The only time that I had any of my 5 babies weighed on day 3 was when the midwife..a wonderful woman...failed to get around to it until then ! She didn't bother to weigh her immediately after the birth as 'she looked to be a good weight' and then only bothered on day 3 as I insisted that I needed to give all interested parties a better answer than 'she's a good weight' My midwife stated that whatever weight she was at day 3 would almost be the lowest that she would ever be as it was common knowledge to her that nearly all babies lost weight during the first couple of days. When I quizzed her about this she replied 'sure Melanie , use your common sense , the wee things are all knackered and sleepy after their journeys and are having a bit of rest and giving you time to recover before getting down to the serious business of feeding and growing...it seemed like a good answer at the time! Not something I 've recorded for a Neonatal screen for 15 years. When they ask for weight I always use the birth weight, and that it what all the Midwives do where I work. The hosp Day 4 weighs, but working in the CMP (ACT) I don't weigh unless I'm concerned about output and then not until day 4. I think I love this midwife! we could be soulmates? My Mum (a Maternity Nurse in the late 1930's) who caught lots and lots of babies in the S.w of W.a. told me that the real weight was after they had their first poo. I just think that this obsession with weight starts when babies are born. what does it weigh? comes after What is it? This obsession carries on throughout life and one is never the right weight . I definitely am not!. and I love myself anyway. Baby toddled off with them...in the toilet!"), then after a few years all of a sudden it's not ok to gain, and we are supposed to "lose" part of the body we've worked very hard to gain. The very term "lose weight" sets one up to gainmore of it. Most of us are very successful! and so on. My routine practice is not to weigh babies at birth. I do this for many reasons but primarily to slow things down for the newly born soul. I weigh the next day or so. I also advise the families not to dress the baby but just a nappy and baby wraps for the same reason. These were homebirth babies, weighed on day7. This is so simple that I don't mean to insult any one babies don't need to be weighed at birth the next day if the parents want to know is better also believe it or not I now suggest all mothers in the private system have their babe weighed on the SAME scales ie wait till they go to PN ward .I have proved that at one part priv hosp in melb there is a 300grm diff in LW and PN scales . Look at the baby is it alert and feeding whats happening in the nappy dept and forget the scales . Irene You can probably guess my advice about weighing on day 3 - STOP it! It only causes stress to mothers and many midwives and doctors. Observation of feeds, output (passage of mec and transitional stools) and baby condition and behaviour is much more useful than day 3 weight to assess how well the babe is making the transition to extrauterine life. This way you identify if and what the particular problem is e.g. poor attachment and deal with that rather than jumping in with artificial comps which will create more problems. If there is in fact a general problem with babies not adapting and feeding well you would need to look at what is happening to them in your system. I would look at the basics - narcotic use in labour, do they go to the breast within the first 1 hour of birth, are they demand feeding, rooming in with mum or being babysat at night to let the mum get some rest...mothers and babies do and will make this transition well and healthily as long as they are not sabotaged by our poor professional assistance. I guess my question would be why are you weighing babies on day 3? Unless your babies are not peeing, pooping, are lethargic, irritable, and not feeding, WHY? Our routine to weigh babies in Seattle was at birth and then on day 7. Now of course a baby with the symptoms mentioned above would be weighed, feeding would be assessed and if necessary baby would see a paed and/or lactation consultant. Theoretically, babies born to diabetic mums (both pre-gestational and gestational) can be hypervolemic at birth, appear puffy and bloated and then pee out the excess fluid resulting in an exagerrated weight loss. This can be exaccerbated by hypoglycemia following birth (after being cut off from mum's hyperglycemia and the resulting over production of insulin) resulting in a lethargic, irritable babie who wont feed. Irene, I realise this is a totally isolated incident, but just for comparison... Baby #1 - born May 99 at St. Vincents Private in Melb.was born at 3830g, got down to 3350. His delivery was instrumental (due to PE diagnosed in labour) and I lost a litre of blood from episiotomy which tore - according to OB (there was no PPH)- and was not transfused due to good pathology results. Midwives wanted to supplement with formula - I wouldn't allow it and he after day 3 he began gaining at 400g+ per week. Baby #2 - born March 02 at Mercy Hospital Melb. was born at 3950g and weighed on day 2 (at home) at 4000g. Birth was induced by ARM due to fear of onset of PE and labour stimulated with synto because they were busy! But delivery was normal - no analgesics, no instruments. I put the loss of 10% of #1 baby down to blood loss and traumatic delivery. Maybe I'm kidding myself?? I guess also with #2 breasts were primed and ready to go - "knew" what to do, so to speak? Thanks for your reply Marilyn. It is 'routine' here to weigh on day 3 whether mum is diabetic or not. Personally, I think it just stresses the mums out and they begin to get anxious and then the 'rot' sets in. I was interested to hear that you only weigh on day 7, depending on the progress as you stated. I reckon that is a much more sensible regime, as breastfeeding does not always go as the textbook says.
