Don't use MySQL if you want to get your data back in a consistent state
reliably.

Use PostgreSQL instead.

Avoid the embedded database.

 provide very critical control of the positioning of text?

using MySQL?

  Will using Mail Merge also be a part of what the OP wants to do?

 I'd really rather be able to do this job running OO's Base and MySQL
 directly from Mac OS X, rather than starting my Windows XP Virtual
 Machine, mechanized via Parallels, so that I can use MS Access
 from my MS Office XP Professional software.

I suggest to have a look at our forums where there are many sample
databases and you can work with the people there to get you up to speed.

To create Forms and or Labels use Writer
To place data on your Form or Label go to Insert -- Fields -- Other 
(Cntr+F2) and select the Database Tab
Now select Mail Merge fields from a database and click on the table or 
view and select the field you like to insert.

Rest is logical
