I'd like to note that, on Xubuntu at least, these crashes are eliminated
by removing libva-wayland2.

When I came back to my computer, it was unresponsive to keyboard/mouse
input and the display was frozen. Upon rebooting, I found the following
error in the previous boot's log:

Unfortunately, Xorg.log had already been rotated, so I was not able to
capture the issue from Xorg's side. I have attached apport output from a
successful boot.

I am using Ubuntu 20.04.06 with xserver-xorg-video-nouveau 1:1.0.16-1
and linux-image-5.4.0-146-generic version 5.4.0-164.163. My graphics
card is an "NVIDIA Corporation GK107GL [Quadro K2000]".

It's possible this is a duplicate of Debian bug #864349

from upstream Debian no longer pull it in. Until the xorg packages have

The problem exists with the AMD graphics drivers and using Xpresent.
Window managers can work around it by not using Xpresent, but there's
also a kernel fix available.

  Update based on further research.
  
  This only happens when the secondary external display is operating at a
  different pixel width to the internal. In this case eDP is 1920x1080
  whereas the external HDMI-A-0 is natively 1680x1050.
  
  It is caused by xfwm4's recent switch from using glx to xpresent for AMD
  GPUs.
  
  The underlying bug is in the AMD driver.
  
  I was able to reproduce on an external 1920x1200 display only when it
  was set to a non-native 1680x1050 resolution.

  Two identical Lenovo E495 laptops with 20.04 installed. The problem occurred 
initially on the laptop that is having package upgrades applied regularly.
  
  With dual monitors and the external monitor placed left or right the
  display has a blocked staircase effect shown in the attached photograph,

  More detailed investigation suggests it only happens when the X
  coordinate of the two monitors is different. The symptom looks like an
  off-by-one error because it appears as if the display is divided into,
  say, 10 rows and 15 columns but the first row has 16 'columns' worth of
  blocks on it and so wraps to the beginning of the 2nd row, and so on.
  
  On the laptop without package upgrades being applied this didn't happen.
  So I upgraded it (314 packages) and restarted and it too sees the same
  problem.
  
  I suspected libxcomposite1 and downgraded it to 1:0.4.5-0ubuntu1 but
  that didn't solve it.
  
  I now suspect libxcb but so far haven't been able to prove it.

- a NULL mode.  If it's killed, it remains in the mode it was previously

Bryan, thank you for taking the time to identify the commits that
correct the bug.  I am able to reproduce the bug, and after applying the
patches, I can also confirm that the issue is resolved.

I've merged the 4 commits you identified into a single patch (preserve-
mouse-cursor-after-vt-switch.patch) and have provided the attached
debdiff. If we can organize a significant amount of testing, can we
consider getting these changes into Xenial?

As requested I've downloaded and installed the latest available upstream
kernel files located in the v4.5-wily foler following the install notes
linked in your previous message:

linux-image-4.5.0-040500-generic_4.5.0-040500.201603140130_amd64.deb

I then rebooted and selected Advanced options for Ubuntu and Ubuntu,
with Linux 4.5.0-040500-generic.

Unfortunately the issue still remains.  The mouse buttons still function
but the cursor does not move from the centre of the screen.

Christopher,

I've just booted into my Xubuntu install to run usb-devices.  Typically
the mouse worked on this particular instance.  I ran the command anyway.

I then rebooted straight into the live version of xenial that I
downloaded last night.  The mouse was back to not working and I've run
the same command to see if there are any differences.

I downloaded the xenial-desktop-amd64 ISO last night, imaged it to USB
and have just tested running it in live mode.  Unfortunately it's
exactly the same.  The mouse buttons work, but the pointer won't move at
all.

I have a Cougar 450M gaming mouse.  I am dual booting between Windows 10
and Xubuntu.  The mouse works perfectly in Windows but fails dismally in
any flavour of Ubuntu (tried Ubuntu, Lubuntu and Xubuntu) including
during the OS installation phase.

On very rare occasion it will work when I boot into Ubuntu, but 9 out of
10 times the only functions available are the mouse buttons and scroll
wheel.

There is also a DPI adjustment button on the 450M which cycles through
the various available settings and changes colour based on the current
DPI setting.  I have noted that if you press the button once, the button
changes colour as expected, but if you continue to press the button,
nothing happens.  It seems to freeze on that setting.

Since upgrading to latest kernel (4.2.0-11-generic) multi screen setup
no longer works. It does seem intermittant, but in 9/10 cases it will
not drive triple monitor setup, however with some unplugging and
replugging of the DP one screen "may" appear which mirrors the local
display, I can then add the other monitors.

If the laptop screen blanking kicks in it never recovers.

Thanks for the response. To be honest, I'd forgotten about this bug. It was so 
weird/specific to begin with (only one website ever caused problems). The issue 
also seems to have disappeared on upgrading to 14.10. 
It never occurred to me to upgrade the BIOS - the utilities are always 
Window-specific and I'd never gotten around to learning how to do it on Ubuntu 
despite the fact I haven't run Windows since Ubuntu 7.10...

Since the issue doesn't occur on 14.10 shall I mark it as resolved?

I think I figured out the failure in the logic.  You are right, since we
are clearing the caches in that particular function, it makes sense to
make sure they are truly freed.

The above fix uncovered another issue with the order of events when the
Configure new displays when connected feature is enabled. I found that
we are clearing our caches in the middle of the screen event (thats the
leftover reference you found). I reordered the code for that function,
and now I think I should have a fully functional patch.

Please try the attached patch and let me know if the issue is still
resolved for you.

I've tested the patch, and noticed that without the patch xfsettingsd
sometimes crashes when an external display (in this case, a TV) is
unplugged.  Without the patch, xfsettingsd no longer crashes.

Seems to be a font issue: the crash always happens as the page tries to
load objects from fonts.smdg.ca. I considered blocking smdg.ca with
NoScript but the site becomes unusable. Could this be a duplicate of Bug
#738526? I always get an X crash the first time I load the page after
reboot. If it just kicks me to the login screen I can try again and it
works. A few times the crash has been bad enough that I end up at a
blinking cursor instead of the login screen and I reboot with Ctrl-Alt-
Del. If I go back to the website after a reboot, X crashes again.

I appreciate the form letter for the report.  :S

I attempted to run this on 10.04 and I get the following error: The
requested URL /+request-token was not found on this server

My guess is lucid is locked out due to lack of support...  I can run
this on the 14.04 text based install, but that's not going to show you
the difference.  What do you want me to do?  I know when I looked at
upgrading this a year ago, there was still some problems with intel and
HDA audio, so I just figued I'd wait for the next LTS release.

Let me know what you want for logs, as this is a show stopper from a
released installer on this hardware.

This doesn't occur every single time I try to open the website, but
fairly consistently on the first instance after starting the computer.
(After one xserver crash it usually works the second time.) I'm using
the pipelight plugin to enable the Windows Flash player (since most
websites will not stream shows these days using the older Linux plugin)
but this crash occurs when opening the main page, before the flash
plugin is called.

What happens: I click on the icon for the foodnetwork.ca main page on
the Firefox new tabs page, and X crashes immediately, sending me back to
the login page.

What should happen: the page should load, allowing me to click on the
'videos' link, taking me to the page where I can select the program I
want to watch.

I have never encountered this problem on any other website, but run into
it regularly on this site.

No they are not, for a while now. They had been occurring about every 6

  Apparently random logouts since upgrading to 10.11. No obvious reason
  behind them, although I have noticed that after the first in a 'set',
  if I log back in immediately after, in most occasions it repeats the
  logout within a minute or so. The Wi-fi has not reconnected by this
  time. If I wait on the login screen, then login, the wi-fi is
  connected and the problem doesn't seem to come back for some time.

Unfortunately I'm unable to test because native Raring became completely
unusable on MacBookAir3,1 due to LP Bug #576648 so I switched to running
virtualized Raring. I'll continue testing when I switch back to native.

Installing any nvidia-* driver package manually via apt-get or using
System Settings - Software  Updates - Additional Drivers results in
black/blank screen on boot on a MacBookAir3,1. No xorg.conf is getting
generated during install and I'm guessing that the video card may not be
getting properly probed on boot resulting in the black/blank screen.

After installing/purging every nvidia-* package and not having a usable
X after reboot I finally reinstalled nvidia-experimental-310 and
manually ran nvidia-xconfig (booted into X using nouveau). This finally
generated an xorg.conf and I had a working X after reboot.

I'd expect to have a working X out of the box after installing an
nvidia-* driver package; I didn't think I'd have to manually run nvidia-
xconfig.

nvidia-xconfig generated the following xorg.conf:

even after I got X working properly after running nvidia-xconfig,
brightness wasn't adjustable on my MacBookAir3,1. I had to manually edit

Christopher, is there anything further you might need me to check? I am
planning to apply a workaround this week of a GeForce 650 I got a good
deal on :-)   I'll hold off on swapping the cards in case you need me to
follow up on anything. I'm not using the Firestream for any GP-GPU
computing or any of the heavy number-crunching it's intended for so it's
a bit of a silly card to have in my machine anyway.

Christopher: installing fglrx-updates has the same effect. Graphics are 
completely shot on rebooting, and I'm unable to run aticonfig --initial or 
amdconfig --initial successfully as before.
I'm sticking with the open source drivers for now and am looking at swapping 
out this card for a supported one.

I have failure with both fglrx and fglrx-updates in 12.10, and the 12.11
beta driver fails to install in 12.04. (Although the latter may be
because I'm doing something wrong, as I noted in bug 1075035.)

I've installed 12.04, updated it, and tried installing the 12.11 beta driver 
form the link in #6. In case I did something wrong, here are my steps: 1. 
Download and extract the file. 2 In the directory of the extracted file, I ran 
sudo sh file.run
The installer program came up, but it only returns the message Your graphics 
adapter is not supported by this driver. Installation will not proceed.
I tried building an Ubuntu package for the driver but that also failed.

I haven't yet tried the workaround in bug #1058040 that involves
downgrading X.Org, but it seems like any attempts to run a newer driver
on 12.10 fail for me.

With the open-source drivers my previously whisper-quiet desktop
computer sounds like a jet engine when playing a video full-screen, so I
think I'm going to have to either go back to 12.04 or update my graphics
card. (I'd been meaning to do the latter anyway - the firestream card
isn't really appropriate for what I use it for.)

12.04 install on this machine (my home computer) and I've got a really

program that depends on qt3 - but no AMD graphics.)

I can add the AMD Firestream 9250 to the list of affected video cards -
just upgraded and I've got the same problems as everyone else in this
report. I'll try the open source drivers until I can get enough of a
working desktop to investigate further. (I'm having trouble even viewing
a website on firefox on a 24 monitor!)

Public bug reported:

I upgraded my desktop at home to 12.10 today, and wound up with complete 
failure of graphics: compiz would not work and hence Unity would not start - I 
had only a desktop and a terminal (accessed via Ctrl-Alt-T). Screen resolution 
was very poor (and used only 75% of the screen on a 24 monitor) - I couldn't 
even view the full width of a page in Firefox.
Apparently AMD is still releasing drivers for the Firestream series, so I don't 
know what went wrong in my case. I can try reinstalling fglrx to see what 
happens, or whatever suggestions might help diagnose the issue.

WORKAROUND: I've managed to get back to a working desktop by purging
everything related to fglrx and using the open source drivers.

I tried the workaround, but ran into some problems:
 - in step 5, the command apt-get -y reinstall was not recognised. The 
reinstall seems to be the wrong code. I couldn't recall what the correct 
command was, so I did this step via synaptic.
 - step 6 failed because there was no package fglrx-installer. The repository 
I added in Step 2 didn't have this package, but it did have plain ol' fglrx, 
so I tried installing that instead.
 - In the final step (7), the command aticonfig --initial returned
aticonfig: No supported modules present

Rebooting resulted in the same mess as before, so I purged fglrx again,
and rebooted with the open source drivers.

I got no problems. Try this: add i915.semaphores=1 to your kernel
options in /etc/default/grub (AND run update-grub AND reboot).  E.g:

  GRUB_CMDLINE_LINUX_DEFAULT=quiet splash elevator=noop
i915.semaphores=1


I also do created /usr/share/X11/xorg.conf.d/20-intel.conf with this in it:

  Section Device
  Identifier Intel
  Driver intel
  Option AccelMethod exa
  Option MigrationHeuristic greedy
  Option DebugWait true
  Option SwapbuffersWait false
  EndSection

^ ^ again, reboot if you do that and/or play with those options.

Bryce,

It is reproducible and happens every time I sleep/resume. It's similar
to another bug I encountered a while ago in Oneiric and for a while in
Natty Beta 1 where the background would disappear on sleep/resume but,
if I left-clicked anywhere on the Dekstop, the image would reappear.
That's not the case with this manifestation; the background disappears
until I re-select it from Appearance.

After doing testing with LP Bug #1051035 and LP Bug #1051020 I no longer
see this issue with all packages updated as of 17-Sep 21:00 UTC and
running Linux medic 3.6.0-030600rc6-generic #201209161835 SMP Sun Sep 16
22:36:28 UTC 2012 x86_64 x86_64 x86_64 GNU/Linux.

I will test this again after rebooting to the stock Quantal kernel (on
the off chance that this is actually a video corruption issue related to
the above two bugs).

After a suspend/resume my desktop background image changes from my
custom image back to black. When I go back into gnome-control-center
(Gear - System Settings... - Appearance - Look) the background image
is still set there and I have to toggle something (change from Fill to
Scale and back to Fill) to get the background image to show back up
again.

Plymouth Ubuntu logo screen. It's just garbled corruption from unflushed
video memory. Sometimes the last image on screen before reboot is

It appears that a few of the dependencies for xserver-xorg-video-all
have had their package file truncated in the 12.04 LTS 64 Bit Alternate
ISO (ubuntu-12.04-alternate-amd64.iso). This causes Ubuntu setup to fail
during the Select and install software phase giving an error message
that xserver-xorg-video-ati and xserver-xorg-video-nouveau could not be
installed.

Here are the package names from ubuntu-12.04-alternate-amd64.iso

My suggestions:

1. DebugWait / SwapbuffersWait: create the file
/usr/share/X11/xorg.conf.d/20-intel.conf with the following:

Section Device
Identifier Intel
Driver intel
Option DebugWait true
Option SwapbuffersWait false
EndSection

2. Add i915.semaphores=1 to the kernel boot command line in 
/etc/default/grub/ E.g.:
 GRUB_CMDLINE_LINUX_DEFAULT=quiet splash i915.semaphores=1 (then run 
update-grub and reboot).

3. turn off font anti-aliasing,
4. set the DPI for your screen correctly,
^^Unfortunately multiple sub-systems manageo both of those. Suggestions welcome.
