In my business I see quite a few broken modules each year. As I recall, in every instance there is a visible point where the impact occurred. Typically a small radial pattern is around the impact point but then random patterns over the rest on the module. Look close. Hope this helps. If you don't see any sign of impact, look closely at the area in front of the junction box. Any discoloration is an indication that it has been hot in that area. This often is the case with a shorted diode. The heat will stress the glass and cause it to fracture or make it susceptible to fracture from any minor stress. The glass will usually have larger fragments in that area and finer fragments farther away. A shorted diode should be warranty issue. A few years ago there was a rash of this type module glass breakage caused by flaws in the glass, that would not show up until a year's worth of thermal cycling. I saw several of the non-frame glass only modules fail in California sun. Without a frame they would curl-up into a U shape. Usually the glass needs to cycle from very cold to very hot several times, or be very hot in sun then a sudden rain to cause this glass flaw to fail. Like electronics, if this did not happen in the first year after installation it most likely was something other than flaw in glass. Anyway, I thought this was going away with better quality controls on the glass used in module manufacturer. I would guess that it was the glass and tempering not complete, or done correctly. I worked at a glass temperaturing facility most of the sheets can out fine, but every now and then a few would shatter while on the belt. The temperaturing put to much stress in teh glass.
