What do you mean by using a Windows box? Does it mean gparted disk format
tool of GUI Linux?

Appears that your MMC is in bad shape.
Try reformatting the card. If you still get the errors, try a
different card. If you are using Linux, I found that using a Windows
box to format it initially worked for me, then used 'fdisk' on my
Linux machine to format again as mmcblk0p1=fat and mmcblk0p2
