Ubuntu LTS has special policy for some programs (e.g. Firefox) to
upgrade them. I'm using Ubuntu 16.04 and have latest stable Firefox. So
this is not a problem.

So this ideapad Z510 came with FreeDOS, ran nicely since and under
Ubuntu 12.04 (up to .5) LTS and never had Windows 7-10 on it.

Rebooting to USB and mounting the root partition:

mount /dev/sda6 /media/ubuntu/GUID
mount /dev/sda5 /media/ubuntu/GUID/boot
mount /dev/sda7 /media/ubuntu/GUID/usr
mount --bind /dev dev
mount --bind /dev/pts dev/pts
mount --bind /proc proc
mount --bind /sys sys
chroot /media/ubuntu/GUID
grub-install /dev/sda
Installing for x86_64-efi platform.
grub-install: error: cannot find EFI directory.

At least /boot/grub at this point got populated with the modules that
were missing before.

No partition has the ESP flag and/or file system type EF.
The 18.04.1 installer seems to neither expect this (which it should check 
upfront rather than near the end of a lengthy install) nor try and create one, 
or a way for non-EFI (legacy) boot.

Anything into grub-install /dev/sda --efi-directory=/boot/efi(/EFI(/ubuntu)) 
yields
grub-install: error: /boot/efi(/EFI(/ubuntu)) doesn't look like an EFI 
partition.
...but these directories wouldn't meet the specifications 
https://wiki.archlinux.org/index.php/EFI_system_partition#MBR_partitioned_disks 
of a dedicated(?) primary(?) 100-256MB FAT32 anyway.

For the record, the installer crashes whether online or not,
and even after manually setting the /dev/sda1 partition type to 0C,
but the system in Legacy mode (UEFI-only fails to find media) does restart into 
a GRUB rescue prompt,
so /dev/sda itself is bootable: just the next-stage config obviously wrong.

On 16.04.2 LTS, xine installs to the toolbar but fails to start without 
perceptible cause.

If Right Ctrl is defined as the (standard) Compose key,
VirtualBox must not default to Right Ctrl as Host Key (anymore),
which it will never recognize as such then,
yet this trap persists through Ubuntu 16.04.2 LTS.

When caught in a VM window, few users will find the "emergency exit"
back to Unity of killing the host process from a Ctrl-Alt-F1 console
(text/character mode) shell.

/xineliboutput-sxfe:vdr-sxfe.desktop has no effect this time around,
even after reboot.

BTW /usr/share/app-install/desktop/xineliboutput-fbfe:vdr-fbfe.desktop
has it too.

Since /var/cache/vdradmin-am/usr/ through 
/var/cache/vdradmin-am/usr/share/vdradmin-am/template belong to root:root 
rather than vdradmin-am:vdradmin-am (and look like they have wrongly been 
placed into a relative path),
the Perl Zlib issue can be fixed by sudo chown -R vdradmin-am:vdradmin-am 
/var/cache/vdradmin-am/usr && sudo service vdradmin-am restart

Still the same old (and now new) bugs in Ubuntu 16.04.2 LTS,
not logging or returning any reason why "sudo service vdradmin-am start had" no 
effect either.

For a web interface, there really shouldn't be a weak, plaintext default
PASSWORD = linvdr in /var/lib/vdradmin-am/vdradmind.conf in 2017 (or
ever have been).

After logging in, one is greeted with just this:
Not a SCALAR reference at /usr/share/perl/5.22/Compress/Zlib.pm line 482.

Users are also likely to be misled by (what is only due to their
respective rights):

$ vdradmind

@Rolf - thanks for mentioning iptraf-ng. It's working fine here on
Xenial 64 bit. IMHO in scenarios such as these where a 'mother' package
becomes obsolete and is replaced by an '-ng' version, the old package
should be shipping with a warning upon installation that it's deprecated
and should be replaced with the new version.

How else should less-informed mortals (such as the bug reporter and
myself) know iptraf-ng even exists?

As the Bluetooth stack is working these days, even on the 64-bit edition of 
Ubuntu 16.04.1 LTS, most of the recommendations to aggressively reinstall or 
reconfigure seem obsolete indeed:

I.e. the workflow when pairing a phone needs to be fixed to include prompting

as well as hints to turn Bluetooth off and on again once after pairing, on both 
the computer and the phone,
and that a little lock icon appears next to the Bluetooth icon in the taskbar 
once a file transfer has successfully been initiated.
Also, the meaning of the Reveal button in the pop-up notification may need to 
be clarified.

However, another error follows, also repeatedly:

For this one, suggestions not directly applicable to LTS seem to be made in bug

I concur with Armindo, this is a bug (in the installer?) which has been
around for many, many years. Apparently the system fails to do a proper
locale-gen at the end of the installation.

When installing Ubuntu, I always choose US English as language, time
zone The Netherlands, keyboard layout US International (dead keys). This
combination of settings *ALWAYS* produces this result:

"locale-gen step" should be automatically done during the installation
of the system (and if this is done already, it's doing it wrong and
should be fixed).

This is still not properly integrated into Ubuntu 14.04 LTS (especially
noting how it has been advertised for years, see above):

The user has to find http://www.citrix.com/downloads/citrix-
receiver/linux/receiver-for-linux-130.html and be knowledgeable about
CPU architectures to navigate his or her way past the ARM opening by
default to AMD64 (even on Intel) or x86, then know to look for a .deb
archive (i.e. be acquainted with Ubuntu's upstream and the package
management thereof) and get past the pop-up suggesting some "secure
downloader" to manually retrieve the archive.

Then on icaclient_13.0.0.256735_amd64.deb Ubuntu Software Center will report 
the following anyway:
icaclient
Citrix Receiver for Linux
Dependency is not satisfiable: ia32-libs

(cf. http://ubuntuforums.org/showthread.php?t=2176616#post12798626 on
this one: 'ia32-libs is past "being phased out", it's gone.')

Just in case anyone's wondering: Counterintuitively trying to use 
icaclient_13.0.0.256735_i386.deb on 64-bit will hit another block at the same 
point:
Cannot install 'libwebkitgtk-1.0-0:i386'

That's worlds away from what a user of remote managed desktops can be
expected to figure out solutions for.

(And don't get me started on the EULA...)

installed the year-old elementaryOS.org Luna as a lightweight Ubuntu
12.04 LTS derivative.

Had to use both

May return to Lubuntu 14.04.1 LTS for further testing soon if a plan is
in place indeed to actually debug these issues (rather than additionally
setting the hoops on fire, once anyone actually volunteered to jump
through them ;)).

Except for some erratic flickering on resume, the mainline kernel just
out returns from suspend without crash after this & a reboot into it:

sudo bash around it all, since under Ubuntu in the words of Connor
"there is no root but the one we make for ourselves" (and wget repeated
before the last URL of course).

To no avail anyway (while suspend worked under e.g. JoliOS 1.2):
1. Screen stays black after reopening the lid.
2- Machine will blink it's "crescent moon" sleep-mode LED but won't wake on any 
keypress (including power button) after being sent into Suspend via the 
bottom-right taskbar entry.

Hibernate also has a menu entry but returns immediately to the
LightDM(?) logon prompt, just like under the standard kernel.

You certainly realize that this is an impossible feat for most people to
even DARE TO pull off without a dedicated test system.

Upstream testing as things stand is beyond Rocket Science for in the
latter one would have at least a trajectory. ;-} Getting fixed at some
point in a higher kernel (far beyond what the LTS will ever see) helps
only temporarily if the issues (with references to related bugs removed)
keep regressing - and mostly remain undetected as the bug reporting pop-
up at reboot does not actually send out anything through Firefox.

Please specify in all of these cases what you understand to be "the very
top line at the top of the page (not the daily folder) following
https://wiki.ubuntu.com/KernelMainlineBuilds";, or better yet, the
respective wget&dpkg commands (this not even being possible from a
standard repository through the default synaptic or apt?!):

Freezes at resume also occur on Kernel 3.13.0-32-generic #57-Ubuntu SMP Tue Jul 
15 03:51:08 UTC 2014.
While the /usr/share/apport/apportcheckresume reporting window pops up 1-2 
reboots later, it is not traceable how https://wiki.ubuntu.com/Apport "uploads 
the collected information to the bug tracking system. After that it opens the 
packages' bug filing page with a sensible default bug title and leaves the rest 
of bug filing process to the web UI."
Neither Firefox opens nor anything in it.
If there is any way to find the uploads and verify whether they have been 
completed, please advise.

Christopher, why is this supposed to be a "graphics driver issue" that
would need to be split up into isolated bugs per GPU and per system (so
that none shows the "big picture" and aggregate Bug Heat of likely Mode
Setting or similar hardware re-initialization troubles on resume) ?

With the references of the original report

individualized instances may be made appear as of Low importance and
remain Unassigned for long (to what end though?), but freeze-on-resume
means a serious total loss of all unsaved data, potential file system
corruption and severe risk recurrent from earlier LTS releases where the
circumstances triggering it and their "solution" (some kernels simply
unaffected) never seem to have been fully understood either.

Guidance as to what contributions may be provided for the purpose of
debugging is much appreciated.

Per Wahlström at https://bugs.launchpad.net/ubuntu/+source/pm-
utils/+bug/999638 #18 et seq. had already made every conceivable effort
more than 2 years ago but there still seems to be no indication who
picked up how from there.

The following from a resumed-to Ctrl-Alt-F2 text console results in the
same sort of crashes, so the cause seems to be something other than
(only) the display auto(re)detection:

chvt 7
xrandr --newmode "1920x1080_60.00"  173.00  1920 2048 2248 2576  1080 1083 1088 
1120 -hsync +vsync
xrandr --addmode DFP-1 "1920x1080_60.00"

The final crash hit so hard it corrupted all of /dev/sda's file systems
(requiring to press F on each to even reboot).

Looks a lot like https://bugs.freedesktop.org/show_bug.cgi?id=35648 back
with a vengeance.

Curiously I can actually resume from a manually typed pm-suspend as root
to the Ctrl-Alt-F2 text mode console without immediate crash.

However, even chvt 7;xrandr --auto will freeze the machine (including
keyboard LEDs) afterwards, i.e. I cannot get back to anything running on
the graphical desktop.

dmesg just before shows the attached entries, interestingly hinting at a 
recurrent problem of reading the display descriptor:
HDMI: invalid ELD buf size -1

Now I wonder how I can force-feed an overriding EDID file to the new
driver(s) - as used to be possible through the old /etc/X11/xorg.conf
when there still was one

, and whether that might work around the issue.

Also affected on a Hewlett-Packard HP Compaq dc7800p Convertible
Minitower/0AACh, BIOS 786F1 v01.32 07/21/2011 with nvidia GeForce GT 520
board.

Workarounds by Rebecca Palmer at #5 (many thanks!) did not entirely
suffice: Had to additionally add "blacklist nouveau" to
/etc/modprobe.d/blacklist.conf even after sudo apt-get remove nouveau as
well as de- and reselecting the proprietary one from System Settings /
Software & Updates / Additional Drivers - which (even while the system
was actually using nouveau) had been shown all the time as "Using NVIDIA
binary driver - version 311.38 from nvidia-331 (proprietary, tested)"
with a green light for "NVIDIA Corporation: GF119 [GeForce GT 520" and
as "1 proprietary driver in use."

Symptoms:

nvidia-settings shows only minimal menu with no information on
monitor(s) or GPU.

prime-select query returns the string "unknown" rather than "nvidia".

/var/log/syslog is flooded with lines (dozens per second) to the tune of
"kernel: [ 1195.548660] nouveau W[   PFIFO][:01:00.0] INTR
0x0100: 0x0005".

/var/log/syslog has these lines:
kernel: [   11.695523] NVRM: The NVIDIA probe routine was not called for 1 
device(s).
kernel: [   11.695526] NVRM: This can occur when a driver such as: 
kernel: [   11.695526] NVRM: nouveau, rivafb, nvidiafb or rivatv

nouveau (or possibly just some of the related mode-switching) is
deprecated due to resume issues in Ubuntu 14.04 LTS (of which this is a
fresh install)

Remaining issue now that the proprietary nvidia driver finally loads is that 
Systems Settings / Sound / Play sound through: "HDMI / DisplayPort2: GF119 
HDMI Audio Controller" works only intermittently and for 2 rather than 5.1 
channels.
Any (auto-)tweak for the ELD (captured through the "Acquire EDID..." button 
from the DFP-1 menu of nvidia-settings) to be loaded from file (whichever comes 
in place of /etc/X11 these days) rather than by DDC would be very welcome.

Other than the shinier graphical boot, no improvement whatsoever from
switching back to Nouveau (which does not seem to support HDMI Audio
though, i.e. only usable on monitors that still alternatively allow
analog sound), on HP dc7800p CMT with GT520:

The apport reporter just appears repeatedly afterwards (still not
popping up the browser or indicating where the collected crash data has
been stored).

via sudo nano -w /var/lib/vdradmin-am/vdradmind.conf
and ENABLED="1" (from 0) in sudo nano -w /etc/default/vdradmin-am,
then sudo service vdradmin-am start (an unexplained sequence inextricably 
complicated for most in 2014).

It is possible to select Try rather than Install on the Live CD, then open a 
Ctrl-Alt-T window and invoke the following before starting the installation 
process via the icon on the trial desktop:
sudo rmmod mei_me
sudo rmmod mei

After installation has completed, /etc/modprobe.d/blacklist-mei.conf may be 
created as a workaround with these contents (then reboot):
blacklist mei_me
blacklist mei

Even in Ubuntu 14.04 LTS, /etc/default/vdr is still missing ENABLED=1,
so cannot auto-start the service (cf. user review of the package).

/var/lib/vdr/setup.conf still has a MinUserInactivity = 300 rather than
0, i.e. will inappropriately shut down the service after a while.

Users have little chance to find out that these changes are required,
nor are they informed about needing w_scan (in package w-scan) to
populate /var/lib/vdr/channels.conf - let alone femon (missing from the
default repositories) to get signal parameters.

The hints to disable "light locker" presumably refer to Xfce, i.e. are
not workarounds on the default Unity, where the machine crashes on
resume in spite of all equivalents turned off in the attached settings
(and locking up without even permitting Ctrl-Alt-F1 or -Del).

3 apport crash reports for an HP "dc7800p" (with package nvidia-331)
have been submitted within the past 30 minutes, but since no reference
is displayed (no "bug filing page" displayed after upload, contrary to
https://wiki.ubuntu.com/Apport#What_does_it_look_like_for_users.3F),
someone else will have to dig them up wherever they have gone, and
attach them to this bug.

Sigh (or in Bavarian actually, fittingly "mei" ;-)), this same bug since
12.04 seems back with the Ubuntu 14.04 LTS, stopping the installer
(which loops slides infinitely without message nor progress) after it
has mounted the /-to-be below /tmp to copy from USB.

Using the daily 64 bit image from Feb. 27, 2014, I tried installing
Ubuntu today from a USB stick to the secondary partition of my laptop's
SSD.

Since I needed to install it to the secondary partition, I chose 'custom
install'. I selected sda2 as the root partition, chose not to format it
as it had already been formatted (with ext4) and I wanted to keep the
disk label intact. I then got a warning that I didn't have a swap
partition. But since I have 8GB RAM and do not use hibernate, I decided
to continue.

I selected 'Download updates while installing' and 'Download 3rd party
software'.

Somewhere along the installation, while I was clicking through the slide
show, ubiquity crashed.

I'm confused. I just got this error while trying to installing linux-
generic-lts-saucy on my 12.04.3 machine. The build for both nvidia-304
and nvidia-319 failed:

nvidia-304 304.88-0ubuntu0.0.3: nvidia-304 kernel module failed to build
nvidia-319 319.32-0ubuntu0.0.1: nvidia-319 kernel module failed to build

While I understand the build failing for nvidia-304 (which has status
'Triaged'), I don't see why it would fail for nvidia-319 (which has
status 'Fix Released').

I wonder if either:

1. The bug was fixed for nvidia-319-updates instead but nvidia-319 was 
mistakenly tagged 'Fix Released'
2. The bug has actually not been fixed in nvidia-319
3. I'm experiencing a different bug

I will install nvidia-304-updates and nvidia-319-updates to see if that
fixes anything (I expect it will only work on 304-updates), anything
else I can try?

When running saucy final beta from a USB stick (option "Try Ubuntu before 
installing"), ubiquity does not show an interface when started. As a result, 
Ubuntu cannot be installed.
- Double-clicking the icon on the desktop seems to produce no result (nothing 
happens)
- Clicking on the 'Install Ubuntu 13.10' icon in the shortcut bar on the left 
side has the same result (none)
- Running "ubiquity gtk_ui" from a terminal fails:

Ubiquity is already running!

- After doing a "sudo killal ubiquity" and then running "ubiquity
gtk_ui" again, no warnings are displayed, but the ubiquity user
interface still does not show.

After killing ubiquity again (from another terminal, CTRL+C does not
work), the terminal shows the following information:

nor with nvidia-319 per

seq., calling the beta a "fix released" (in particular to an LTS) may be
premature, even though no crashes have been detected yet on Saucy in
further testing of HDMI audio with the latter proprietary driver either,
and after installing pavucontrol from the Universe repository.

Neither System Settings / Sound (which will not switch back to 5.1 from
a 2.0 speaker layout, and keep the 5.1 speakers silent even if shown)
nor pavucontrol (which does not have HDMI audio in the left tab's drop-
down at all) offer a usable selection (and testing option) for the
surround output on Saucy anyway as of yet, so this is limited to command
line use with speaker-test -c6 and the like at this time.

resumes from suspend only to a black screen without access to a Ctrl-Alt-F1 
etc. text console or even working keyboard LEDs.

Interestingly, it detects what the earlier kernels listed above seem to
have had trouble handling on resume:

Using nvidia-319 (rather than nouveau), there seems to be "room" for
flooding the syslog with mei_me errors instead now (alternating
unexpected resets, disconnects/reconnects) as in

previously made 13.04 unbootable (from USB at least).

[Impact] 
Playing sound, opening pavucontrol mixer or hotplugging HDMI sink hard-freezes 
Ubuntu 12.04 LTS unless module snd_hda_codec_hdmi is blacklisted.
This is neither a workaround nor remedy, as additional digital sound cards 
would have to be installed (and fine-tuned for delay compensation) to 
re-establish required functionality thus removed from HDMI.
Similar issue exists in https://bugs.launchpad.net/bugs/1169984 & duplicates, 
potentially making access to display or even surround amplifier a DoS vector.

[Test Case]
While running speaker-test, VLC, Xine or other sound sources, open pavucontrol 
or connect another monitor/amplifier combination that is a sound sink with 
different speaker setup.
Playing sound before and after amplifier is turned on may suffice to reproduce 
crash, but no 100% trigger has been found yet.

[Regression Potential]
Per https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1169984/comments/124 
the bug may actually be the regression.
Current situation is to risk severe crashes (for reasons inscrutable to average 
users) or lack proper audio (after module blacklisting, which also not feasible 
for many end users).
Avenue&side effects to potential fix yet to be determined.

[Other Info]
VDR / Home Theater systems presumably have to remain on 12.04 LTS for years.
Proprietary Nvidia driver may be implicated.

but an Ctrl-Alt-F1 console is still usable,
showing a syslog full of nouveau PFIFO warnings (mostly INTR)
and (unlogged) errors to the console, some referring to SUBFIFO 0: ch0 [DRM].
No shortage of mei_me entries either, and a stack dump which I will try to 
extract for you.

In response to various requests by the author of #29:

No luck with 13.04 which keeps repeating mei_me issues on a text console
without even booting to the graphical desktop.

In a multi-hour effort, the

beta-released/ just out though boots from USB, albeit slowly, and then
showing only Front Left&Right speakers in Unity's "Systems Settings /
Sound" even when set to 5.1 for the GF119 HDMI port.

However, once figuring out the only (sub-)device that works, Saucy
13.10ß2 producing a slightly different ELD (all as attached in this and
subsequent posts) can be taken through the command-line speaker-test -c6
motions in pink-noise, sine and wave without provoking the freezes that
haunt 12.04 LTS.

pavucontrol is not apt-get installable though in the (USB-booted) beta.

With no known method of USB-booting in a way that includes the
proprietary Nvidia driver and hibernation, as a mere mortal user with
one single machine in this place I do not yet see a feasible approach to
more elaborately test this (beyond what you call "Incomplete"), short of
