I suppose, what it really takes is for you to get started inputting
content and tag-relations, whereas subitems should always tag to
parent items in the respective tag-hierarchies.

For adding content, the NewHere button from MPTW might come in handy
for you:

The power of tagging is that it goes well beyond hierarchies.
Actually, hierarchical structures may really just become apparent
after a while, once you've added lots of content. I agree that a mere
taglist won't do for you, so that the tags-tab in the sidebar won't be
your preferred spot to browse information.

Try identifying your top-level-tags and make them accessible in either
of the sidebars or in a header-menu or via special summary tiddlers,
giving you an overview over subcategories... and then let
TagglyTagging have its way to allow you to drill down.

TagglyTagging has extensive customization abilities, even allowing you
to see a sitemap of subitems, which I can well imagine will make your
day.

Also, to me, Eric's GotoPlugin is simply essential when it comes to
finding content quickly:

At last, maybe something equivalent to the top section of my MainMenu
in tbGTD...

...will improve your navigation of categories. It requires:

[tw] list of tags with related tiddlers

I would like to have a table of all the tags in the wiki (similiar to
allTags ). and for every tag a list of all the tiddlers that use
it.

I used the foreach plugin and based myself on the example at:
http://tiddlywiki.abego-software.de/#[[List%20all%20Tiddlers%2C%20grouped%20by%20Tags%20%28including%20tiddlers%20with%20no%20tags%29]]

Problem is, this produces a list of all the tag-combinations that
exist in the wiki (and lists the tiddlers for that combination).

If I have only 10 tags in my wiki, it should produce a table with only
10 rows; not with all the possible combinations...

suggestions?

TagglyTagging will do what you want. Check out 'Using TagglyTagging' to 
organise your data at the link below. It didn't exactly make it transparent to 
me, but it got me started. You have to work with it for a while to get the hang 
of it. The cool thing is that you don't have to have your tagging categories 
and relationships defined ahead of time. The can be revised and restructured on 
the fly, so you can start taking notes, apply initially appropriate tags, then 
revise tags and tagging structure as your conceptualization of the content 
develops with your note taking.

Probably the best example of a really well organized TW is Garret Lisi's 
Deferential Geometry. I have only the vaguest idea what he's talking about, 
but you don't have to understand the content to be impressed with how he uses 
TW.
