[Histonet] Pathology Transcription

I have a couple of questions...
 
One, what do you all use for resources for finding a good pathology 
transcriptionist? We've done the
typical things but have been unable to fill our open position with a strong 
candidate.
 
Second, I would like to cut down on the amount of paper shuffling our 
transcription department does.
It's unbelievable! The transcriptionist handles the paperwork about 4-5 times 
before it is stacked and prepared by another area for scanning. Any 
suggestions? We cannot go completely paperless at this time due to the
upfront expense but I am interested in trimming down the handling that is done 
in transcription so the 
transcriptionist can focus more on typing rather than hunting requisitions. 
Part of the issue is the pathologists
desire to hold each requisition in their hands so I am fighting that battle as 
well.

As we reuse block file drawers, we run a piece of bold colored tape (as wide as 
the front of the drawer) across
the front. We use a different color for each year which helps quickly 
distinguish one year from the next when pulling or filing blocks. Works very 
well here and is inexpensive.

 First time poster. My institution has a 25 year retention on paraffin blocks. 
 The 1989 batch was just destroyed and the storage containers were returned to 
 me. They appear to be the same size as the current sekura tissue block 
 storage units we use. Does anyone know of a commercially available label 
 overlay for the drawers? I could cut down adhesive labels, but wanted to see 
 what was out there on the market.

[Histonet] Sakura Cryostat

Is anyone using the Sakura Cryo 3 Plus cryostat for routine frozen sections?
A nearby facility is inquiring but I have no experience with it myself. Any

Our pathologists dictate their requests and once transcribed, they are printed 
on the histology printer by transcription. This allows us to track through the 
LIS who transcribed and sent the request to the printer and also which printer 
the request was sent to. Discrepancies usually require retraining and are 
easily resolved.

 Currently, our histology lab is located on the opposite side of the 
 laboratory from the pathologists. They each have an excel sheet, with scanned 
 physician signature, on their desktop for use when they order special stains, 
 IHC's, referral testing such as cytogenetics or EM.
 They fill out the case number, block, stains or testing required and print 
 the request to the Histology printer. This form is used for cutting at the 
 microtomes and completed and initialed by the tech completing the stains and 
 entering the charges into MT. It is then given to the pathologist to sign as 
 an acceptable or unacceptable stain (for our Histo QA records). Then, I 
 review the sheet, give any feedback to the performing techs if needed, and 
 document when the final stain was acceptable to the pathologists.
 
 The issues we are having are:
 
 1)  The pathologists may or may not print the sheet to the correct 
 printer, since they use multiple printers throughout the day.
 
 2)  Did the pathologist actually print the order?
 
 3)  Orders may be mixed up with other items printed in Histology.
 
 4)  There is no way to track who ordered what if a sheet is missing. Did 
 the order get put in and printed? Did it go to the wrong printer? Did histo 
 staff misplace it?
 
 5)  There is an abundance of paper printed and shuffled back and forth 
 daily that must be reviewed and followed up on.
 
 How are other labs accomplishing pathologists ordering these tests, tracking 
 pending requests and QA documentation? We currently have Meditech and 
 although we have looked extensively at it in the past, we have not found a 
 Meditech solution for this to date. Are you using other applications such as 
 Excel, Access, etc., or other tracking programs separately?
 
 We currently do not have any barcoding in our laboratory.

Well said Ian and I couldn't agree more. I certainly understand the need for 
consistency and regulations. We are appealing a couple of our deficiencies 
because we have the documentation although it was not documented the way the 
inspectors wanted to see it (as part of a particular form); instead we have a 
couple of forms which captures the information in a number of ways. We'll see 
what CAP says...stay tuned.

 I recently had the great honor to serve as a CAP inspector in Arizona. The 
 lab we inspected will be accredited.   
 
 Lest we forget as inspectors or future inspectors, I hope the below 
 perspective illuminates the purpose of the CAP inspection and helps preserve 
 the integrity of the program if adhered to.
 
 As inspectors, we are trained to be thorough but to  simultaneously use 
 objective judgment i.e.  there may be several ways to meet the intent of 
 the question and that we must review all pertinent documentation and then 
 determine if the process meets the intent of the requirement.
 
 Keeping in mind that the purpose of CAP inspection is not punitive but 
 quality laboratory improvement to fulfill the regulatory purpose of the 
 inspection (CLIA). Our focus should be, in the interest of time and  
 thoroughness major compliance issue rather than nitpick. This helps us 
 maintain professionalism and  preserve the peer-review nature of the 
 program.
 
 So what is it that constitutes compliance? 
 Per CAP:
 - One, that the laboratory has defined a policy, a procedure, or a plan of 
 the three P's for how they are going to do things in the lab. 
   - Secondly, actual practices that matches those three P's. 
   - Finally, documentation to support the fact that practice has indeed 
 matched policies and procedures.
 
 So what is a deficiency? 
 
 Per CAP:
 A deficiency means that the lab did not meet the intent of the checklist 
 item.  It's not the wording; it's not the specifics. It is the intent.  If 
 any of the three above criteria are not yet met, we should cite a deficiency 
 We are admonished to remember that there may be many ways to accomplish an 
 objective. The lab may not do things exactly the way that your lab does, but 
 may still be meeting the intent of the requirement(s). Citing a laboratory 
 for not doing it the way we do it is a common inspector error.
 
 Per CAP, partial compliance is the following:  If there is partial 
 compliance (e.g., some records are inconsistent, one bottle of reagent was 
 not labeled completely, a few temperatures were not recorded, etc.), you must 
 judge whether the degree of non-compliance is likely to have adverse effects 
 on test accuracy, patient care, or worker safety. Also, determine if the lab 
 staff was aware of the inconsistency and if corrective actions were 
 performed. If adverse effects are likely or if there are definite patterns 
 (e.g., missing temperatures only on weekends) without corrective actions 
 only then we must cite a deficiency.
 
 Bottom-line, If you feel you were incorrectly cited since you met the intent 
 of the question, you should  appeal to  CAP. It is an inspected lab's right.  
 If a phase 11  deficiency, submit your evidence of compliance and the Lab  
 Accreditation Committee will either overrule or sustain.  I suspect they will 
 overrule.
 
 Just saying, but based upon just what you described, it sounds like you all 
 met the intent of the question.

What we had here, which did not meet the CAP inspectors requirements 
apparently, were the procedure (of course); a form with each observation of 
each task documented along with any corrective action necessary; the 
correlation of proficiency tests, educational assessments  and performance 
reviews for technical staff; daily evaluations from the pathologists concerning 
staining, microtomy and grossing; and educational training documentation. We 
had no idea with all pieces of this documentation that we were anticipated to 
have more. The form for DO of each task was not detailed enough (despite 
listing each task and proficiency or corrective action of each task) according 
to the inspectors. I was hoping someone could share a form so as to assist us 
in seeing what holes there are in our form. While this DO form is definitely 
not our only form, the inspectors specifically commented on this one.

Is the question that Ronnie posed yesterday requesting justification of the 
need and extent of competency documentation for histology, or is it just a form 
needed? The general checklist pretty much sums up the necessity for doing, and 
required elements to me- GEN. 55500 and CLIA. Maybe I am not understanding?
As for a form, I would expect that the specific items on any forms will vary by 
your personnel and by the testing and processes you perform. 
What I did to document initial training and competency was a make a summary 
checklist for each bench with tasks and direct observations DO for initial 
training   documentation of satisfactory performance before patient testing.
I just put all those checklists together in a summary table for each person. 
High complexity; such as grossing, IHC, FISH scoring get more attention and 
documentation, the waived tests, you have more discretion,- but I thought it 
easier to do everything about the same. I have not been inspected on this 
document yet ( so can't say if CAP will have issues with it- but will know 
soon...) but here is basically what I did to meet GEN.55500 or the main parts; 
 
 
Defined how competency is monitored- method and frequency ( just included as 
part of the competency SOP) 
Orientation and initial training documentation in a checklist for general lab, 
safety 
Training checklist on each technical bench, instrument, major procedure
PT records and performance/results
DO- a practical assessment ( block, slides, stains),  for the assessment of 
previously analyzed specimens, and a PI feedback checklist for the technical 
from this audit of issues- how/what to improve 
Check off in performing QC, calibration, patient ID procedures ( acceptable 
error rates), examples for file
DO of grossing, other performance such as instrument programming/maintenance
Written quiz, policies  procedures, troubleshooting( problem solving 
documentation) 
Continuing education participation records

We looked at several a few months back and liked the Sakura model
best but put it on the backburner. Now we're ready to purchase.

[Histonet] Quality Assurance

Hi All.I have recently been given a new position and am now in charge of 
quality assurance for our entire organization.
While I have been involved in many QA projects and have carried out QA 
throughout histology, I haven't
had much experience is looking at QA in its entirety. If anyone has suggestions

We cut 3 levels on one slide. Each level typically has two sections across the 
slide. If there are multiple pieces of tissue that need to be embedded in a 
little larger mold, we put 3 levels on 1 slide by breaking away the excess 
paraffin around the sections while they're on the waterbath. Then we can then 
fit 3  levels on 1 slide but each level will only have 1 section. Hope this 
makes sense.

I've used Ventana for years and have never had that problem. We use Superfrost 
Plus
slides from Cardinal. I can certainly see that their resolution would be too 
time consuming and I too would be concerned that the staining would be false if 
any reagent in the detection kit did not spread properly over the entire slide. 
I suggest you press it a little further with Ventana. I think there could be 
other instrumentation issues that would cause your problem.

Histonians!
                Has anyone else had incorrect  IHC  staining due to overly 
charged slides that are hydrophobic? My Ventana service department was out 
because I kept complaining about incomplete staining. For instance, the control 
at the top of the slide stained with  both DAB and hematoxylin. But on the very 
same slide the hematoxylin was missing from the patient tissue.
                I told them how this unnerved me. If the same thing happens but 
the hematoxylin stains and the DAB does not...I could be looking at a false 
negative. This may have happened already as far as I know.
                The solution according to Ventana is to take 4 slides from each 
new box of slides (1/2 gross) and do a vortex mix on each of them one at a 
time. Then you know you have valid slides to stain patient tissues! I have no 
time for this. And I'm trying to believe it but it's just not scientific. How 
is it that my results have been free of this phobia for over a year? Why 
didn't I get this (EVER) with Dako instrumentation? The answer, I was told, was 
that the humidity was a problem. If I were to take this to it's ridiculous 
conclusion, I would need to inform oncologists not to operate during these 
incorrectly humidified days. It IS summer so I'm not sure how much humidity is 
required.

1.      What brand of slides do you use?

2.      Have you had the slide phobia so that it compromised the results? 
(Slide-phobia is for retired Histologists, no?)

3.      (I think I know the answer but I just have to ask) Do you validate 
slides?

Histology has a very complex workflow AND requires artisan level workmanship 
to deliver a product. Those two together nearly guarantee mistakes, mostly 
minor, but sometimes literally life-threatening to patients. The goal is to 
instill a sense of Best Quality in the techs. A large part of achieving that 
attitude is to ensure the pathologists and administrators are behind the techs 
100% and ALLOW the techs to do Best Quality - ie, accept that Best Quality will 
sometimes mean slower turnaround time. Does that aspect mean more people are 
needed? That's your call, but can be determined by workload accounting.

The attitude should be that the SYSTEM makes the mistake, not the individual. 
It is not likely a person makes a mistake on purpose, but instead is it some 
aspect of the system that allows them to make a mistake (though shortcuts can 
be thought of as intentionally risking making mistakes on purpose, the 
purpose being to save time or effort).

Workflows can be engineered to ensure some mistakes don't happen. Protocols 
must be followed to the letter by EVERYONE. No workarounds allowed (a 
workaround is an indication that there is something wrong in the system - the 
employee feels the need to take shortcuts. Why?  BTW, Bill Gates said the most 
important word in his vocabulary is why. Why is something done the way it is? 
Why does a mistake happen at a certain point? ). In failure analysis a problem 
is approached by asking 5 levels of WHY? After asking WHY 5 times back down the 
workflow chain you usually find the root cause of a problem. If not, you keep 
asking why until the root cause is found.

For instance, we worked out a slide labeling protocol at the microtome that, if 
followed, will ensure the tech does not make labeling errors. All participated 
in working this out and so have bought into the system. All new employees are 
trained in that system. That will eventually be followed by barcoding, but that 
is a year away at least. But our protocol has nearly eliminated labeling errors 
(we still get a few sneaking in here and there but as we catch them we try to 
figure out how to engineer them away).

We also finally instituted the printing of cassettes directly from our LIS 
rather than using a stand-alone printer or hand-writing. That has almost 
totally eliminated cassette labeling errors - we used to have hundreds per 
month, mainly by residents putting in cassettes that they did not enter in our 
LIS, or making simple typo errors on a stand-alone cassette labeler, or 
hand-written cassettes. 

All these methods need to be investigated. 

Rewards are also very helpful. We give out Bear hugs that are $5 gift 
certificates to the campus store, cafeteria, various food vendors in the 
institution, etc. it's a small reward, but people actually appreciate it. We 
also have Star Awards of $50 gift cards for those times when someone does 
something more beyond the usual. The receiver chooses the card they want from 
about 2 dozen available (coffee shops, VISA, various stores, etc).

First I want you to excuse me, but I do not think that you are really qualified

With 10 years of experience you should know that the first step for quality of

[Histonet] Slide Scanner/Imaging System

I am looking for information on whole slide imaging systems that are relatively 
small. A desktop version that would scan 1-4 slides at a time would be 
sufficient
and the images would need to be viewed via a secure web location. I know there
are several systems out there but am looking for recommendations from those 
already in use.

Try floating your sections on room temperature (in a shallow dish) water then 
carefully, on a slide, transfer to the waterbath. Sections will spread better 
when they hit the warm water.

I'm having a problem with my tissue folding.  It's tiny folds in mouse tissue 
that are too little to see without the use of a microscope.  I was wondering if 
anyone has any tricks that might eliminate this so I can ensure that I'm 
turning out the best quality slides without having to review every slide I turn 
out under the microscope.
 
I'm using distilled water in my water bath and it's set to about 40 degrees 
Celcius.

I have to say I disagree with this interpretation.  The commentary in the 
7/11/2011 checklists indicates that regulations apply to A laboratory must 
evaluate and document the competency of all testing personnel for each test 
system. A TEST SYSTEM is the process that includes pre-analytic, analytic, and 
post-analytic steps used to produce a test result or set of results.  To me 
this includes both histology and pathology office staff.

This is the opinion on myself and our compliance person.  Hazel can you tell us 
where to find the CAP quote.

CAP does not consider us testing personnel. How they come by this is a mystery 
to me.  In a recent memo from CAP it describes this:

 Why does CAP require the completion of the Laboratory Personnel Evaluation 
Roster form and when was this process implemented? 

As part of CAP's deemed status with CMS as an accrediting organization, CMS 
required CAP to implement a more stringent process to document that accredited 
laboratories have appropriately qualified personnel and adequate documentation 
of personnel qualifications. The Laboratory Personnel Evaluation Roster form 
requires laboratories to confirm that personnel files contain the information 
necessary for laboratories to be in compliance with the CLIA personnel 
qualification regulations and CAP Checklist requirements prior to the 
inspection. It is also used by the inspection team to assist in the auditing of 
the records during the inspection to confirm compliance with the Checklist 
requirements. The process of completing the personnel form took effect in 
August 2009.

And goes on to say:
Do I need to list histologists on the Laboratory Personnel Evaluation Roster?  

Typical histologist duties (e.g., fixation, embedding, microtomy, staining and 
cover slipping) are not considered testing. Therefore, it is not necessary to 
list these personnel on the roster. However, if the histologist is performing 
any part of the macroscopic tissue examination which is considered high 
complexity testing, it is necessary to list those personnel on the roster. Such 
personnel must provide documentation at minimum of an associate's 
degree/transcripts or high school diploma or equivalent for individuals 
performing grossing at the same laboratory prior to September 1, 1997.

Not only is it easier, it's more consistent. We use Richard-Allen Hematoxylin 
II and Eosin. They stain beautifully. There are many different vendors, you'll 
have to find one your pathologists like.

We are using the reagents from Leica, formerly Surgipath, Gill's II Hematoxylin 
(01520) and Eosin (01520) on our Leica Autostainer. But there are many other 
vendors that are very good too. Way back when, we used to make our own 
hematoxylin and eosin. But it really is so much easier to buy it ready made.

[Histonet] Slides

Hi all! We are having issues with the tissue falling off of our slides during 
routine
staining. The tissue primarily lifts from the bottom sections on the slides. We 
have
had our stainer checked and have checked ourselves, the oven appears to be
working properly and the water pressure is fine. I tried two different slide 
manufacturers, both silane coated, and are getting the same result. Not sure 
where to go at this point except to try anon-coated slide. Any suggestions on 
a manufacturer or vendor??

[Histonet] Small Slide Scanner

Hi all. Is anyone out there using one of the small, low volume slide scanners 
for use in frozen sections? We have looked at a couple of the larger scanners 
for other purposes but wanted to know how the smaller ones would work for our 
pathologists to review frozen slides from off-site facilities. Of course, a PA 
would gross the specimen at the facility, cut the slides and then load them 
into 
the scanner. Any thoughts?

We have our xylene, alcohol and formalin all hauled away.
Where you  and the quantity are may dictate how to handle the alcohol and 
formalin, every city/county are different. Be sure you get the information in 
writing if you get someone of authority to tell you it's okay to pour down

This is a question specific to Florida.  Can you share with me what your
facilities do with regards to the disposal of formalin and alcohol?  With
regards to formalin, do you recycle, haul it away, neutralize etc?  With
regards to alcohol, do you haul away, pour down the drain, recycle etc?
Does anyone have any specific regs that could help answer these questions?

Thanks to everyone for the feedback on IMEB. It appears there have only been a 
couple of successful recent transactions with them. Most of the feedback I 
received was not favorable so it appears the company is in
a good bit of disarray for reasons unknown. At this point, I've not gotten
a response from the owner regarding the return of our entire deposit but if
he actually comes through, I will let you all know. In the mean time, be leary 
of future transactions with them. Thanks again for the feedback.

I was telling of my situation and asked if others had recent problems. I 
received many responses with two being positive and most of the others negative 
(a couple were offering suggestions on how to handle). I have heard this 
company 
was very reputable which is why I chose to do business with them in the first 
place but have since heard that recently things have been quite rocky. I do 
feel, at this point, that others should be leary before ordering from this 
company. I'd hate for someone else to be in the same situation as we are, 
particularly during these difficult economic times (we are out a large sum of 
money at this point). I also stated that if the issue is resolved, I would let 
everyone know but up until now it has not been. 

You are right, we did not have an account with IMEB but what was promised in 
the 
beginning is not what played out. I have been in this field for a very long 
time 
and realize how the accounting works but I also realize that a full refund is 
not unreasonable given I jumped through every hoop they requested I jump 
through. Keep in mind, restocking fees typically occur when items are already 
shipped or delivered to the customer and they return them. This is not the case 
here, they have both the item and our money. Again, if this issue is resolved 
to 
my satisfaction, I will gladly let everyone know.

I usually do not respond to postings because I have seen people that may not 
agree with a posting get roasted by others but I felt like I had to chance it 
with this posting.  I have had many excellent transactions with IMEB and would 
recommend them to anyone.  Their products are trustworthy and their prices are 
excellent compared to other companies that sell the same products.  I have 
known 
the owner and several of the reps for many years and have never heard of an 
incident like this.  I too, had to put a deposit down when I made my first 
order 
because we did not have an established account with them.  I am assuming that 
you did not have an account already established since they were checking your 
credit status?  It does take time to fill out the paper work and for them to 
check your credit references.  It is that way with almost any new company that 
you place a large order with.  In this day and time trust is a hard thing to 
come by on a new account.  It is terrible that people that may not have used 
them now have a negative thought in their minds about the company. We all have 
at some time something that we can find not quite to our liking about any 
company and I think we need to be careful when we are saying statements like  
It appears there have only been a couple of successful recent transactions with 
them.  This can be interpreted by some to mean that there have been many 
negative interactions so stay clear of them.  Maybe a lot of their customers do 
not reply to posting or do not even subscribe to the histonet.

[Histonet] Problem with IMEB

Hi all.
Has anyone had difficulties in purchasing refurbished equipment from IMEB?
I have been trying to purchase a T2000 ThinPrep processor and sent 50% payment 
with net due in 30 days. This transaction started about a month ago and they've 
had our partial payment for more than 3 weeks but had not shipped the 
instrument. I've repeatedly called to get the status of shipment but have 
gotten 
the run around with checking our credit references. This transaction has left a 
very bad taste in our mouths so I proceeded to request our payment back and 
cancel the order yesterday. Now, IMEB says they'll return our deposit minus a 
20% restocking fee for an item that never even left their facility. 

I was surprised to hear of this from IMEB given I'd heard they were a reputable 
company. My opinion has definetly changed! Just wondered if anyone else has 
had a similar experience with them???

Thanks for the information. I actually asked for the owner today, he'd 
previously been out of the country, but was still unavailable. I've repeatedly
told them we'd done everything they'd requested but they were still sitting on 
our money and the instrument. It wasn't until I threatened to cancel that they 
offered to send the instrument to us immediately. Poor business practice as far 
as I'm concerned and I'm now very leary of the remainder of their business 
practices (service, etc...). 

I hope they come through with a complete refund of our money. I'll be sure to 
let everyone know how it turns out.

[Histonet] Extended processing for breasts

me? Up until now, we've had someone in over the weekend to add breast tissue to

Every tech in my facility uses something different. Some use fingers, some use 
forceps (usually curved), one uses a teasing needle and one uses a paint brush. 
It's up to the individual's technique but we do try to discourage using one's 
fingers too close to the blade.

[Histonet] Hematology Stainer

Hi all.
Does anyone use a small, desktop hematology stainer to perform
Wright-Giemsa stains on bone marrow and blood smears? (We currently stain a 
dozen or fewer slides per day.) If so, any
information you could provide would be helpful. We are currently handstaining 
and have variations in staining. I'm looking to streamlining the staining  and 
stains used.

I have to agree with Jay. A couple of years ago, we purchased new, expensive 
lab 
chairs (from an office supply store that customized) and they look great even 
now. I think it's worth the money from an ergonomic standpoint and also because 
they hold up much better.

    Good luck finding good task chairs that don't cost a fortune.  The good
ones are all very expensive now days.  I think that buying cheap ones is a
false savings, however, as they will fall apart in short order.  I have

[Histonet] VIP 6 Users

Hi all.
For those of you using the VIP 6, how and when do you change or rotate 
reagents? I'm particularly interested in those who count cassettes as a means 
of 
initiating reagent changes. We have mostly small biopsy specimens but do use 
sponges frequently. I'm looking into reducing our reagent costs but, of course, 
will not sacrifice processing.

Has anyone successfully lobbied their institution for an automated coverslipper 
for safety reasons?

Still coverslipping manually-stained IHC, neuro autopsy and special stains, 
sometimes hundreds per day. There has to be a better way.  Under budget 
constraints. That's why I'm wondering if anyone has used concerns about 
histology staff safety, specifically techs under direct exposure to 
toluene/xylene, to enable purchase of an automated/robot coverslipper.

I'd be interested in anyone's experience with this approach, successfully or 
unsuccessfully.

Cardinal. We had sections washing off and uneven staining. We switched to VWR 
VistaVision Histobond Slides and have had no further issues.

Hello, 
We are a lab that has been running Pathvysion Her2 FISH for years
without any major problems.  Recently we have been experiencing more and
more tissue section loss. I have noticed that some of the loss is
starting during pretreatment. My temperatures for all steps are what
they should be.  During each temp. dependent step, we have two
thermometers giving similar readings. Nothing has changes regarding our
slide baking.  I have a call into the vendor of the plus slides to
verify the integrity of the lot.  I have recently noticed the labeling
on the pathvysion bottles is slightly different; I also have a call into
them as well.  

Does anyone have any thoughts?  Does anyone know of processing variables
that may have lead to this

staining. We have two VIP's and a Leica stainer. We use Permount for 
coverslipping (manually with glass). Does anyone use a xylene substitute 
that you would recommend for this combination?

I have a tech who is employed as Histotech eligible, but he was not able to 
pass 
the HT exam and now is no longer eligible to thake the test because he has to 
get his associates degree in science.  The lab management wants to change his 
title from histotech eligible to histotech non-registered without pay 
increase.  
But I feel the pay should be increased. So I would appreciate comments on the 
following:
1.) should title be changed from eligible to non-registered?
2.) After having done this for 8 years should his pay stay the same?
3. Should job responsibilities remain the same

How close on your stain line is your eosin in relation to the oven?
It's possible if it nearby, the oven is evaporating your eosin more
rapidly than expected. Just a thought.

I agree Sally; we cover as soon as the run is over and had to adjust the level
of our H  E since the frosted end of the slides were getting stained as well.
We also try to limit the sections to the bottom half or third of the sldie.

I just ran into an interesting issue that I thought I'd share. I was trying to 
place a credit card order with Beckman Coulter but was denied due to them not 
liking my email address. Mind you, I was willing to pay with a credit card. I 
placed the order on 9/2 and have been fighting with them ever since. 
Ridiculous! Today they were standing by their policy of not accepting my email 
address as a business address so I'm taking my business elsewhere. Just thought 
I'd share this with those of you looking for a company to do business with.

Often on-line vendors do not 'like' and sometimes do not accept 'free-mailer' 
email addresses like yahoo, g-mail, hotmail, etc. because they are frequently 
faked or spoofed.  This is often an issue with the payment processor (the 
company the supplier hires to process credit cards) not the supplier directly.  
These processors frequently get burned by scam artists and it can sometimes 
cost the processor lots of money, hence their reluctance to process free email 
addresses.  They are looking for pay for service addresses from internet access 
providers like Quest, Mediacom, or your company, etc.  For those (and there are 
many of us these days) who save money by using a free-mailer, this issue can be 
a problem.  You can complain to the supplier and they may be able to pressure 
the processor to some degree but it is almost certainly not directly their 
issue.  Look to see whose secure site you are transferred to when you go to pay 
for your purchase
 on-line.  That may tell you the company who is actually setting this rule.

If Beckman is actually processing their payments and making the rule directly, 
then try using a different email address (the best ones that are set up via 
your own company, ie yourn...@yourcompany.com).  In the absence of a 'real' 
email addy, I presume you have tried the telephone ordering approach.  If they 
won't take an order over the phone without a 'real' email addy, then you are in 
a world of hurt.  Be sure that you are using a physical address for your 
location and ship to address that is trusted and can be independently 
confirmed.  That may help them be less concerned.

[Histonet] Negative IHC controls

I have a question concerning ANP.22570 on the CAP checklist. Could you all tell 
me how you are handling
negative controls for IHC staining? The question actually states (and I've 
confirmed with CAP) that we should be running two types of negative controls. 
One for reagents and one for each antibody in a run. I'd like to know
what the practice is. This seems very costly and time consuming. Thanks in

We had the same problem here and after much QC to follow
trends, found that it was the same two clinicians. They
had 2 of 8 exam rooms with the wrong lubricant and also
needed more information on collection. Since then, no

I know this many not be the forum to ask this question, but I know many
of us have cyto depts. within our labs.

We have been seeing an increase in the number of unsatisfactory urine
specimens from our Urology group lately and it is due to lubricant on
the ThinPrep filter.  We have worked with the Urology group and they are
using the recommended lubricant from the manufactory.  We see the
problem even when the urine is diluted in CytoLyt for several hours. Has
anyone else experienced this problem?

[Histonet] Intellipath Instrument

A colleague of mine is looking into purchasing an immuno stainer. Biocare
has offered a good price on the Intellipath instrument, antibodies and 
reagents. While we know the antibodies are reagents are excellent, we 
know nothing about their instrumentation. Does anyone have any first 
hand experience with the Intellipath instrument, service from Biocare, etc...? 
Any information would be helpful.

[Histonet] Bone Marrow Smears
