Actually, if you get to the first setup screen you can activate voice over by hitting home button 3 times. At least that is how it has been in previous setups. I think you flick up and down until you hear it say tap to continue, or words to that effect... in English, or language of your choice, and that's the language it'll then install in. As Alex pointed out, after the initial setup a triple click of the home button does nothing until you go to aAccessibility settings and set it to toggle Voiceover on or off. It will not do so automatically once the iPhone is set up for the first time.
