As our base numbers of caesareans are getting larger, there
will be more in the future.

If only we could reduce significantly the number of
inductions, then maybe we could reduce

the number of caesareans??

Where is the evidence for inductions at 40 weeks please?

Our local council donates 2 Australian native trees that we hand out to
each new mum and our "Birth to Kindergarten" literacy programme donates
a book with advice to mums/dads/family to read to the new baby every day
of his/her life.  Perhaps you could plant a tree and give your Will a
book, and get him a new one each birthday!

It must have been thrown out, but I don't remember doing it, and my  
husband swears he didn't - so it's a mystery!

I was going to bury it on Will's first birthday next month, and then  
get a 'Mother's Love" rose from the ABA website to plant in late  
July, but I can't now.  I wanted to do something special on his  
birthday but I'm stumped for ideas!  Perhaps I will just go out and  
buy the nice pot on the day, and wait for the rose to be delivered in  
late July.

Any ideas for something to remember his birth by - apart from cake!?

Eugowra is a small village and the former
golf club has been transformed into a restaurant/wedding venue etc so the name
came from the golfing greens.

It is a lovely place, lots of old gum
trees and green grass/lawn which impresses those of us on water restrictions!

Eugowra is certainly worth a visit.

Dear All,

Thanks for the continuing information re diabetes insipidus
and breastfeeding.  I returned to
work to note this mother was bottle feeding, no reporting by midwives postnatally about breastfeeding.  I did note in her records LMO wrote “ to trial breastfeeding” and on rounds this am
asked how the feeding was going. 

Unfortunately, with no encouragement in the first 24 hours,
mother is well settled into bottle feeding.  I have suggested never too late to start
but got no where.

Actually a disappointing breastfeeding shift today as a day
11, post LSCS mum of 34/35 gestation prem in our unit to learn breastfeeding, on
3/24 tube feeds with a couple of breast feeding attempts.  Tired baby, has not got the sucking
organised, is gaining weight.  Has
had its more wakeful times at night but given bottle feeds.

Mother very fragile today as now a
wound infection, tired, nipples sore adamant about baby having bottle.  Tried a few seconds of supply line, but
mother too negative 
and of course baby “woofed “ bottle down!

Got that all off my chest!

In the middle of all that, had a
great but vocal birth, baby breast feeds great!

Many thanks to all for information
and links for diabetes insipidus.  Yesterday’s prostin
had not worked!!  Dates
?? Previous baby 10lb, so guess why they are inducing
this mother.  Our unit has a
midwives/doctors breakfast meeting 3 or 4 times a year, at yesterday’s
meeting information given that April is a well booked month, my suggestion that
we have an induction free month was met with silence and no comment!

This is a very interesting question. I have not come across diabetes
insipidus in a breast feeding woman before. I have looked up my books
and
surfed the internet.  It will be a very delicate balancing act, as
diabetes
insipidus is a lack of anti-diuretic hormone (nothing to do with
diabetes
mellitus, except you pee a lot), resulting in problems with fluid
balance.
Fluid balance obviously is important for milk production (and many other
things).It appears safe to take vasopressin and breast feed. Vasopressin
(antidiurectic hormone) is apparently similar in some ways to oxytocin,
with
each sometimes producing effects on the other. I think it may be helpful
to
contact the Lactation Resource Centre to get some quality information to
guide the mother and her care givers. There may be a small fee. I will
paste

Excitement in Parkes, NSW where midwife
Lee Ramsay was awarded OAM for midwifery and ante-natal services to community.  Lee, now 70 has been continuously employed by the
local hospital, many years as NUM and still .8 shifts per fortnight.

About time some of our midwives are recognised!

We are being made aware of safe sleeping / co-bedding with
babies.  Is there any data or information
about the safety of baby in another room with baby monitor?

Am I just getting past it? 
I feel very sad that babies sleep down the hall and parents think all is
wonderful because the monitor is on, especially as the monitors are only one
way!

have been told by a pregnant woman that she was reluctant to have a strep B
test at 35-37 weeks. She was informed by a doctor in the A/N clinic of our
public tertiary hospital, that if she went into labour with an “unknown
status” and attended the delivery suite , her baby would have to be given
IMI antibiotics until the baby’s screening swabs came back 48hrs
later.  She felt that to protect the baby, she had to consent to A/N
screening.  Those who recognize the description of this hospital, is that
true?  Or has this lady been unintentionally misled?  Does this
happen in any other hospitals?  Feel free to email me off line if you

Dear Midwives,

I have just returned from our small hospital “midwives
and doctors breakfast meeting”.  This is to encourage communication.  We have 4 GP/Obs
and 9 midwivies.

On discussion was a new policy for epidural-top ups: both
pethidine only and marcain/fenytal .

Policy is now insistent on bp obs 5minutely for 30 minutes for both top-ups.

Other hospitals have had the pethidine only top-ups:  prior giving top-up bp,
in 5 minutes and then in 15 minutes.

 

Does anyone have an email address for me to contact?

 

Also does anyone have policy or guidelines re allowing dads
to cut cord?  This meeting has
decided that no cord clamps (plastic) will be put on set up so the forceps are
used, Dad can do a token cutting later (?how later) when cord clamp (plastic)
is to be put on.

I was hailed down when I suggested that a well baby could be
put onto mum and continue with the cord clamp/ dad cutting cord when ready.  If the baby needed
active resuscitation then quick transfer to resus.
trolley would be normal procedure.

 

As you will have noticed our GP’s only do active 3rd
stage, mothers have never heard of physiological 3rd stage even
though same discussed at ante-natal classes.

Dear Andrea,

Over my 30+ years of a midwife, I remember
2 births that were influenced by the “ultrasound” reports.

The first was mother to be of her 5th
boy, turned out to be a girl so result was happiness all round!

The second event was a mother having 2nd
baby, first was a boy and ultrasound said this one would be a girl.  Over the labour, mother and support
people talked about the “nursery being newly painted pink, girls names chosen, grandma had jewellery engraved with this
name etc.  Baby was a boy!

The atmosphere in the birth room was one
of shock, mum took a couple of hours to just hold her
new baby!  I felt incredibly sad for
this baby and family but the next day all (at least on the surface!) seemed ok.  I think the baby was being bottle fed, though
this was decided before the birth.

I have a client who is due to
give birth soon and she really wants a baby of a particular
gender but the ultrasound has shown the baby to be the opposite. This
is much more than being a little disappointed and it does not
appear to have eased over the course of the pregnancy. 





 





She has expressed concerns about how
this may potentially affect her labour. She also expresses enormous guilt
over having these feelings when she already has healthy children, including one
of this gender. She is also concerned about the effects her feelings/thoughts
are having and will have on her baby's development.





 





Assuming the ultrasound is correct,
has anyone had experience of this either as a mother or care provider? Any
words of wisdom?

triplets happened at Parkes (home of “The Dish”) 23 years ago,
before ultrasound was available locally.

We were confident of twins, all babies did
well.  These days we are not allowed to

Hi all, have been lurking for a
while, love the interesting discussions, thankyou. I'm a  married mum
of two and hospital midwife by convenience. Anyway, at work last week a woman
came in at 32 weeks , triplets, in good labour at 7 cm, all head down,
 off to her c/s as planned with a huge amount of fuss, mum stressed
out to the max. I understand all the risk factors and the reasons for a
c/s ( prem as well) just wondered if anyone out there has been at a
triplet vaginal birth in Australia? Despite the risks I just had
this big gut feeling  that everything would have been all right. The
babies were all fine except the third who needed a bit of CPAP once they got
into the nursery. My Auntie had a vaginal birth of triplets in a community
hospital in Perth in 1979, she's vague on the details but all was OK with the
boys.

Seems to me since our GP’s have attended ALSO in the
last couple of years, they jump in, take over birthing and treat all births as
shoulder dystocia!  Ending up with horrible dragging out of babies.  I probably have over reacted, but 2
recent births I attended ended up this way.  Small rural hospital,
and I have been a midwife for 33 years.

Dear Alison,

I am a midwife in small public hospital.  It is important to maintain privacy to
mothers in labour, quietness and I like to have soft lights.  Reduce the support teams
chatter to minimal.  This is sometimes
difficult when you get both mother/mother-in – law present, plus sisters and
partner:  they all want to chat about
everything else in their lives.  The
use of a sheet as a birthing cape can help mother form her own world.

With the birth of my own babies, I felt the
“all fours” position disempowering. However I let mothers chose their
own positioning.

It would be great not to need cannulation skills, but in a
small rural hospital we deal with general patients, doctor not on site.  Trauma, acute chest pains etc all need
cannulas to commence life saving treatment.

Is no-one doing IV antibiotics for group B strep positive
mothers in labour?  By doing these
ourselves, we keep the doctors away.

It is rare that at Parkes we use epidurals during labour,
but I am supportive of mothers having epidural for elective caesareans.  This is where we use epidural top-ups,
in the first 24 hours or less.  Therefore
we need policies to cover as our midwifery care includes adequate pain relief
post Caesar.  We do not have the
doctor at the hospital, or if he is he may be in emergency, I don’t need
our mother in pain to have to wait til he comes to the unit.

Justine, just a note to say I was looking
for policies about epidural top-ups.  This does not mean I as a midwife use
them unless of extreme need.  We do
use deep water though mothers at home don’t have access due to water
shortage.

My last 2 births have been fantastic
outcomes for mums due to my support and belief in creative movement to achieve
normal childbirth.

We certainly don’t leave policies to
the doctors, they need to be evidence based best
practice!  This helps us as well!

I thought midwifery was about well women and normal childbirth.

Shouldn’t this be left to the Drs! Perhaps if midwives said this we would

I am astounded that the majority of women can access Pethidine and an epidural
and yet a tiny majority have access to deep water
(sorry showers don’t count!)

Natural pain relief, that’s the domian of midwifery

Our hospital policy requires yearly accreditation based on
top-ups using marcain/fentanyl ie
BP readings every 5 minutes for 30mins.

I have had copy of ?where that pethidine/saline top-ups have
pre top-up bp/pulse, check site etc, then in 5
minutes and infurther 15minutes.

This copy has been taken from our unit.
