From what I understand is that Remedy licenses go by the Host ID is is
basically the serial number of the network card in the server. However
usually purging the old licenses and getting new ones with your new Host
ID usually fixes the problem.

Anybody know where I can find the names of the fields missing? I too noticed
that some of the fields are not showing up in the Vendor form, however I don't
know what the field names are to add them manually.

Check your password on your LDAP configuration form. Chances are the
password is different there then it is on the server you are trying to
connect to.
