Hello Mária,

OK, just had to figure out how to do this when you use the regular Keypad in
the Phone App, but here is how you do the + in both situations (when adding
a contact and when dialing a number on the keypad):

If you are adding a contact it's easy. Once you are in one of the phone
number fields, a telephone style DialPad shows up instead of the normal
virtual keyboard. Only difference is that to the left and right of the 0 you
don't have the Star and Pound, respectively, but a Shift key to the
left of the 0 and the delete key to the right of the 0.
If you want to put a + as the first character, just touch the Shift Key
(you may have to double tap or split tap on it if you are still using iOS
5). Now the 0 to the right of it turns into a +, just touch it and the + is
inserted into the field, now type your country code.

When you are in the Keypad just to dial a number once you can, of course
dial 011 followed by the country code, at least here in North America it is
011, I think in Europe in most countries you would just dial 00 followed by
the country code, e.g. 001 for Canada/the US, 0049 for Germany, 0044 for the
UK etc.
If you want to dial the +, you can turn the 0 into a + in the same way you
would bring up accented characters on the keyboard. Here is how:
1. Touch the 0 and then touch the screen with another finger, basically, do
a split-tap. This will prevent the 0 being inserted/dialed as soon as you
lift your finger. 
2. Lift your finger after you touch the screen with a second finger.
3. Do a double tap and hold. You can do this anywhere since focus is still
on the 0. You will hear the DialTone play as long as you hold down, hold
maybe for a second or so and lift. If you check the Phone Number field at
the top it should now have a + in it. Keep in mind that Voiceover does not
seem to announce the +, but if you hold for a second or so it will be
inserted.

Give this a try and let me know if it works. If dialing 00 and the country
code works for dialing directly I think that is easier than to do the split
tap on the 0, then a double tap and hold.

holding 0. I haven't figured out how to do that with Voice over.

If using iOS 6, you may need to turn punctuation to all in order to hear
those symbols.

Where you dialing from?  I'm sure you can do it all with numbers; usually 
starting with 0 - From the US to dial UK, 01144, followed by 10 of their 
digits;  never include our leading 0, that is for internal use only. But as 
far as I know,  and given this used to be done with old phones that had none 
of this digital stuff;   it had to be;  so you just need the local number 
for dialing international, then the country code, then the full number for 
the individual in that country.

I am sure you can add that sign using voice over, not sure how. But you can 
add 00 before the number in place of plus signs to make it an international 
number.
