What I face exactly is that it is toggled off when you shutdown your Mac.

Note the following sequence:
1. Turn on VoiceOver in Accessibility options in login menu
2. Restart your Mac
3. You will notice that Voiceover is turned on
4. Reopen Accessibility options in login settings and you may notice that 
Voiceover is not checked.
5. Restart your Mac again, and you won’t hear Voiceover since it is turned off.

That occurs with me since Mac OS 12.1 (I didn’t install Mac OS 12.0)

Hi Jason and Jonathan,
Okay, I did not interact with the Users Table. It’s working fine now. I am on 
the phone with Apple Accessibility Support as I write this. He is also 
concerned that the setting is not sticking, so he’s discussing with his 
engineering colleagues as I wait. Thanks for the assist, I will definitely save 
these e-mails in case I need to do this again.

Hi Jonathan,

Well, I will check again tomorrow when I am able to use the Mac. In the 
meantime, I upgraded to 12.1 a long time ago I didn’t encounter this problem 
until this week. I then upgraded to 12.2 of Monterey and everything was working 
for a couple of days, and then again it stopped working. There must be 
something related to my system that is causing this. Thanks.

It seems that the 12.X updates turn off this option.
To turn it back on.
1. Open System Preferences.
2. Click on "Users & Groups"
3. Click on "Login options"
4. Unlock System Preferences by clicking on "Click the lock to make changes" 
button if necessary." button.
5. Click on "Accessibility Options"

6. The first checkbox in the table will be VoiceOver, click this if it is 
unchecked.

Hi Jason,

Well, I looked in the login options area and I cannot locate anything to talk 
about enabling voiceover there. I saw something in VO utilities where voiceover 
was unable to that’s checked, but I don’t see anything in the login and option 
screen.

I have run into a problem for the second time this week. What is happening is
