Duplicate sids would give you machine errors of the type...
machine name not found
duplicate name
unable to join the domain
unable to contact the domain
or other type of errors.  Since I have been syspreping or sidwalking, 
I have not seen these errors in a while.  So if the errors are not 
exactly worded  I have not seen a duplicate SID cause you not to 
get a desktop.

I would go with the virus or something else corrupting the logon process.
It is possible that your machine was compromised and your virus 
scanner did not restore the logon executable properly.  I had this 
happen a couple of times.

You will also get domain groups (i.e. Domain Admins), not correctly
nesting in local groups.

 

Also remember: you have to check the generalize box in the 2K8 flavor
of sysprep ... otherwise you have to run something like newsid...

Don't know about a trace, but even if it doesn't fix this particular issue, I 
would roll the SIDs of any machines that were imaged as this will eventually 
rear its ugly head in some fashion. Usually on a Friday right before a long 
weekend.

Random explorer.exe issue

I have an issue with explorer.exe not starting on boot.  It has been happening 
throughout my organization, randomly.  Rarely hits the same person twice in a 
row.  They end up coming to me after trying to reboot 2-3 times, with no luck.  
I tell them how to go into task manager and start a new explorer.exe task, and 
all their icons pop up, and they are happy.  However, I'm at the point of 
realizing there must be some underlying issue, as it is extremely widespread.  
The randomness is what really bothers me though, as there are rarely solutions 
for random issues.  But, I'm hoping that you guys can come up with something.  
I have googled it, and there is plenty of hard disk space, and the registry 
keys are correct. (userinit, and shell)

Any help/advice/tips are greatly appreciated.

My first guess would be that a virus or some sort of malware got into your 
network and is spreading causing the problem. You might try to download a copy 
of malwarebytes from malwarebytes.com and run it on any machine which has been 
affected. It's a free app (although you can buy an enhanced version, the full 
version works well for scanning and cleaning.) I suspect you'll probably find 
you have some sort of infection.

There are no errors in Event Viewer, neither System nor Application.  We are 
running XP SP3.  I don't think it's a virus, since it is really random, and 
rarely occurs twice in a row on the same user.  It's been happening since we 
rolled these new desktops out a few months ago, and they immediately got 
Symantec installed during that process.  As I mentioned in the initial post, I 
checked the registry keys that my Google-fu came up with, and they are normal.

Last time I saw this (4 or 5 years back) it was an issue with an NT-style logon 
script that had been modified.  IIRC, something in the script couldn't complete 
and was waiting indefinitely for input.

can't recall if it was directly modified or if someone changed a gpo that 
disallowed it to run properly.  Might be worth checking if those kinds of 
changes were recently made.

Nope, I'm the only one that would make changes to gpo's.  The only new
thing with the new computers was setting up a new password retention
policy, which I made a new, separate GPO for.

I've seen images go bezerks if they are re-syspreped too many times, I
keep my limit to 2.  I think this is even documented somewhere on MS...

That's all I got...  I'm sure you checked, but anything in the event
log?  

Perhaps run it through bootvis once to see if it clears up the log jam?

No, I haven't.  Some of the people that it happens to only get it once,
and not since, others once in a while, and so far, one user who is
telling me she's gotten it every day for about a week.  I'll try the
restore on her computer, if I can get on it.

Not using roaming profiles, and haven't had a chance to really test
other profiles, as it is really random.  I have even gotten it myself
once or twice since we rolled the new PCs out, which was about a month
ago, or so.

Are the computer SIDs being rolled? Duplicate SIDs will create quirky
issues.

Windows XP Pro, recently deployed from a Ghost image.

 

Really random issue, although for some of my users, it's pretty much
every time they log in.  They turn on their computer in the morning, log
in, and get nothing except their wallpaper.  No icons, no access to
Start, etc.  When we go into Task Manager, explorer.exe is not running,
so we start it manually.  Everything is fine from then on, at least for
that day.  Some users don't get this issue again.  I have one user,
however, this happens to her every day.  What makes it worse, is that
she is a Director, so it becomes a hot issue.  

 

I've done google searches, finding this issue in various places.
However, none of them seem exactly like what I'm seeing.  There are no
errors/warnings in Event Viewer, so I'm just not sure what's going on.

 

Any ideas?

Hmm - instead of trying random things in the hope that they fix the problem, I 
think you should follow the advice of others here in terms of trying to 
isolate/understand the problem first.

- Perhaps check that explorer.exe isn't crashing when it starts
- Perhaps try to see whether the problem occurs on a per-machine or per-user 
basis (e.g. does the problem follow a user around, does the problem occur for 
all users on a system etc)

 No, I haven't.  Some of the people that it happens to only get it once,
 and not since, others once in a while, and so far, one user who is
 telling me she's gotten it every day for about a week.  I'll try the
 restore on her computer, if I can get on it.
