I had gone to JJ hospital, Mumbai in mid Novemberfor getting my disability
certificate for the first time.
While all the medical tests were completed, they said that the government
had changed the procedure and they were waiting for the updated process

The form is bilingual. You can use NVDA for best results. There is no
substitute of captcha.
