If the server runs fine (for, say, 15-30 min after starting up), back up all
escalations to a .DEF, delete them, stop ar server, enable escalations, restart

I don't think you want to do enableLookups
If you did this -- you may give your DNS server a huge pounding...
(Maybe it all gets cached -- so I could be wrong -- however -- it reminded me
of a bug I found a long time ago where the DNS server was getting sent a lot of
unnecessary requests)
Also setting the network card settings to full duplex if it is not already set
to that, will help grease up the system a bit.. Do this to both the Database
server as well as the AR Server..

You need to do one import per form type.
As for mandatory fields, first of all, manually create an entry on the form,
and see which ones it demands (not many), then you'll need the data set id I
mentioned below.

1) Create/Open the CSV and sanitise the data.
