Doesn't always work.  A highway could be realigned without changing mile
posts and exit numbers on unchanged portions.

If the road is mapped as a single carriageway and the distance marker is clearly
visible from both sides of the road, then it would be simplest to make it a node
on the way itself.  Otherwise, it should probably be a separate node on one side
or the other.

Of course the length of a road and the distance along it can be calculated
from the OSM data and do not need special nodes every 1km or so.  But if the
object exists in the real world, it can be mapped.

Calculating distance should be done by software by calculating and
adding length of segments (ways). No relation needed.
The signs however could be of navigational aid. E.g. "turn left into
the minor road 500 m after the sign saying 'Skara 14 km'". It could be
made to fit as an extension to the destination_sign relation [1],
although the name of the tags would be somewhat non-intuitive for this
purpose, and routing software would be saying "Continue on the road
towards XX" for each sign even though there is no intersection or
exit.

In the uk km 0 is often on a road that was never built!
The km markers on the m6 start counting at the london end of the m1 so
it isn't always the same road that the end is on

These markers usually have a reference number so that the services can find
you, and an arrow pointing in the direction of the nearest emergency phone.

You may even find them more often then 1km more like 100meters

If your talking about what I think your talking about, they exists down
every motorway in the UK.
