xkill (the kill button) is located at:

/usr/X11R6/bin/xkill

you can make a link to that

(at least thats where it usually is...might want to try locate xkill to verify)

one thing you may want to note:  xkill seems to have a habit of only killing the 
window itself sometimes I've had instances where I've used xkill and the process 
behind it was still showing up (using ps ax | grep ) and I still had to use 
kill -s 9  to kill it fully.

Just open a console and type "xkill" (without the quotes) on the CML.

Then you can left click on the runaway app same as with the xkill icon.

If you insist on an icon you could always roll your own using the same xkill 
command and an icon of choice.

Rob, not sure about the 9.1 stuff, but normally you can:


  


and bring up the skull/deaths' head and zap whatever you want by clicking on 
it. I alway remove the kill icon from my 9 and 12 yr olds comps (do you need 
to ask why? ) but still needed an easy way for me to stop things in 
case they had problems. Works fine here... :-)
