I know that most VIP's have been available in floor and countertop models.  Has 
anyone ever looked into the possibility of converting from one type to another, 
or are they completely separate models?  It doesn't seem like rerouting the 
lines to support a change like this would be a massive undertaking.  Just

coverslip them themselves and two request assistance with staining and 
coverslipping.  We label and set-up the cassette and slides.

My understanding is CMS (Medicare) has ruled that h.Pylori stains cannot be a 
standing order.  The Pathologist must look at the HE's first and deem the 
stain is needed based on the inflammation.

Back in the 70's (when I was six, but still a histotech!!!) my Pathologist had 
gotten tattoo pigment powder in two colors and that's what we used to mark 
tissue.  Don't know where it came from and don't know where it went!!!

Hi Histo World, as I was cutting to day I was thinking why don't we see if we 
could get specimen marking ink directly from a tattoo vendor? When I first 
started in histo I was told the ink we use was actually tattoo ink. As we know 
as soon as somebody labels something as a medical supply the price is 
increased. Just a cost saving thought, I mentioned it to my immediate 
supervisor but she think it would be a liability issue. I thought we could 
test/validate it on skin tissue left over from a mastectomy or extremity. Any 
thoughts?

I am processing some extremely small specimens - pin tip size.  These are eggs 
with a protein covering.  I have tried using eosin to color the tissues before 
processing but the color came out before paraffin.  The coating on the eggs 
will not absorb the dye.  Does anyone have a suggestion for dyeing or marking

We were recently contacted by our hospital indicating that we are in violation 
of OSHA by using the process we currently are using.
Currently, we provided prefilled 10% neutral buffered formalin
containers to the surgical suites, birthing units, etc.   They fill the
containers with the specimens and return them to the pathology lab.   We
have done this process for many many years and have never been questioned for 
this by either CAP or Joint Commission.  What is your process for specimen 
collection/transport?  Are the specimens put in formalin in the surgery 
suites/birthing unit/etc or in the pathology laboratory?  How about placentas, 
are they sent in formalin from the floor or are they put in formalin in the 
histology lab?

At our hospital, for hospital inpatients and outpatients the Pathologist bill 
for their professional fee and the hospital bills the patients for the 
technical component.  For non-patient specimens (patients seen at other offices 
and facilities), the Pathologist bills the patient globally for both tech and 
pro fee and pays the hospital for the technical services on their industrial 
account.

Good Afternoon,
Our OR furnishes a specimen log that their personnel fills out with date, time, 
patient label, lists of all specimens, dr, their initials and Pathology 
personnel signs off when they get the specimen out of the window.  If there are 
any problems or issues, we call the OR and meet someone from the case in the 
window to resolve the issue.
I think it is the OR's responsibility to get the specimens from the OR to the 
window.

How about the thickness of the tissue?  Once we were cutting skins at 3 microns 
and the pathologist complained the Eosin was too light so we started cutting 
them at 4-5 microns.

I have already done some experimenting with...1) time tissue spends in 
Eosin 2) making sure that the  alcohols after Eosin are the proper 
concentrations3)

 reducing the time that the tissues spends in the alcohols atfer Eosin... I 
have even gone as far as 4) increasing the rinse time in water after the 
decolorizing and bluing

 steps. 5) I have checked the pH of the water as well.

Any help and suggestions would greatly appreciated!!

We divide the work up by trays and mark the number of trays on a dry erase 
board every day.  I have 3 1/2 (1 part time) Pathologist and each day one is 
Primary, one is secondary and then there is the Cytology/Frozen section 
Pathologist.  They rotate this each day.  When the Hemepath guy has a lot of BM 
slides, we give him less surgical trays.  They are great about helping each 
other out if one is super busy!!!

[Histonet] Multiple IHC on Sentinel Nodes

Good Morning to All,
We do cytokeratin IHC on all blocks from the sentinel node specimen.  Sometimes 
we have had up to 12 blocks from the sentinel node specimen.  SO, given the 
previous information on Histonet, can we bill 88342 x 12 on a sentinel node 
when all blocks from that specimen are being stained.  Or is it 88342 x 1???

for facilities that are NOT staffed 24/7, how are fresh specimens handled after 
hours?

Up till now, OR staff have called the pathologist on-call to determine how best 
to handle the specimen, but that process has broken down on the OR side of 
things. The OR Director does not want 2 different ways of dealing with fresh 
specimens and they now want a process that will be the same 24/7.

As we will not be staffing the lab 24/7 on the odd chance a fresh specimen 
might come, I am interested in what other facilities may be doing.

[Histonet] Where Can I Buy Pneumo Control Slides

know they are hard to come by, but my favorite source Newcomer Supply does not 
have any and I ordered some from Sigma-Aldrich and now they are discontinued.

Happy Friday to you!!!
We normally stamp our requisitions with a number and when accessioning we tell 
Meditech what the number is rather than Meditech assigning a number to the 
case.  So, when there is a down time, we continue stamping the requisitions, 
labeling the specimens, making the cassettes and grossing in.  After Meditech 
comes back up, we accession all the specimens at once.

[Histonet] RE: Specimen Collection Instructions

Mike,
Pathology and Lab print a collection manual every few years with the 
requirements for blood test (tubes, amounts, etc) and collection requirements 
for cytology and tissue specimens (fresh, frozen sections, fresh fluids).  We 
also have the manual on-line and we give the physician offices access to this 
on-line manual if they have any questions.  It seems to work well for us.  The 
on-line manual is best because if you make changes, you don't have to reprint 
the whole book.

his/her specimens. To me this is like telling him/her how to practice
medicine.

[Histonet] A PRN Histotech Position in Charleston, SC

Good Afternoon,
We just got approval to replace our PRN HT that moved into a FTE position.  In 
fact, the position # is 8978 on the tridenthealthsystem.com site under careers. 
 It is not guaranteed hours per week, but on an as needed basis.  Lately we've 
needed someone a lot!!!  Preferable hours would be starting between 4:30-5:00 
am and work until the work is caught up.
Keep checking the website and fortunately for me, I am leaving tomorrow morning 
to go to the mountains to do some fly fishin' and relaxing!!!  We'll talk when 
I get back.

[Histonet] A Spreadsheet for ER/PR Benchmark Comparison

I spoke to Joan at CAP just yesterday regarding this question.  I too, am 
getting ready for CAP, so I feel your pain!
She said you can use the benchmarks in the notes under the question in the 
checklist or you can used other published articles that offer benchmark data.  
Just state what your reference is and where it is from.  She also said you can 
use proficiency testing data.

Jennifer,
When we previously used a glass coverslipper, we used ThermoFisher's 
Fisherfinest Superslip coverslips cat #12-545-88.  They have some type of glass 
beads in-between to keep them from sticking together.  They actually look 
dusty, but they are not.  They are kind of expensive, due to the treatment, but 
we very rarely had any stick together.  I think we paid about $150 per case 
which has 10 of the 1 oz boxes in the case.

Good Afternoon,
I am having an issue with my Pathologist who thinks when the patient tissue on 
an IHC stain doesn't look like what he thought it should look like, he then 
wants to stop doing Immunos and send everything out  This may happen about 
twice a month, but the control always stains beautifully.  We recently had the 
stainer serviced and the drop zone was adjusted to alleviate any problems with 
distribution of the solutions.  On a recent test slide of a new control block 
of colon cancer, he also didn't like that CD X2 did not stain some of the 
tissue components, but the rest of the tissue on the test slide stained 
beautifully.  Please advise me on what to say to this Pathologist.

 Most studies will let you submit 10-20 unstained recut slides on Plus slides.  
On legal cases, we always give recuts if there is enough tissue.  When slides 
will not suffice, we will send the block and keep a record of the block being 
sent.

We have an off site hospital and the courier has a large deep plastic storage 
bin, like you get at Walmart, that he brings the amputations in.  It is a blue 
bin (of course, not clear).  It works very well.

We have a new Leica/Surgipath cassette labeler after using the old Surgipath 
cassette labeler for approximately 10 years with no problems.  Our Biomed would 
change the print head ever year or two because the pins would wear our.  The 
new Leica/Surgipath labeler prints accession number and patient's name.  You 
print cassettes one at a time and you can set the labeler to automatically 
advance the case number or block number.  It's fast and easy!

Good Morning,
We process about that many blocks per day also, and I would say YES it is worth 
getting a cassette labeler.  Leica/Surgipath has a small semi-automatic 
cassette labeler that we are in the process of buying and we LOVE it.  It will 
print the accession# and the patient's name on the front of the cassette which 
complies with CAP's 2 pt identifiers.  The automated part is you can set it to 
advance to the next acc # or advance the block or specimen numbers.  Tonia

area.

a chemical resistant pen that works great.  We write the number on top and the 
patient initials  on the side.  We are considering getting a cassette labeler.

[Histonet] Any Other Manual Cassette Printers

We have a paraffin scraper that our Housekeeping staff uses. It was purchased 
from   American MasterTech Scientific (item CPW04200E, and replacement blades 
item CPW04201P). We also have perforated anti-fatigue mats in aisles around the 
cutting stations which trap the wax and can be vacuumed/swept away when the mat 
is flipped.

This may seem like a insignificant question, but we consistently have issues 
with injuries to our maintenance staff from cleaning the wax off of our floors. 
 Housekeeping has laid sticky layered mats all over our floors (like large 
mouse traps), which they peel off layer by layer as they become wax covered.  
Apparently these are very expensive.  Does anyone have any suggestions?

At this point, our Hematology Dept assists on BMs on the floors.  If they 
could, they would pass it on to us, but we do not have the staff (or desire) to 
do it.  My past 2 HT hires came from places where the histotechs assisted on 
the BM collection.

[Histonet] Yellow Walls in Pathology-Deja Vu

Good Morning to All,
A few years ago I sent a inquiry to Histonet asking if anyone has ever had this 
issue in their tissue processing area?  We are having this issue again and I 
cannot pinpoint any changes or issues that have arisen.
We have our VIP tissue processor, paraffin pot and embedding center next to 
each other.  Our Immuno stainer is across the room and I have 4 microtomy 
cutting stations on the other side of the room.  Good ventilation all around.  
The ceiling grids are yellow, the walls and on top of the IHC stainer lid.
Anybody else ever had this problem?

[Histonet] Does a Cytotechnologist Meet CAP Grossing Requirements?

In your opinion, does a graduate from a Cytotechnologist Program (ASCP) with 
additional documented training meet the new requirement for grossing as high 
complexity testing personnel under CLIA???  Our PA that works for our

We have run into a question about CPT charges and I was hoping to get a more 
definitive answer than what we currently have.  I know if you have two tonsils 
in one container, one with a stitch and one without, you can charge for both 
tonsils.  How about if you have one container with a placenta and a stillborn 
fetus?  One suggestion is to charge an 88307 and the other is to charge 88300 
AND an 88307 (to cover the gross only on the fetus and the tissue that was 
submitted for the placenta).  What is the standard, shall I say legal?, way of 
charging for these specimens?  Must you choose one of the charges or are we 
allowed to use both charges?
