Thanks for your suggestions.  On Apple support advice we reset device
settings and this restored Voiceover to normal working. Thankfully we
didn't have to do a full restore which would have been the case if
Voiceover had been corrupted in some way.

I was quite surprised how little had to be done afterwards to restore
full functionality, all the apps retained their login status.

It might sound silly, but make sure your screen is clean, I have seen strange 
things happening with a greasy or dirty screen and if you have a screensaver on 
that phone make sure it is still applied well.

I bought a Samsun Folder 2, and that happened with it, had to return it and get 
refund. I know someone in Washington State now, who has one and is enjoying it.

space on the phone which was 1.27GB, surely enough to run Voiceover?

Has anyone come across this before and know of a quick fix before I go to Apple 
support who I  expect will tell me to restore the phone.

I don't know why, but it happens to me too.
You can try using hearphone . 
After you can go settings/general/accessibility and call audio routing, hope it 
is the good words. You will have many options.

Hi,

yes, I have already tried that, too no avail.  I have spoken to Apple 
accessability who were very helpful.  It looks like a reset first and if 
that doesn't fix it then a full restore which is a bit of a pain!

This turns speech on/off but leaves Voiceover running.

You can use Voiceover with Braille without speech.
