Hi Just posted this on a similar thread.  Although I don't use Voice 
Over I've had the same problem with non-vibration since upgrading to IOS 
8.0.2.  The only way I could get the vibrate to work on silent was to go 
into Settings / Sounds / Ringtone.  Then for my current Ringtone, I set 
Vibration to one of the patterns available (initially it was set to 
'none').  Hope this helps

Hi all.  Thanks for getting back to me on this issue.  I did test it 
without voice over being on and sure enough it vibrated.  I'll have to 
call my carrier and let them know that it's a bug with voice over and 
IOS 8.  They suggested that I replace the phone but if I needed to do 
that the phone wouldn't vibrate at all so I'll take this bug as is and 
just report it to Apple and my carrier.

This may be another of the iOS 8 bugs.
I just tested my work iPhone 5 and if I have vibrate on ring on and vibrate
on silent on, I get exactly what you get.  If the phone is not muted, it
both vibrates and rings.
If I mute it, then it vibrates once and then VoiceOver says accept.
If I set the vibration to off for ring, but on for silent, then when the
phone is muted it vibrates until it gets answered or the person hangs up.
It does the same on my personal 5S.

So, I think in iOS 8, if you want it to vibrate when muted, you have to set

I am pretty sure this was not the case in iOS 7, but I could be wrong.

I have now tested the phone vibration feature. On my 5S, running iOS 8, it 
works the same as it has always worked. Vibration setting, win on, vibrates 
until the phone is answered, or the call ins. This is the case whether or not 
the mute switch is active.
