We recently hit similar issues when upgrading from 5.1 to 5.3.2.
For some reason it reads the standard .sys or .opt file you have to
change the =generate to =prompt.

Had someone mounting the tape in a drive manually and went through all devices
until we found the right ones, we got there in the end.

the syntax we would run for checking in new tapes to be labelled
LABEL LIBVOLUME 3583LIB CHECKIN=SCRATCH SEARCH=BULK LABELSOURCE=BARCODE

We have found if you are using 100mb network for example
you have to ensure that all interfaces are set to the same speed etc eg:
Host 100mb/Full switch port 100mb/Full and backup server 100mb/Full

We have had a similar problem with a ATL library for our main frame
recently which we are currently upgrading.
The best carts to remove we found were carts that were 100% full or
close to this percentage and mark these carts as unavailable.

This has worked for me in the past similar crcumstance.
To Restore a node file for example that is no longer backed up by TSM
*Create a new dsm.opt changeing the node name to afs200 file eg
dsm.opt.afs200 then change the original to dsm.opt.old and change the
afs200 one to dsm.opt.
