory behind what I do and apply it better.  There are so many people who can 
do histo whether it is routine, special procedures, or management.  
We shouldn't look down on those who took the OJT route, sometimes they just 
don't know about a formal program (like me).  You never know where the next 
great manager, director or tech is coming from, so don't count them out.

Jon There is a route with associates and training I believe. 
Of course I can't speak for the BOC, and I am sure that you want to help your 
employees as much as you can. I do see your point about the similarities in 
tasks. My thought would be that the exam eligibility states that they have to 
have recent experience in fixation, embedding, microtomy, and staining 
(histology) and the associated theory knowledge. EM is on the exam study 
topics, but also with the theory/experience for all those routine histological 
techniques, is how I read it. Take a look at the exam outlines, that should

This is an issue with the cassettes not your printer or ribbons.
If you take a straight edge and put it up against the edge of the cassette you 
will see that he cassette is concaving causing the styles to loose contact with 
the cassette.
It is more of a problem at the middle top edge of the cassette. It is only with 
the histoscreen cassettes and I bet Thermo is telling you that this is the 
first they are hearing about this problem.

I am looking for help with a cassette printing problem that we are 
experiencing.  We use the Thermo Histo Screen cassettes and the Surgipath  VCP 
2001 cassette printer.  Case numbers are quite smudged or missing completely 
when the batch has finished processing.  This is only happening to the 
HistoScreens because we also use a routine cassette from another manufacturer 
and there are no problems with print quality.  I have switched lot numbers of 
the printer ink ribbon and continue to have problems.  It is not happening with 
every cassette on the batch so I do not believe it to be the printer itself.  
Is any one else experiencing printing quality issues with these cassettes?

I agree with Joe, Mopec makes a very good product. In the last 4 years I have 
purchased 6 MB6000 and our pathologist are very pleased with them, especially

A laboratory I'm doing some work for is planning to remodel within two
 months. We see about 5,000 specimens a year, with a moderate number of
 hospital major specimens including placentas. Currently we have no

[Histonet] Leica Bond III

I want to poll Leica Bond III users to see if they are able to successfully run 
EBER Insitu on their unit?
We continue to get inconsistent staining and inconsistent background staining.
I have been told it's a pH problem, it's a tissue processing problem but we are 
able to do the stain with great results by hand. 
I welcome any and all suggestions

[Histonet] HM525 Cryostat

I purchased a new Microm HM525 cryostat about 3 months ago.
The unit overheated causing some of the internal parts to warp.
Has anyone else experienced this or similar issue?

[Histonet] Bond III

We have been using the Bond III for about 4 months and have starting 
experiencing staining on our negative control slide.
We inserted and additional blocking solution and it help for the IHC stains but 
we are still experiencing the problem the with insitu.
Has anyone experienced this problem before and how did you solve it?

Is anyone else out in histoland having issues with the Leica Slide labeler not 
printing 'permanently' on their  recommended slides.   We are using the 
recommended clipped corner, extra adhesive slides for our HE but every so 
often the print rubs off after a couple of days.   The slides then need to be 
paper labeled.   I am afraid that over time the slides will not be readable.

If you are using one of the older film coverslippers there is a roller 
following the application of the film.
Make sure that roller is still in the correct place or that one of the springs 
is not missing or in need of replacing.
The job of that roller is to squeeze the air bubbles from under the film.

1) What happens to the money if I were to submit my transcript and my fees for

for the course ID have to contain BIO or Chem?
3) Can the courses taken be similar? Example: School A: I took Chem10: Intro to 
Chemistry  School B: Chm:3 Chemistry basics. If these classes are 
interchangeable between schools do they still count or does it have to be a 
more difficult chemistry course for it to qualify?
4)Is there anyway to check if you do qualify before paying exam fees?
5)Do you have to pass the class with a C or higher? I took a BIO class years 
ago and received a D, but I was given the credited hours for the course.


Route 2 for HT certification for ASCP
At least 60 semester hours (90 quarter hours) of academic credit from a 
regionally accredited college/university, with a combination of 12 semester 
hours (18 quarter hours) of biology and chemistry, or an associate degree from 
a regionally accredited college/university, with a combination of 12 semester 
hours (18 quarter hours) of biology and chemistry, AND one year full time 
acceptable experience in a histopathology (clinical, veterinary, industry or 
research) laboratory in the U.S., Canada or an accredited
laboratory* within the last ten years.

100% is unrealistic unless your staff never calls in sick, take vacations, your 
lab never gets research projects that are needed today and you are always fully 
staffed. Realistic reside between 90 and 95% which is obtainable but not overly

Charlotte, NC from 1993 (yuck).  Their goal was 90% of the slides on time which 
they correlated to 2 days or less late slides per month. Their goal was also to 
have all the slides out by 11am..

Could I get feedback from you guys about what you expect for this PI goal? I'd 
like 100%, but sometimes staffing just doesn't stretch far enough!!

Fellow Histologists:

This is my first time using Histonet but I need to ask if anyone has started 
having problems with their DAKO Artisan kits recently? We have started having 
leaking kits for Gram Lot # 10043062 and Expiration Date 4/30/2011, and plunger 
issues with the Iron Lot # 10047616 Expiration Date 10/31/2011. We have also 
found that the GMS kits Lot # 10049354 and Expiration 11/30/2011 we have 
received in the last two shipments are from the same lot but they do not work 
with our validated protocol. 
What is strange is that the previous GMS kit worked on the same control that we 
are using now, but does not work with the new kit, but when stained with 
PAS-Fungus we see the same fungus as we saw with the previous GMS kit. I think 
it could be something with the shipping method but wanted to see if anyone else 
has seen a difference.

I recently ran a PAS/D stain and had some issues with it.  Both with and 
without slides came out looking the same so I'm guessing my digestion step 
didn't work!  I used a Malt Diastase solution (0.5g to 500mL water) for my 
digestion.  This is the procedure I used:



1.Deparaffinize and hydrate to water.

2.Place the sections labeled “with” in diastase solution preheated to
37˚C for 1 hour.  Hold the sections labeled “without” in distilled water.

3.Wash in running water for 5 minutes

4.Place all section (with and without) in 0.5% periodic acid solution
for 5 minutes

5.Wash in 3 changes of distilled water

6.Place in Schiff reagent for 15 minutes

7.Wash in lukewarm tap water for 5 minutes (immediately sections turn
dark pink color).

8.Counterstain in Mayer’s Hematoxylin for 3 minutes.

9.Wash in tap water for 10 minutes

10.   Dehydrate starting with 95% ETOH, clear, and coverslip.



I am wondering if my solution possibly got too warm in the oven and hindered 
the enzyme activity, or is it possible I left it in too long?  Any tips would 
be much appreciated!  Oh, and I have about 300 slides to stain, so spitting on 
them is my last last last resort!  Haha!  Thanks in advance for all your help!

[Histonet] RE: Hematology stainers

I thought I was the only histology lab that got stuck staining bone marrow 
smears.
I started with staining smears manually, then went to the hematek and finally 
discovered the Wescor stainer. The wescor stainer has really brought 
consistency to this area. The only potential problem is you have to keep up the 
maintenance, which is not extensive. I highly recommend the wescor and it is 
very nicely priced.

  If given the choice- what would be the best Tissue Processor to get for a 
small Path Lab.  We have always had VIPs. Ours is about 16 y/o and we need to 
replace it and use it as a back up or for small biopsies. I was looking at the 
new VIPs and our medical repair people are very happy w/ the VIPs in the past.  
I am looking at VIP 5 and 6.  Do not need anything too fancy.  Just want a 
workhorse that has little or no issues and is user friendly.

The problem with the Nexus is that you can not do silver and AFB on the same 
run.
We use the Dako artisan with very consistent results and it is very easy to 
use.

I agree most of your times are too long, but you can still get sections if you 
put water on your ice block and allow the tissue to rehydrate a bit or put a 
little ammonium hydroxide and water on your ice and sit your blocks to be 
section in this solution.

Having some experience processing and embedding rodent tissues myself (by 
hand), I would say that you are over-dehydrating the tissues. Try cutting back 
the alcohol incubation times to 30 min or even 10 min each.

After applying the ink to the tissue, we take weak acetic acid solution and 
blot it over the ink which helps it adhere.

As I look through and monitor questions, it is apparent that our field is 
declining. These are very basic questions not about special stains or IHC 
stains but basic histology that should have been taught in histology 101. My 
fear is that as we get older and leave the field, who and what will be left to

My first reaction to the what is happening to our field, was WOW.  It seemed 
unkind to me, as if they original poster should not have asked these questions. 
 With further reading of the replies to this post, I am not so sure it was an 
unkind response, but one of potential shock and dismay to the idea that labs  
might not be producing the quality work that most of us employ on a daily basis.

Amy, in answer to your questions, I will echo some of the sentiments that I 
have read here.

1. Facing of blocks.  We use one blade to face blocks and another, new blade 
when we do our actual sectioning.  In my case, I face as many as I can, knowing 
I am going to toss that knife when I am done facing.

2.  Soaking of blocks.  After facing my blocks, I will put them on a cold, 
damp, ice cube tray.  This will achieve two purposes for me, a) to chill the 
block and b) to introduce moisture into the faced tissue.  If I get a block 
that is particularly dry or hard (some calcified tissues for example), I will 
face them, put them face down on my waterbath and allow the hot water to 
penetrate into the tissue for 15-45 seconds, depending on the block.
After cooling on the ice tray, they usually cut like butter for me.
Typically, my blocks are not on the ice cubes for more than 15 minutes.  As I 
cut some, I will rotate the blocks around the ice tray, adding more warm
blocks for cooling.

3.  Freeze spray.  I hardly ever use the freeze spray.  About the only time I 
find that I need it is if I have a particularly fatty tissue and it doesn't 
want to section.

4.  Tissue processor changes.  This is definitely something that is site 
specific. In our case, we do base it on volumes.  If we have a small volume of 
our little biopsies, we might not change the machine weekly, but every two 
weeks.  Generally our large specimen machine is changed weekly.

If our Pathologists need a second opinion we eat the charge.  If it is 
requested by the clinician, we request that he/she be billed.   If it is the 
patient we bill the patient (or ask the consultant to bill the patient)  All of 
this depends on patient status/medicare etc.

I would like to know how other pathology labs are billing for consultations 
that are sent out by pathologist for second opinion.
Our process is to notify the physician's office that a case is being sent to an 
expert and, if required, the physician's office is responsible for obtaining 
precertification if the patient's insurance require it.
Unfortunately, this has caused us a number of problems.  If the consultant is 
not in-network, the insurance does not cover this expense, and the patient is 
responsible for the bill.
Is it a better option for the hospital to receive all bills from consultants, 
and in turn, the hospital will bill the patient?  If so, are there problems 
associated with this?
Or are other laboratories having the consultants bill the patient's insurance 
directly, and if so, are they experiencing similar problems?

We use the accu-edge low profile microtome blades (exclusively, as they work 
best for us).  We noticed an oil/lubricant of some sort on the blades in the 
last box that we opened (we checked one other box and it has the same, although 
none previously did, or at least we didn't notice it).  The oil is giving us 
problems with our sectioning.  Has anyone else noticed this, either now or in 
the past?

Also, does anyone have a suggestions of other blades we should use?

[Histonet] Sakura Tape coverslipper

Has anyone been experiencing a brown cornflaking artifact on their slides after 
being coverslipped.
I was told by Sakura that it was do to the slides drying out when transferred 
from the stainer to the coverslipper.
Which can't be correct because on the new sakura coverslipper there is a 
holding area which is filled with xylene so if the tissue dried out it would 
rehydrate before being coverslipped.

I think that the propidium iodide will stain the nuclei or DNA, similar to 
DAPI, so this mounting media will counterstain the nuclei and its visible in

of your stain.

mounting medium.  By mistake, today they were coverslipped with a mounting 
medium that we use for coverslipping our FISH slides.  This mounting medium is 
described as Mounting medium for fluorescence, with propidium iodide.

Will this make a difference when reading the slides??

Just talked to Jim at CAP and he said the term Processing still can be the 
transfer of tissue from one container to another (cassette) to be processed in 
the tissue processor.  If there is any analytical thinking involved (dying 
margins, measuring...) the process becomes Grossing and falls under the high 
complex rule.

If individuals other than a pathologist or pathology resident assist in
gross examinations, do such individuals qualify as high complexity testing
personnel under CLIA regulations?

NOTE: The laboratory director may delegate the dissection of specimens to
non-pathologist individuals; these individuals must be qualified as high
complexity testing personnel under CLIA regulations. The minimum
training/experience required of such personnel is:

 1.  An earned associate degree in a laboratory science or medical
laboratory technology, obtained from an accredited institution, OR
 2.  Education/training equivalent to the above that includes at least 60
semester hours or equivalent from an accredited institution. This 
education
must include 24 semester hours of medical laboratory technology courses, 
OR
24 semester hours of science courses that includes 6 semester hours of
chemistry, 6 semester hours of biology, and 12 semester hours of 
chemistry,
biology or medical laboratory technology in any combination. In addition,
the individual must have laboratory training including either completion 
of
a clinical laboratory training program approved or accredited by the 
ABHES,
NAACLA, or other organization approved by HHS (note that this training may
be included in the 60 semester hours listed above), OR at least 3 months
documented laboratory training in each specialty in which the individual
performs high complexity testing.

There is about three brands of paraplast and they all cut differently. In one 
of my labs I use paraplast plus and it ribbons very well however the blocks 
have to be colder then if you are using TissuePrep from Fisher.
Leica may have sent you one of the other types of paraplast. (paraplast, 
Paraplast Plus, Paraplast Xtra)

It depends on your volume and how long you maintain your slides. Because you 
are a children's hospital you keep them longer.
The Leica brand is very troublesome in high volume situations. I personally 
prefer Sakura tape for the speed and fast drying time.

We use the Leica CV5030 the advantage of it is that it can be attached to their 
Auto stainer providing continuous workflow as rack and coverslip 30 slides per 
run, without the need to physically transfer slides rack between the 
autostainer and coverslipper or used as a stand alone coverslipper if needed, 
though at time it can be problematic.

For a start glass cover slip must be kept at 37 oC prior use otherwise they 
tend to stick to each others.
Also does not recognised all the slides type unless they are Leica one, or the 
coverlslipper as been calibrated to use a specific slides type.

In the past I used the Sakura Acetate film coverslipper, and their were no 
major problem with it though remounting section was a very tedious process.

After you stop laughing seriously I need some help here, apparently I got my 
fingers in some silver nitrate yesterday and touched my face under my nose over 
my lip and now I have black spots that won't come off.  I have done this on my 
hands before but never on my face.  So far I have tried soaking a cloth in 
hydrogen peroxide hoping to bleach it with no luck, I even tried putting some 
gold chloride on it to see if I could tone it down, to no avail.  Any ideas?  
Make up only goes so far and lasts so long.

states this should not be done.  I have been looking for something that says 
the bottles should not be pre-labeled before the patient is seen, but have 
unfortunatley have not been able to find any.

Once upon a time in a far away land, we used to boil our embedding molds in 
boiling soapy water, over an open Bunsen burner, followed by an alcohol rinse 
then air dry. One time the fire alarm was activated and we had to evacuate the 
hospital. We were out there quit awhile. When we received the all clear to go 
back into the hospital, I was the first one back in the lab and the fire 
department was there, looking into our pot that had boiled out and was smoking 
up the lab. This wasn't the cause of the first alarm, but it did set off the 
second.

Dear Histonet Friends,

I just wanted to share an incident we recently had with an old paraffin pot. 
One of my techs came in on Sunday to embed some tissues, went into the 
processor room and smelled something burning. He noticed our old paraffin 
pot had charred looking labels on the outside so he went over, opened the 
lid and poof!!! the pot went up in flames. The thermostat had gone haywire 
and heated the paraffin to flash point. Opening the lid gave it the oxygen 
it needed to ignite. He triggered the alarm, made the appropriate call and 
then put it out with an extinguisher. Of course it kept re-igniting because 
he could not get behind it to pull the plug. The fire dept finally was able 
to get it pulled out and unplugged. Needless to say the tech was shaken and 
the room was a mess. I applaud his courage and am not sure I would have done 
the same. There was enough xylene and alcohol on the 4 processors to cause 
quite an explosion but everything else was in a flammable cabinet. I was 
wondering if this type of thing had ever happened to anyone else?? Needless 
to say, we have de-comissioned all old paraffin pots and will order only 
those with over temp safety features. I guess I just wanted to remind 
everyone that fires can happen in the lab and do probably more often than we 
hear about. This was the first time for me and I have been in this business 
for over 20 years. Take care and be safe.

My lab is Dermatology research and processes skin samples only.  I have tried 
the UltraLight Fixative by Pacific Southwest and found that it works quite 
nicely .  My HE are picture perfect, and my receptors on my IHC's stain well. 
I use it because it doesn't seem to be as harsh or drying as 10% Formalin.

We have had this discussion many times in our institute and would like some 
outside thoughts.  Is there a charge/cpt code that is acceptable for 
the technical aspect of a peripheral smear?   Hematology doesn't stain
our
slides.  They make the smear and we accession, stain and coverslip, etc.

But the pathologist is the only one actually interpreting the smear.

We don't need anymore pathology laboratories.  What we need is support of 
existing laboratories, especially hospital-based labs.  GI and GU physicians 
are Cherry-picking the technical revenue that should be going to hospital 
labs.  Let's reform health care; make it more efficient and less expensive.  We 
don't need to be putting more money in clinicians' pockets.

Allison we use tissue prep paraffin and it comes in cardboard containers, we 
save the empty containers to pour the waste paraffin in. Once it solidifies we 
dump it in the biohazardous container to be disposed of.

Good Morning Histo Land, I was just informed that on my last test sample for 
formaldehyde vapors was very high.  Right now I have to wear a respirator while 
grossing and assisting grossing.  We are looking at why the Mopec grossing 
station is not working in fact we have not had really good results since we put 
it in.  I'm wondering if anyone else is having the same problem or know of a 
better solution.  Our grossing station is the one that is self contained with 
filters. Also is anyone using with good result a formalin substitute? If so 
which one?
