I don't know how to explain the causes, but a reset on the drive
worked for me.. :)

We had a problem similar to yours. We had a problem when we started our
TSM server up our drives failed. So I checked the /dev folder and
checked to see that the tape drives were there. What happened was our
device path had changed, and the new path was /dev/IBMtape0n when it
originally was /dev
