I don't think this bug is confined solely to fglrx. I'm running a clean
install of Arch Linux and using vga_switcheroo with xf86-video-intel and
xf86-video-ati and when I switch to the Radeon card X segfaults in an
almost identical way. Don't know of the root causes are the same...

-I installed windows 7
-I installed Ubunutu 12.04
-installed grub
-system worked on the laptop, no external monitors yet
-connected external monitors (one vga, one hdmi, one displayport). Only 2 
worked, the displayport monitor not recognized at all
-I upgraded all of my packages via the update manager
-rebooted
-when my system booted up, I got a blank screen. I tried rebooting a few times, 
trying the recovery mode into basic graphics but that didn't help.
-I unplugged everything including my external monitors and then rebooted, and 
then my system booted properly and then I could plug in my 2 external monitors 
again and everything worked.
-I tested a couple of times and if I have the monitors plugged in I get various 
errors like 'bad lun' or graphics error during boot, errors shown in the 
console, no gui

the right protocol to do this:

First you should test the bug yourself, in this case it's really easy.
Second if you can't reproduce it you should ask the original reporter or
the others affected by the bug to see if they can still reproduce it.

You can just go to http://get.webgl.org/ and see that webgl in fact
still does not work with Chromium. At least for me it doesn't even tho
it does with Firefox.

Fair enough.  I never was one for the verbose messages, but I did
consider asking for a fresh set of logs from Quantal.  In the end I

Chrome or Chromium so can you please go to that site you mentioned in
the last paragraph of comment #9 and run apport-collect 685321

And of course let us know which version of Chrome/Chromium you are using
and whether you manually over-rode the gpu blacklist/software rendering
list or not.  I'm guessing that you are doing this because the page at
http://support.google.com/chrome/bin/answer.py?hl=enanswer=1220892
states that on AMD/ATI cards on under Linux WebGL is unsupported and is
disabled by default.  I would assume that Google have already talked to
ATI/AMD about this for the binary fglrx (Catalyst) driver and to the
kernel, Mesa and DDX developers about this for the open-source radeon
drivers.

Way too late for this, had to remove Ubuntu for this and other reasons
(wifi wouldn't re-connect after a dropped connection and other things as

   -I bought a new alienware laptop with nvidia GT 650M graphics card and
 onboard Intel HD 4000 video
   (http://www.dell.com/us/p/alienware-m14x-r2/pd.aspx, click tech specs
 to see hardware)
   -I installed windows 7
   -I installed Ubunutu 12.04
   -installed grub
   -system worked on the laptop, no external monitors yet
   -connected external monitors (one vga, one hdmi, one displayport). Only
 2 worked, the displayport monitor not recognized at all
   -I upgraded all of my packages via the update manager
   -rebooted
   -when my system booted up, I got a blank screen. I tried rebooting a few
 times, trying the recovery mode into basic graphics but that didn't help.
   -I unplugged everything including my external monitors and then
 rebooted, and then my system booted properly and then I could plug in my 2
 external monitors again and everything worked.
   -I tested a couple of times and if I have the monitors plugged in I get
 various errors like 'bad lun' or graphics error during boot, errors shown
 in the console, no gui

different boot failure today, never made it to the low graphics mode
screen. It stopped with the following text on the screen:

process 422: arguments to dbus_server_disconnect() were incorrect, assertion 
old_refcount  0 failed in file ../../dbus/dbus-server.c line 786.
This is normally a bug in some application using the D-Bus library.
process 422: arguments to dbus_server_unref() were incorrect, assertion 
old_refcount  0 failed in file ../../dbus/dbus-server.c line 749.
This is normally a bug in some application using the D-Bus library.

I found this in the boot.log

Nice of D-Bus library to blame something else, but not bother to name
that something else.

Seems impossible currently to use Intel integrated graphics (Sandybridge

Hmmm, if it's officially not supported then I guess there is nothing we
can do. I have a Optimus dual-GPU configuration with Nouveau driver so
it's not a surprise that it doesn't work.

Maybe you are right and this bug should be closed and new ones should be
filed if someone has this problem with supported hardware. Even the
original reporter doesn't have supported hardware.

T430S :

2. apt-get upgrade
3. Removing /etc/X11/xorg.conf if exists

I was having similar issues on Ubuntu 12.04.1 with my Dell Latitude E6530 
(Intel graphics). 
It would completely freeze at random points during the day, most likely when 
browsing the web using Chrome/Chromium.

It seems that after doing a apt-get dist-upgrade with 
ppa:ubuntu-x-swat/x-updates set up as a source, I have not seen the issue :o)
Currently running 3.2.0-33-generic

I guess this bug is a conglomeration of a number of issues with similar
symptoms, but hopefully this helps someone.

I suggest changing title in Ubuntu 12.04 always resets refresh rate to
60 hz after reboot,  I have nvidia latest 304.60 on lcd monitor  that
starts with proper resolution after adding lines in xorg.conf section
monitor and section screen but cannot start with my preferred refresh
rate of 75 hz always have to use nvidia-settings, I tried using xrandr
to force 75 hz  with the command xrandr -s 1440x900 -r 75 but it sais
Rate 75.0 Hz not available for this size, so it seems as if X completely
avoid taking into consideration refresh rate different than 60 hz, here
my xorg.conf

Just to add... the fix of downgrading my packages has also activated the
following bug:

8086:2562 [drm:intel_prepare_page_flip] *ERROR* Prepared flip multiple times 
every frame with intel graphics

This i originally got around by using the following commands:
sudo add-apt-repository ppa:glasen/intel-driver
sudo apt-get update
sudo apt-get install xserver-xorg-video-intel
but this does not work anymore?

so i am officially at a loss with what to do.

It looks like the nouveau driver may be interfering. Try blacklisting it
and see if that helps. Use these commands and then restart

sudo depmod -a

The content of new tabs in Firefox is sometimes corrupt - the content is
not drawn in the correct location or it's shifted/wrapped around in the
current window. There can also be visual artifacts such as 'static' in
certain parts of the window.

I use fullscreen windows almost exclusively, and I've seen the problem
twice today when opening documents in a new tab in a fullscreen window.
I don't know if the problem affects windows that are not fullscreen
windows.

This is a recent regression; it started around the time I upgraded from
unity 6.4.0-0ubuntu4 to unity 6.4.0-0ubuntu6.

See the attached screenshot for an example. I've blurred or redacted
parts of the window. The multicolored static at the bottom right is part
of the image, not something I've added. The big black rectangle is
content I've redacted.

Things to note in the screenshot:
* There appear to be 5 distinct panes: left side, right side top, 'static', 
center bottom, right side bottom. Each contains different parts of the browser 
window.
* Content in the bottom-right of the window is scaled down a lot compared to 
the rest of the content
* Content in the bottom right pane might be from another tab - note that it's a 
Google Doc that includes the text, All changes saved in tiny letters. The 
content of the main tab is another Google Doc, which has different text in the 
same position: Last edit was made redacted

When the FF window is in this state, a workaround is to take the window
out of fullscreen mode; it redraws properly. The windows stays drawn
properly if you make it fullscreen again.

To reproduce:
1) Open a new tab

Expected results:
New tab is drawn properly

Actual results:
New tab is not drawn properly; it may be shifted/wrapped around the screen, and 
may have other visual artifacts.

Mesa 9.0 was released in Ubuntu 12.10 because this bug is not known to
be a problem any more with the latest Compiz/Unity packages released
with 12.10. So the original bug in Mesa is largely irrelevant right now.

experiencing the same issue. If you think you are experiencing this bug

I believe bug #1068165, bug #1069774, bug #1069210, and bug #1075027 all
have the same underlying cause, but I'm not marking them as duplicates
of each other because (1) I'm not 100% certain, and (2) I want people to
be able to find the bug reports when the automatic crash reporting
prompts them to do a search (I believe marking a bug as a duplicate
hides it from the search results, which would lead to more duplicates
being filed).

If someone can verify that these bugs are all caused by the same
underlying issue, and can confirm that marking a bug as a duplicate does
not keep people from finding it via automatic crash reporting, then
please pick one of the reports as the master (probably bug #1068165) and
mark the others as a duplicate.

I can add the AMD Firestream 9250 to the list of affected video cards -
just upgraded and I've got the same problems as everyone else in this
report. I'll try the open source drivers until I can get enough of a
working desktop to investigate further. (I'm having trouble even viewing
a website on firefox on a 24 monitor!)

HD 3000 or HD 2000 series card as noted in the Bug Description and Bug
Summary, your problem is not covered by this report. As well, as per

AMD is still releasing new beta upstream versions of fglrx for your
card, while this has not occurred for HD2000-4000 series cards.

I upgraded my desktop at home to 12.10 today, and wound up with complete 
failure of graphics: compiz would not work and hence Unity would not start - I 
had only a desktop and a terminal (accessed via Ctrl-Alt-T). Screen resolution 
was very poor (and used only 75% of the screen on a 24 monitor) - I couldn't 
even view the full width of a page in Firefox.
Apparently AMD is still releasing drivers for the Firestream series, so I don't 
know what went wrong in my case. I can try reinstalling fglrx to see what 
happens, or whatever suggestions might help diagnose the issue.

WORKAROUND: I've managed to get back to a working desktop by purging
everything related to fglrx and using the open source drivers.

failure of graphics: compiz would not work and hence Unity would not start - I 
had only a desktop and a terminal (accessed via Ctrl-Alt-T). Screen resolution 
was very poor (and used only 75% of the screen on a 24 monitor) - I couldn't 
even view the full width of a page in Firefox. It sounds like the same problem 
as bug #1058040 but my card doesn't fall into the category of AMD cards that 
are no longer supported by the fglrx driver. I've managed to get back to a 
working desktop by purging everything related to fglrx and using the open 
source drivers.

Public bug reported:

Ubuntu 12.04 with Ati Mobility Radeon 5400 using the standard fglrx
driver (updates don't install properly).  Each time I try to enter sleep
mode and wake, all I get is a black screen with no underlying
functionality (I tested loading a terminal with key shortcuts and
entering commanding a shutdown.

I have a bit more information.  Same symptoms, different desktop
managers.

I've noticed that this is *not* happening in Unity or KDE.  However, it
*does* occur with both Xfce (known) and LXDE.  Not sure if that helps,
but it might help find a common factor.

I tried the workaround, but ran into some problems:
 - in step 5, the command apt-get -y reinstall was not recognised. The 
reinstall seems to be the wrong code. I couldn't recall what the correct 
command was, so I did this step via synaptic.
 - step 6 failed because there was no package fglrx-installer. The repository 
I added in Step 2 didn't have this package, but it did have plain ol' fglrx, 
so I tried installing that instead.
 - In the final step (7), the command aticonfig --initial returned
aticonfig: No supported modules present

Rebooting resulted in the same mess as before, so I purged fglrx again,
and rebooted with the open source drivers.

This sounds a little circular. It doesn't work with Mesa - it's
blacklisted by google - not a bug because it's blacklisted?

FWIW it seems to work fine with Chromium from Precise after disabling
the blacklist. Wonder what's different.
