Have you tried having someone read to them before or after they read and
them have them draw a picture about what they read.

I used to teach first grade.  Have them draw their mental image after
reading the sentences.  Then answer the questions.  If they can't draw it
they have to back up and re-read until they can.

Sounds like u ought to get your speech pathologist involved ... Could be a 
language disability... Either receptive or expressive
