Did this issue start happening after an update/upgrade?  Was there a
prior kernel version where you were not having this particular problem?

Would it be possible for you to test the latest upstream kernel? Refer

v4.16 kernel[0].

If this bug is fixed in the mainline kernel, please add the following

Looking closer at the logs of the crash in comment #81, I see that there are 3 
calls into qlt_unreg_sess(), for the same port, in a span of less than 2 
seconds. Between the time that the first instance of qlt_free_session_done() is 
queued and it actually executes, the port has a re-login event. It's difficult 
to tell exactly what all the login messages mean, but it appears that the login 
procedure continues even though qlt_free_session_done() appears to have logged 
out the port. There are then two more qlt_unreg_sess() calls during the login 
sequence, but both of kworker threads associated with those seem get stuck in 
the "wait for logout complete" loop - at least according to the stack traces in 
the dump. qla24xx_fcport_handle_login() seems to be getting called repeatedly, 
or at least as many times as qlt_unreg_sess() gets called. Unclear if that is 
normal, or a reaction to simultaneous attempts to logout and shutdown the port.

It also seems strange that more mutexing is not happening, although I
can't see conclusive proof that these contradictory events are happening
simultaneously.

I do see that qlt_free_session_done(), among others, makes calls to
qla2x00_fcport_event_handler() which would also handle the login. There
may be some interesting stack traces in that core file, that might give
a better picture of what is going on.

Was that core file preserved someplace that we can look at?

Regarding the "init" condition of the work item in the crash analysis, besides 
INIT_WORK() this condition would also be present after using list_del_init(), 
which is done just prior to executing the work function. So, if multiple pools 
were pointing to this work item, when one (any, all) starts to execute it then 
this condition will exist. This state should show the work item removed from 
the pool, although it is only possible for that remove to work for one pool 
(the last one to which the entry was added), and any other pools will not be 
able to get rid of that work item. The exact result of all this is still not 
clear, but it seems there are several possible ways that the tasks get 
effectively hung.

Unfortunately, the system is unusable this morning. Still trying to
recover it.  May have to flatline it again.

It seems I have gotten myself stuck in a loop:
1. try to reboot and that causes kernel panic
2. after that happens a few times, the NVME needs fsck'd because of corrupt 
group descriptors
3. `fsck -CVvfy` the drive (twice for the ext partition and once for the EFI)
4. after doing 1-3 a few times, packages and symlinks start getting broken.  I 
try to manually repair them until eventually I can't get into the system 
anymore.

I tried to run memtest.  If it is set to 1 cpu at a time, it goes
without error until it eventually hangs on a random (inconsistent) test.
If I run with all cpus, it shows tons of errors pretty quickly.  Always
on the same bit of every bank (ie: 80808080 -> 8080A080) and always off
by two.  But again, it doesn't do that unless multiple cpus are running
at the same time. I thought it could be the other security features
(interleaving, memory encryption, etc) that the BIOS has set to auto.

Launching the live usb and just sitting at a terminal with `journalctl
--follow`, the last thing that happens before it hangs is usually
cleaning temp files; but I haven't run that enough to know if it is a
pattern.

>From the BIOS, I can set it to auto overclock or manual -- there is no
option to disable overclocking; so I cleared the CMOS and tried again
immediately after that without any change.

I have attempted 44 bionic installs this month.  4 of those went through
to completion. Two normal and two minimal.  The rest failed during
ubiquity.

grub-install almost always succeeds when acpi=off and almost always
hangs when it isn't.

I also have to have pcie_aspm=off or the system is spammed with errors
and crashes quickly.  Others have reported the same thing for
threadripper.

I have tried with and without livepatch enabled.

The system is stable when mining or gaming, and seems unstable when
underutilized -- so I tried disabling the C-states in the BIOS.  I have
tried disabling every form of power management I could find in the OS
and in the BIOS.  I am sure I have missed quite a few.

I have tried manually updating the kernel (per your requests) as well as
using ukuu.  Since it is my primary machine, I tend to have things
installed that have to then be uninstalled for that to work well (like
nvidia drivers, virtualbox, etc).

I am seeing a ton of segfaults, even from the live usb.  It more often
happens when the machine is sitting idle for a few minutes (which is
what had me thinking about power management).  I thought it could be the
memory, but since they don't fail memtest (if I run then 1 cpu at a
time)

I know that "Erase disk and reinstall" will not solve the problem.  It
would be nice to figure out how to solve the problem before I do that
again.

So... I'm not sure how I can try a new kernel for you.  If there is some
way for me to update a live usb with an alternate kernel from a live
usb; that might work since I see errors on the daily bionic iso as well.

Wow, that looks *realy* unhappy.

But this requires a *purge*, not a *remove*. Try again with:

sudo apt-get purge python-ubuntu-sso-client

and see if this gets you further along.

I had this issue previously under Zesty and I think it was gone under
Artful (though I'm not sure - the dock started having hardware issues at
that point). I just upgraded to Bionic *and* got a new dock, and almost
immediately started having these issues.

This may be fixed by dkms 2.3-3ubuntu9 which has just been accepted into
bionic-proposed.  It may or may not be the cause of your failure.
Having once been triggered, however, the fixed dkms package will not
automatically fix your upgrade problem.

Please do paste the contents of /var/lib/dkms on your system.

Thanks for your detailed analysis. It was helpful to know all the things
that can nowadays alter ones /etc/nsswitch.conf and change the behavior
in surprising ways.

There is no "wontfix" state in launchpad, so I will mark this as
"invalid" since it was caused by a local configuration issue. Still, as
you said, there is useful information in this bug, and we can always
reopen it if other scenarios where this could happen show up.

That's because in your config tests with vlans on bonds, your call to
ifup -a doesn't actually process the bond or its vlans.  Your ifup -a
starts processing one of the bond slaves, and that causes the bond
interface to by created.  The bond creation is detected by udev, and
udev tells systemd, and systemd then invokes ifup directly on the bond
interface.  By the time your ifup -a has gotten to trying to bring up
the bond (or its vlans), they are already up because of udev detection
which invokes the 'hotplug' ifup that logs to systemd logs, not your
stdout.

ok that seems expected then.  This doesn't 'fix' the problem of the
leaked dst (and namespace), just prevents blocking further net namespace
creation/deletion.

there is a limit, yes, and once it's hit namespace cleanup will
essentially stop, and all further net namespaces will be leaked.
However, there's really just no way to avoid that without fixing the
actual dst leaks, which I'd still like to try to add debug to help
identify.

I've been busy elsewhere last week but I'll prepare the patch to send
upstream this week, and then look at adding debug to actually find the
dst leak.

This is very likely a duplicate of LP: #1766312/LP: #1765939 which has
been fixed in shim-signed 1.34.5.  It is possible that this also
explains why you didn't see a prompt on reboot: the path which triggers
this bug is if you don't enter the same password twice, and if that
happens two times the package maintainer script will break without
saving this setting.

After installing this new version of the package you will want to run
'sudo update-secureboot-policy --enroll-key' to complete the secureboot
policy change.

This bug is missing a justification for why this needs to be in Ubuntu
18.04 LTS.

It is now very late (we are in Final Freeze) and this package is
included in the Lubuntu, Ubuntu Studio, and Xubuntu images.

If it doesn't make it in before release, you are welcome to do a Stable
Release Update instead.

It doesn't work on 16.04 and 17.10 for different reason. The file
/etc/killpower is created but the script /etc/init.d/halt have never
executed on 16.04 and isn't present at all on 17.10. No one is calling
nut-client and nut-server with the "poweroff" parameter. I've tested it
on vm with debug print to serial port. I don't get how it is supposed to
work.
