[Origami] Plane face 6 side antiprism

I was organizing my old models and found one I don't remember having
folded, it's a plain side hexagonal antiprism, folded from a square and
based on a 60 ° triangle grid slanted 15 ° from the vertical.

The one I found was not very well folded and now I can't find the
references or an elegant way to close it. Gerardo told me he thought it had
been taught by Chris Palmer when he was here some years ago, but nobody
that I know of who attended his classes remembers it.

So does anyone know the author of this model? You can see a picture of this

I'm not sure how many of you have heard, so I thought I'd cross-post this:

I've just learned from Facebook that our dear friend Cy Levine
 has passed away. Many of you will
know him from the conventions he attended, where he lit up every room he
was in. He was a close friend of Lillian Oppenheimer, and shared some
wonderful stories of the early days of OrigamiUSA
. I'm pleased and proud to have known
him, and I'll miss him.

I'm very pleased to announce the publication today (24th August) in the UK
of my latest book, 'Complete Pleats: Pleating Techniques for Fashion,
Architecture and Design', published by Laurence King Publishing (LKP) in
London.

The book will be published on 6th October in the USA.  German and Spanish
editions are also available and editions in several other languages will
follow.

It's my 5th title for LKP, continuing the series begun in 2011 with the
publication of 'Folding Techniques for Designers' and subsequently, 3
smaller titles.  The 4 books are here::

If I may say, after 35 previous attempts beginning in 1984, I think 'Pleats'
is my best book!  It differs from the other LKP books because it's larger
(304 pages), hardback (and featuring a large debossed pleated panel on the
front cover), full colour and because it features the pleated work of more
than 40 designers, in all-manner of materials from fabric to wood to
architectural facades.  Origami artists are well represented.  The book also
contains a DVD of 23 instructional videos.

Although more a book of generic folding concepts for designers than a book
of origami models, there's still a lot to fold (more than 200 CPs with
accompanying photos) and much to explore, so I think it should have some
appeal to an origami audience.. 

'Folding Techniques for Designers' contributed to the rise of origami as a
language of contemporary design.  I sincerely hope that 'Complete Pleats'
will follow, introducing designers to the wide and creative world of
pleating.
