Hi Brandy, Our QC program indicates that all slides are QC'd by a tech prior to leaving the lab. Once, QC is done, the tech delineates between the patient tissue and control. This indicates to the lab assistant that they are ready to be distributed. Stain quality is also 'assumed' through the report disclaimer after the case is read out by the pathologist which includes the statement: Ancillary studies were performed on this case with appropriate controls showing appropriate reactivity. Where we do document is when a stain does not work as expected. At the time of tech QC, if the stain did not work, we print the run log, attach the bad slide with comments and perform a repeat following investigation into why it did not work. The repeat results and initials are noted on the log and this is filed in a binder. This saves us a lot of time. If the pathologist has an issue with a stain, they submit a QI issue log to me and I research the issue. Hope this helps.
