I've been told that the HPV testing will be discontinued as of end of year. HPV 
expresses the P16 gene but so does other type of cancers associated with HPV.


It is possible to have a P16 pos and HPV negative test.
But is it not possible to have HPV pos and P16 neg.  if HPV is positive then 
the P16 is also positive.


So the P16 cannot be used to replace the HPV testing can it?

Hi Chris,
CAP actually has a specific checklist question on Negative Controls.
The question is ANP.22570 and if 
I remember correctly it is quite extensive. Most of the CAP questions are 
about 7 to 10 lines. The Negative control question is more than a page long.
Get your hands a copy of the most current checklist for anatomic pathology -
That will include Immunohistochemistry.

By the way, the rule is that a negative control must be included with each run 
and there 
is much, much more.

Histonetters,

I have been made the IHC lead in a fairly large laboratory
that is seeking to become CAP accredited. I am definitely a newbie when it
comes to CAP. A question was brought up today about how we do our negative
controls. Specifically, should we run another negative control with any IHCs
that need to be repeated or if we should run another negative control if at a
later date a pathologist orders additional IHCs on a case. With our current
protocol, we run one negative control per detection kit used, per case.
Consequently, most cases end up with just one negative control slide. We do NOT
run another negative control if we have to repeat one of a group of immunos or
if a pathologist orders additional stains at a later point. I am wondering if
this is good practice, and acceptable with CAP. I would love to find some
literature that could help me make a case either way on this. Actually, any
literature or pointers regarding IHCs and becoming CAP accredited will probably
help save what little hair I have left.

Why do people rehydrate after dewaxing?
Both are unnecessary, under usual conditions.

Methanol was used in the early days as a peroxidase blocker by itself.
The combination was devised as a belt and bracer method.
As you stated, you use aq H2O2 effectively.
So do I and many others.

However, for unfixed frozen sections, I would never use aq H2O2, if I wanted 
sections remaining on my slides

After dewaxing, rinse sections in 4x 100% alcohol, rinse in water, endog. Px 
block while you make up you AR solutions

When I asked them that question, they said that it was usually at the 
discretion of the pathologist in charge.  But instead of taking my word for 
It, email them  and when you receive their response, print it.

I have a question regarding a new CAP checklist question
dealing with ER/PR validation.  The new question is ANP.22976 ER/PgR
Validation and states: 
  
If the laboratory performs immunohistochemistry for
estrogen receptor (ER) and/or progesterone receptor (PgR) as a
prognostic/predictive marker on breast carcinoma, the laboratory has documented
appropriate validation for the assay(s).  
 Note: Initial test validation should include a
minimum of 40 cases (20 positive and 20 negative cases) for
FDA-approved/cleared tests; laboratories should consider using higher numbers
of test cases if a Laboratory Developed or Laboratory Modified Test is to be
validated.  Validation should be performed by comparing the laboratory's
results with another assay that has been appropriately validated. 
Acceptable concordance levels are 90% for positive results and 95% for negative
results.  If significant changes are made to the testing methods (e.g.
antibody clones, antigen retrieval protocol or detection system) revalidation
is required.  
  
Our original validation was done in 2005 and was performed
with 20 cases.  Do we need to revalidate to be compliant with this
question?  Should we start from scratch or just do an additional 20 cases?

I would be worried too.
Wearing gloves in the lab is always a good practice and 
I believe, a requirement in my lab whenever handling reagents or 
the possibility of coming into contact with reagents.

I am asking this because in my job we mount slides by hand, and my
coworkers don't like to use gloves because it leaves a residue of latex in
the back of the slides. I really don't feel comfortable mounting without
gloves because I heard that xyelene can cause cancer. Some people I know
personally has told me that this is not possible, but I read in some places
that xylene could a possible carcinogen.

I have already gotten contact with xylene in my hands a couple of times and
I am worried.

As Tunde says, the Checklist 7/11/2011 question ANP.22570 does explain that you 
can do as your pathologists suggests, 
BUT there are rules to be followed if that is what you will do.

A negative tissue control must be processed for each antibody in a given run. 
Any of the following can serve as a negative tissue control:
1. Multitissue blocks ...
2. The positive control slide or the patient test slides, if these slides 
contain tissue elements that should not react with the antibody.
[this is the one you want to follow]
3. A separate negative tissue control slide.

The type of negative tissue control used ... SHOULD BE SPECIFIED in the 
laboratory manual. ...

I use Dako's Myo-D1 cat. M3512. Use it at 1:25
I use Dako's labeled polymer kit, Envision + Mouse.

Use Trilogy (from Cell Marque) in a steamer. That means:
Start the steamer and put a two Coplin jars in filled with Trilogy.
Place all your slides in 1st Coplin jar for 30 min. right then, at RT.
The temp will go up to 95 degrees as the Trilogy acts as a deparaffinizer.
Then put he slides in the 2nd Coplin jar for 30 min. and Trilogy acts as AR.

Our staining was also light.  I called CAP and they said to leave the
result area blank, mark the bubble for unable to analyze  (11), and to
write an explanation on the last page.  My pathologist is questioning
why we wouldn't mark specimen unsatisfactory (33).  What are others
that had light staining doing and/or what have you been told by CAP?
CAP also told me that they didn't have any replacement slides to send
out.

ANTIBODY LOTS
I have a Dako and I have a Bond too.
For the Dako I use my positive control. I use 3 slides.
One is treated as the negative control - no antibody
One is treated with the current lot 
One is treated with the new lot.
I keep all QC slides together.
I keep the 3 slides together.

For my Bond,
I use 2 slides of my positive control
When a new lot comes in I run the new lot and the negative control.
Then I compare them to the previous lot that I ran in the past.
I keep the slides in chronological order - the previous lot goes back with
The slides from that date. The NEW Lot slides get filed with today's date.

DETECTION SYSTEM LOTS
For the Bond I QC the detection kit using an antibody like Ki-67, it works well 
and always shows good nuclear staining.
I indicate the received date, and QC date.
I run the QC on my Ki-67 positive control.  I use 2 slides.
One is treated as the negative control - no antibody.
One is treated with Ki-67.
I compare these to the previous Detection kit QC slides

I created a written procedures for all of the above.

Yes, you should run negative controls with each run - today and tomorrow.
Something can happen in tomorrow's run so tomorrow's negative control
will have different issues from today's negative control.
The negative control should go through everything that your
slides go through, whether it's today's great run or 
tomorrows electrical black out or whatever mishap.

Whenever you run a test you must have a positive control and a negative control.

[Histonet] Any problems with Bond Max Heater Assembly (SSA)

That too has happened to me and the process that you do to remedy it,
works for me too. (take off coverslip properly, re-apply DAB, and you
get a reaction.) My experience is that the slides are not shot, as described 
by another Histonetter.
There was definitely something else going on.
I work with the Dako too.
Is it possible that the Autostainer was not programmed to apply
antibody in the crucial zones?
Look into that possibility.

I need to pick the brains of the IHC experts out there in histoland. I run
IHC on the Dako autostainer. On one of our runs there was not enough DAB to
dispense on all the slides. My tech dehydrated the slides and cover slipped
them. We printed the stain log on the run and confirmed that the DAB was not
dispensed on three slides. The cover slips were removed and the slide were
soaked in xylene to remove the cover slipping media. The slides were
rehydrated and DAB was applied. There was still no reaction after 10
minutes, The DAB was reapplied for up to 30 minutes with no reaction. Can
anyone out there give me a reason that the slides would not react after the
reapplication of the DAB. I have done this before when the DAB solution was
short on the run and it worked. I can't figure out why this didn't react. I
called tech service at Dako and talked to the supervisor, she agrees with me
that the reapplication should have worked. I checked the run log and the
only missing step is the DAB. The antibodies with the missing DAB have their
own controls on the slides. Any thought out there as to what the problem is.
I have had the PM done on the Dako which should resolve the problem
according to Dako. There has not been a problem since. This mystery will
drive me crazy until I can figure out why. I could use help. Thanks in
advance, I appreciate any input.

I use it on formalin fixed paraffin embedded human tissue on my Dako.
I use Dako's Target Ret. Soln in a steamer for 40 minutes
Incubate 15 min with it at the dilution of 1:25.
Then I use Dako LSAB2 kit.
It works fine.

Has anyone worked with the MBP (human eosinophilic major basic protein) 
antibody? If so, could you please provide the source of where you purchase it? 
We would be using it on paraffin sections with Dako instrumentation.

Hi All,

Our pathologists have made a complain that the staining of slides have 
been a problem.  Same antibodies which stain one day does not work the 
following day.  This has been going on for a few months.  When we repeat 
stain they either work or not work and we have been sending them to 
reference labs. 

Our tech support person was here and told us that the slides ( fisher plus 
slides) have been a problem with some of her customers.  Meanwhile our 
surrounding labs which are using the same slides have no issues.  She 
showed us how the reagents were not spreading properly on the slides. 

Is anyone confronted this issue?  If so,  how was it solved?

Yes, it's true, only MTM labs sells it and they
Sell it as a kit and they also 
sell it along with a negative control reagent.
That's how I buy it.
They have a license or something and no else can sell it now.
Their cat# is 9518 and I think it's called the 
CINteck Histology V-kit,
(you should double check with them.)
They will make you open up an account...
