Kyle,

Did you try formatting the disks with dasdfmt **before** bringing up the
installer? So the overall steps would be:

-) Punch files to reader and IPL reader
-) *** Start an SSH session as "root" and dasdfmt disks ***
-) Start an ssh session as "install" and start install
-) Continue 

See the bottom of page 155 in  "The Virtualization Cookbook for IBM z/VM
6.3, RHEL 6.4, and SLES 11 SP3" on the Web at
http://www.redbooks.ibm.com/abstracts/sg248147.html

In older versions of the book we used to write "Hey maybe you should try
this ...". In the last one we wrote "Just do this "

BTW, the RHEL supplied PARM file looked like (except for the CMSCONFFILE value):

root=/dev/ram0 ro ip=off ramdisk_size=4 cio_ignore=all,!0.0.0009
CMSDASD=191 CMSCONFFILE=utlz0024.CONF-RH6   

What does the !0.0.009 do?  It looks like a z/VM VM console address in some 
sort of Linux format.  Is that something that needs to be different?  If so, do 
you have any suggestions?

It's saying "Don't ignore whatever device is at address 0.0.0009."  On many 
z/VM guests that is indeed the console.  If it didn't match your virtual 
console, you'd be worrying about a whole different problem: why doesn't 
anything seem to happen when I try to IPL Linux?

cio_ignore=all,!0.0.0009 means the Linux kernel will block all devices
except the console from being visible until they will be unblocked by
other mechanisms like processing various config files. There is
usually no need to change the value and in later versions the 0.0.0009
device id can be (and is) replaced by symbolic name "condev".

Thanks Dan.

BTW, I found the problem.  I received a new (Win 7 PC) recently to replace   
my old (Win XP) PC.  Even though I got an x-server (Xming) installed,   
Windows was not set to automatically start it when booting.   I have rectified 
that oversight and now I am getting the GUI installer.
