Few weeks ago we moved from 10.04 to 12.04 at work. Mostly because of
newer machines had problems with older kernels (no SATA, no sound). I
have to admit that 10.04 was light years away from 12.04 when it comes
to stability. There was mean time between crashes measured in weeks,
when 12.04 can have 10 crashes in one hour. It's really disappointing.
Right now I'm upgrading kernel to 3.2.0-41 and hoping it will fix
freezes and crashes before coleagues at work crucifies me.

Will report after few days if that helped. If not, I have no clue what
will I do since I'm running out of options. So if anyone can help me
pointing in right direction or what can I do to help developers find and
kill this annoying bug, I'm here.

Oh and those 60 machines at work are all from DELL, models Optiplex
380SFF, 390 SFF and 1030SFF. Just what's built in, with Intel graphics
and all runing on Gnome Classic - no Unity. All machines are connected
to network and has mounted Samba and NFS shares, each one of 100 users
uses mostly Firefox, Thunderbird, LibreOffice, Audacity, VLC and some
custom developed software.

Ironicaly Ubuntu HCL lists those machines as compatible when clearly
they far from that.

Seems that strange behaviour happen after I start system to Windows 7, then 
restart my laptop (soft reboot) to Ubuntu.
Restart & restart again to Ubuntu not make brightness keys working.

The issue solved when I shutdown my laptop (hard reboot) then power on
again to Ubuntu. --> brightness keys working again (even on AC powered)

Still broken in 13.04. using a "noname" mouse. lsusb shows "Pixart
Imaging, Inc." as vendor. Problem occurs randomly, but sometimes really
often (every third click).

I am having this problem on two machines as well. Both are as follows:

1. Dell Precision M4600 with Ubuntu 12.04 (fresh build within 30 days)

2. Dell Precision T1650 with Ubuntu 12.04 (fresh build last week)

I am using NVidia drivers at the moment, and will deactivate them for
now, as my symptoms are exactly as described.

tried to bisect this, but I can't see any difference in the xev output
before or after that commit. Tested several revisions after (and
3e15158985) and xev works as expected.

fwiw, my test box here is Ubuntu 12.10 with the server branch above,
rest as-is. mouse used is a trackpoint, which for all purposes looks
like a mouse.

test case was xinit /usr/bin/xev -- /opt/xorg/bin/Xorg -retro, then
clicking+dragging into the xev window. events as expected.

nexus valgrind log for latest attempt

Still a bit buggy. On the nexus7 I can cause it to drop events in the
same way still..

What I do is touch the ubuntu dash icon in upper left, then release
finger and make a dragging motion with the dash icon. I'm not 100% sure
if the touch was fully released, or it just stopped registering my
finger. But this (still) results in the following spam from xserver:

Stopping lightdm doesn't crash any more and shows no leak. Only thing
that may or may not be relevant is a still reachable warning:

I get these errors when trying to install or remove any of the nvidia
listed drivers. Since I cannot remove the driver I have to reinstalll to
get it gone. This would be my 5'th time reinstalling. I have tried all
the drivers. If there is a better way to remove the driver please let me
know so I can help test.

VGA compatible controller: NVIDIA Corporation GF106M [GeForce GT 445M] 

I really need to run the Nvidia driver. Nouveau does not cut it and

Wait: why is this marked "fix released"?  Problem exists on a fresh
install of 13.04.

I don't so much care about compiz not having the right refresh rate
OOTB, but changing it and having that setting forgotten after a reboot
is *maddening*.  It shouldn't take to much to make sure two settings
(detect refresh rate, manual refresh rate entry) don't erase themselves
every boot, and I'd consider the problem fixed if this were the case.

But having to resort to a kluudge (bash script with sleep 5 in it?
Really?) isn't really... optimal.

my patch was only tested on my old box, sis661 chip.
gentoo
xorg server 1.13.0
X crashed on exa code at startup, and the patch just fixed it.
I don't have any other sis chip machines, so I can not test it more.
anyway, I think my patch may be helpful here.

btw: the motherboard of my old box was broken some months ago. any
deeper investigation is now impossible for me  :(

I don't think so.  The screenshot in that bug shows just a couple of
horizontal lines crossing the screen, whereas in my case the entire
display is briefly overlaid with lots of intersecting black lines.
Also, that bug describes lines which stay on the screen for a long time
(even across reboots!), where in my case they only flicker on screen
briefly.

The issue is caused by /etc/X11/Xsession.d/10fglrx file incorrectly

This bug no longer occur since I changed the BIOS Setup and increased
the video memory to 128M. The problem is not related to Chrome nor
LibreOffice, but the NVidia driver running on HP Pavilion dv6000
notebook.

I emphasize that this bug is the same bug 568492. It is a duplicate.
Both effects disappeared after increasing the video memory.

The naming might be confusing, but i915.ko *is* the correct kernel
driver for your graphics chip. And i915_dri.so is the right OpenGL
driver.

xinput_calibrate is also buggy (xorg.conf.d output is wrong; path and
"Options", like e.g. "BottomX" is  "MaxX" etc. are wrong)
