Hi Anna,

I agree that is the way it should be, but if you unlock your phone and don't do 
anything to make Voiceover speak and then repeatedly press the volume down 
button both system sounds and Voiceover decrease in volume, you can turn it all 
the way down where you don't hear anything and also turn it up again.

You are correct, I never noticed that because when I tried it this morning 
Voiceover started reading when I pressed the home button to wake up the phone 
and then it worked.
If nothing is reading it doesn't work, I guess you could probably submit this 
as a bug, I personally consider this such a small issue that it doesn't bother 
me.

Sieghard, have you tried it when Voiceover was not reading anything? I
have also experienced this issue. Not a huge deal but annoying when I
want to change the volume and need to also have voiceOver speak.

Just tried it while Voiceover was reading some notifications on the lock screen 
and the volume buttons do work for me on my iPhone 6S Plus on the lock screen.
